BACKGROUND: Targeted donor screening for strongyloidiasis performed at the time of organ procurement can prevent this life-threatening donor-derived infection.
METHOD: The Association of Organ Procurement Organizations surveyed members to determine the number of US organ procurement organizations (OPOs) performing donor screening for Strongyloides infection and their screening practices.
RESULTS: All 58 OPOs responded to the survey. Only 6 (10%) currently screen donors for strongyloidiasis; most OPOs started 6-36 months before the survey and one started 6 years prior. All used risk-based cri…

Immigrants from undeveloped countries are a growing problem in Europe. Spain has become a frequent destination for immigrants (20% of whom a … children, respectively. The most frequent causes of absolute eosinophilia were filariasis (52.6%), strongyloidiasis (46.8%) and schistosomiasis (28.9%). Filariasis (41.9%), strongyloidiasis (29.6%) and schistosomiasis (22.2%) were the most frequent causes of increased levels of IgE. The area under the ROC curve showed similar values between eosinophil count and I …

A 77-year-old man with idiopathic hypereosinophilic syndrome on corticosteroids presented with fever, diarrhea, and anemia. Initial investigations, including screening for enteric pathogens, were negative, and empirical antibiotics were initiated, with subsequent deterioration. Endoscopy revealed Strongyloides stercoralis in the duodenal wall. Treatment with ivermectin resulted in clinical and hematologic resolution. This case highlights the need to consider strongyloidiasis in the differential diagnosis of peripheral eosinophilia.

We report the successful treatment of an HIV-infected patient with progressive strongyloidiasis as a component of immune reconstitution disease and a review of the literature on this topic. In our experience, pre- and post-antiretroviral therapy intestinal biopsies support a novel mechanism of immune reconstitution disease to Strongyloides stercoralis. We conclude that extended, dual antihelminthic therapy and temporary discontinuation of antiretroviral therapy may be effective in similar patients.

Family physicians in the United States are increasingly called on to manage the complex clinical problems of newly arrived immigrants and refugees. Case studies and discussions are provided in this article to update physicians on the diagnosis and management of potentially unfamiliar ailments, including strongyloidiasis, hookworm infection, cysticercosis, clonorchiasis and tropical pancreatitis. Albendazole and ivermectin, two important drugs in the treatment of some worm infections, are now available in the United States.

Recurrent urticaria, abdominal pain, peripheral eosinophilia, hyperimmunoglobulin E and small bowel abnormalities on roentgenograms prompted evaluation of a 36 year old man for intestinal parasitism. Evidence for helminthiasis as the cause of his symptoms included changing antibody titers to helminth antigens and a dramatic response to therapy. Helminth larvae or eggs were not recovered. Diagnosis of strongyloidiasis, the most likely cause of the illness, is discussed.

This case describes a 62-year-old male who underwent kidney transplantation and was admitted 8 weeks post-operatively for evaluation and management of abdominal pain, nausea, and diarrhea. An extensive workup was performed, ultimately leading to a diagnosis of Strongyloides hyperinfection syndrome. This case highlights the importance of awareness of strongyloidiasis in non-endemic regions, management of Strongyloides hyperinfection syndrome in the post-transplant setting, and strategies for donor and recipient screening.

In summary, it appears that giardiasis, coccidiosis, cryptosporidiosis, strongyloidiasis, capillariasis and perhaps P. falciparum malaria are the only parasitic diseases which cause malabsorption of many nutrients. D. latum and A. lumbricoides interfere with vitamin B12 and vitamin A absorption, respectively. In view of the increasing use of immunosuppressive therapy, it is likely that malabsorption caused by intestinal parasites may become even more evident in the future.

Underdiagnosis of chronic infection with the nematode Strongyloides stercoralis may lead to severe disease in the immunosuppressed. Thus, we have set-up a specific and highly sensitive molecular diagnosis in stool samples. Here, we compared the accuracy of our polymerase chain reaction (PCR)-based method with that of conventional diagnostic methods for chronic infection. We also analyzed clinical and epidemiological predictors of infection to propose an algorithm for the diagnosis of strongyloidiasis useful for the clinician. Molecular and gold standard methods were performed to evaluate a coh…

Strongyloides hyperinfection syndrome is a rare but serious and often fatal complication of strongyloidiasis, usually precipitated by immune suppression (frequently caused by corticosteroidal drugs). Infections with Strongyloides stercoralis are known to occur in British veterans who served in South-east Asia during the Second World War, particularly in those held as prisoners of war by the Japanese. No information on the frequency of disseminated strongyloidiasis in these men is, however, available. A multi-source enquiry designed to detect the cases of Strongyloides hyperinfection syndrome t…

Strongyloides stercoralis can cause chronic infections that may escalate into life-threatening hyperinfection, especially in immunocompromised patients. Severe intestinal strongyloidiasis has been associated with bacteraemia and sepsis, classically due to translocation of enteric organisms across a disrupted intestinal mucosal barrier. We report a patient with lepromatous leprosy on steroids who developed Strongyloides hyperinfection presenting with ulcerative duodenitis and septic shock.
