The underlying pathophysiology of ictal dystonia in temporal lobe epilepsy: an FDG-PET study.

BACKGROUND: Unilateral dystonic posturing of limbs occurs frequently in medial temporal lobe epilepsy (MTLE) but its underlying anatomic basis is still unknown. OBJECTIVE: To investigate the pathophysiology of dystonic posturing, we examined the relation between the occurrence of dystonia and interictal cerebral metabolic abnormalities using fluorodeoxyglucose (FDG)-PET in MTLE patients. METHODS: Video recordings of 30 patients with documented MTLE were reviewed to assess the presence of ictal dystonic posturing. Interictal FDG-PET was performed in all patients. RESULTS: Eighteen patients e…

Disrupted pattern of D2 dopamine receptors in the temporal lobe in schizophrenia. A postmortem study.

BACKGROUND: Anatomical substrates for the clinical efficacy of D2 dopamine receptor antagonism in ameliorating positive symptoms, including auditory hallucinations, in schizophrenia are not fully known. We previously identified a modular organization of D2 receptors unique to the temporal lobe. The dense bands of D2 receptors showed highest frequency in auditory and speech association cortices (Brodmann areas 22, 39, and 42) and auditory-visual association areas (Brodmann areas 20 and 37) but were rarely found in somatosensory association regions (Brodmann area 21). Since the anatomical locali…

[Clinical manifestations in complex partial crisis of the temporal lobe: a video-electroencephalographic study].

We analysed 27 complex partial seizures arising from the temporal lobes recorded on videotape simultaneously with the EEG emphasizing the motor manifestations specially dystonic posturing, ictal paresis and head and eye forced deviation (version). The temporal lobe origin of the seizures was based on the agreement of many scalp-sphenoidal or zygomatic interictal and ictal EEG recordings, CT and MRI findings, interictal and, in some patients, ictal SPECT studies. 8 patients had surgery. In 5 from 7 patients who had temporal lobectomy, mesial temporal sclerosis was the anatomopathological findin…

[A case report of giant epidermoid in the temporal lobe: considerations of the occurrence of intramedullary epidermoids].

A case of giant epidermoid which was mainly located in the right temporal lobe was presented. The intracranial epidermoid is commonly located in the CSF space, such as the cerebello-pontine (C-P) angle, parasellar region, middle cranial fossa and ventricles. The intramedullary epidermoid is extremely rare. We report the case of a 51-year-old female suffering from generalized convulsions. Having fallen into a semicoma due to the convulsions, she was referred to our department on August 5, 1992. Neurological examination revealed slight dementia but no other deficits. The tumor was mainly located…

Surgical approach to arteriovenous malformation of the medial temporal lobe--report of three cases.

We report three cases of arteriovenous malformation (AVM) of the medial temporal lobe and the surgical approaches used. The AVM was fed by the anterior choroidal artery (AChA) in two cases (Cases 1 and 2) and by the posterior cerebral artery in one (Case 3). The trans-Sylvian approach was first used for cerebrospinal fluid aspiration to retract the brain in all cases, and for confirming the feeding arteries to prevent premature bleeding from the AVM in Cases 1 and 2. In Case 1, a corticotomy was then made in the fusiform gyrus via the subtemporal approach to avoid the development of speech dis…

Factorial validity of a neuropsychological test battery and its ability to discern temporal lobe epilepsy from frontal lobe epilepsy - A retrospective study.

PURPOSE: Firstly, to evaluate the validity of a neuropsychological test battery in epilepsy patients, i.e. whether its tests sufficiently al … component analysis (PCA). To further examine its accuracy in two matched subgroups of patients with temporal lobe epilepsy (TLE, n = 35) and frontal lobe epilepsy (FLE, n = 35), a discriminant function analysis (DFA) was used. RESULTS: PCA revealed eleven interpretable factors, accounting for 69.1% of total variance: Divided Attention, Reaction Time, Verbal Le …

Achieving detailed medial temporal lobe segmentation with upsampled isotropic training from implicit neural representation.

Imaging biomarkers in magnetic resonance imaging (MRI) are important tools for diagnosing, tracking and treating Alzheimer's disease (AD). Neurofibrillary tau pathology in AD is closely linked to neurodegeneration and generally follows a pattern of spread in the brain, with early stages involving subregions of the medial temporal lobe (MTL). Accurate segmentation of MTL subregions is needed to extract granular biomarkers of AD progression. MTL subregions are often imaged using T2-weighted (T2w) MRI scans that are highly anisotropic due to constraints of MRI physics and image acquisition, makin…

Divergent Propagation Networks Define Two Pathophysiologically Distinct Electroclinical Phenotypes in Mesial Temporal Lobe Epilepsy.

BACKGROUND AND PURPOSE: Temporal dissociation between electrographic and clinical seizure onset in mesial temporal lobe epilepsy (MTLE) implies network heterogeneity. We hypothesized that MTLE consists of distinct electroclinical phenotypes defined by divergent seizure propagation dynamics. METHODS: We retrospectively analyzed 167 seizures from 13 patients with drug-resistant MTLE who achieved Engel Class I surgical outcomes. Based on the predominant relationship between scalp electroencephalography (EEG) and clinical onset, patients were classified as EEG-first (n=7) or Clinical-first (n=6).…

Brain connectivity signatures of cognitive impairment in temporal lobe epilepsy identified by robotic assessment.

BACKGROUND: Subjects with temporal lobe epilepsy (TLE) often experience cognitive impairment in different domains. Currently, the mechanisms underlying neuropsychological dysfunction in TLE remain poorly understood. The main objective is to characterize the multivariate relationship between brain connectivity patterns and cognitive impairment detected by robotic testing in subjects with TLE. METHODS: Kinarm robotic technology was used to evaluate motor, cognitive, and sensory domains of healthy controls and individuals with TLE. Structural connectivity (SC) and functional connectivity (FC) we…

EEG microstate analysis in temporal lobe epilepsy: a comparative study stratified by surgical outcome.

OBJECTIVE: This study investigated electroencephalogram (EEG) microstate characteristics in patients with temporal lobe epilepsy (TLE) following functional anterior temporal lobectomy (FATL), aiming to identify postoperative alterations in microstate dynamics specifically associated with seizure-free outcomes and to characterize baseline microstate features that may assist in the selection of optimal candidates for TLE surgery. METHODS: A total of 44 TLE patients were included in the study, comprising 27 seizure-free (SF) and 17 non-seizure-free (nSF) individuals. For each patient, a 2 min EE…

Metabolic Brain Network Reorganization in Temporal Lobe Epilepsy With Aware or Impaired Awareness Seizures.

BACKGROUND: Temporal lobe epilepsy (TLE) is increasingly recognized as a dysfunction of the brain network. However, the topological alterations of the metabolic brain network underlying impaired awareness seizures (IAS) remain unclear. In this study, we aimed to characterize metabolic network reorganization in patients with TLE and IAS (TLE-IAS) and to preliminarily investigate the specificity of these alterations by comparing the results with those of patients with TLE and aware seizures (AS). METHODS: This retrospective study included a total of 193 TLE-IAS patients, 30 patients with TLE-AS…

Cross-species transcriptomic analysis of rodent model fidelity to human mesial temporal lobe epilepsy.

Mesial temporal lobe epilepsy (MTLE) is a common form of drug-resistant epilepsy, necessitating reliable rodent models for translational research. Although numerous rodent models reproduce hippocampal epileptogenesis, their molecular fidelity to human MTLE remains unclear. In this study, we conducted an integrative analysis of human MTLE transcriptomic data and evaluated rodent epilepsy models by comparing their transcriptomic profiles with those of human MTLE. By integrating 694 human samples (mean age 39.6 years, 40.3% female, 12 datasets) and 362 rodent samples (postnatal and adult mice and…

Peripheral inflammatory markers and metabolic profiles in temporal lobe epilepsy and functional dissociative seizures.

BACKGROUND: Temporal lobe epilepsy (TLE) and functional dissociative seizures (FDS) are distinct conditions that share overlapping clinical, neuropsychiatric, and biological features, including depressive symptoms, obesity-related metabolic dysregulation, and alterations in systemic inflammatory markers. Understanding the interplay among these factors may provide insight into their shared biological burden. OBJECTIVE: This study aimed to investigate the relationships among systemic inflammatory markers, adipokines, obesity, seizure frequency, and depressive symptom burden in patients with dru…

Evidence for early, clinically silent medial temporal lobe inflammation after CAR-T cell therapy.

OBJECTIVE: Immune effector cell-associated neurotoxicity syndrome (ICANS) is the most frequent neurological complication following chimeric antigen receptor (CAR)-T cell therapy. Long-term cognitive effects remain controversial, even in the absence of clinical neurotoxicity. CAR-T cell-associated neurotoxicity often involves the medial temporal lobe (MTL), yet conventional structural magnetic resonance imaging (MRI) frequently remains normal. There is a need for markers of subclinical neurotoxicity that may predict long-term cognitive outcomes. METHODS: Therefore, we established an imaging pr…
