Treatment of tendinous mallet finger deformity with a part of the flexor digitorum profundus tendon.

BACKGROUND: To introduce and evaluate an updated surgical technique for the treatment of tendinous mallet finger deformity. METHODS: From April 2017 to September 2018, 13 cases of tendinous mallet finger deformity were treated. All patients had zone I extensor tendon rupture, with no residual tendon at the insertion for suture, and no avulsion fracture in the distal phalanx. Extensor tendon insertion reconstruction was realized by suturing the transferred portion of the flexor digitorum profundus tendon with the proximal end of the extensor tendon via a constructed bone tunnel. The treatment

Effect of unintentional partial Achilles tenotomy on Ponseti clubfoot management outcomes.

The congenital talipes equinovarus (clubfoot) is one of the most common congenital conditions requiring orthopedic intervention. This study was carried out to evaluate the effect of unintentional partial Achilles tendon cut during percutaneous tenotomy on the success rate of Ponseti management. Percentage of Achilles tendon cut following percutaneous tenotomy was measured by ultrasound in 16 clubfeet (11 patients). After a mean of 21 months of follow-up, the final results (Dimeglio score) were compared between two groups of patients: those with complete tendon cut and those with partial cut. C

Surgical Strategies for Functional Upper Extremity Reconstruction After Spinal Cord Injury.

Spinal cord injuries (SCI) can substantially affect independence and quality of life, particularly by limiting upper extremity function. Surgical reconstruction offers the potential to restore motion in the hand, wrist, and elbow for those with deficits following cervical spinal cord injury. Techniques such as tendon transfer, tenodesis, and arthrodesis-often used in combination-are well-established strategies for enhancing upper extremity function. Nerve transfers have more recently been employed and differ from other procedures in that they are often time sensitive and should be performed be…

Using three-dimensional gait data for foot/ankle orthopaedic surgery.

We present the case of a forty year old male who sustained a torn carotid during strenuous physical activity. This was followed by a right hemispheric stroke due to a clot associated with the carotid. Upon recovery, the patient's gait was characterized as hemiparetic with a stiff-knee pattern, a fixed flexion deformity of the toe flexors, and a hindfoot varus. Based on clinical exams and radiographs, the surgical treatment plan was established and consisted of correction of the forefoot deformities, possible hamstrings lengthening, and tendon transfer of the posterior tibial tendon to the dors…

Primary End-to-End Repair of Subacute Extensor Hallucis Longus Tendon Rupture Supported by a Preserved Extensor Hallucis Capsularis: A Case Report.

Extensor hallucis longus (EHL) tendon rupture is an uncommon injury that usually results from trauma and often requires surgical intervention. In the subacute phase, primary tendon repair becomes challenging because of tendon retraction, and tendon transfer or graft reconstruction is often necessary. Anatomical variations of the EHL, particularly the extensor hallucis capsularis (EHC), may influence tendon retraction and reparability, although their clinical significance has rarely been reported. We present a rare case of subacute EHL rupture in which primary end-to-end repair was feasible owi…

Surgical reconstructions for adult brachial plexus injuries. Part II: Treatments for total arm type.

Brachial plexus injuries (BPI) contribute not only to physical dysfunction but also to socioeconomic aspects and psychological disability. Patients with total arm-type BPI will lose not only the shoulder and elbow function but also the hand function, making reconstruction particularly challenging. Reconstructive procedures commonly include nerve repair, grafting, neurotization (nerve transfer), tendon transfer and free functional muscle transfer (FFMT). Although it is difficult to achieve prehensile hand function, most of patients with total arm-type BPI can be treated with satisfied outcomes.…

Arthroscopic Subscapularis Augmentation Using the Long Head of the Biceps Tendon for Anterior Shoulder Instability.

The limitations of transferring the coracoid process along with the conjoined tendon are coracoacromial arch damage, technical difficulty, and nerve injury. The long head of the biceps tendon (LHBT) proximal transposition technique has a weaker sling effect and a risk of nerve injury. The arthroscopic subscapularis augmentation technique may have risks of shoulder external rotation restriction and subscapularis transection. Herein, we introduce an arthroscopic technique for the transfer of the LHBT for subscapularis augmentation to address these risks. Indications of this technique were patien

Tibialis anterior and extensor hallucis longus tendon ruptures complicating a closed tibial fracture: A case report.

INTRODUCTION AND IMPORTANCE: Acute rupture of the tibialis anterior tendon and other tendons is an extremely rare injury. It is usually associated with minor trauma in older patients with medical comorbidity. Surgeons must be alert for rupture of these tendons which can complicate a closed tibial fracture. CASE PRESENTATION: A 19-year-old man was transferred to the emergency department with a segmental fracture at the lower third of his tibia. The patient could not actively dorsiflex his right ankle and first toe but was able to dorsiflex other toes actively. The tibialis anterior and extenso

Recognizing Posterior Accessory Muscles During Posterior Ankle Arthroscopy: A Case Report.

The flexor hallucis longus (FHL) tendon is a key anatomical landmark in posterior ankle arthroscopy and a commonly used graft for chronic Achilles tendon rupture. Peroneocalcaneus internus (PCI) is a rare accessory muscle that is frequently overlooked due to its close anatomical relationship to FHL and its asymptomatic presence. Failure to recognize this variation may result in tendon misidentification, ineffective reconstruction, or neurovascular injury. We report a 72-year-old woman with a chronic Achilles tendon rupture treated with endoscopic FHL transfer. Preoperative MRI demonstrated an
