In 17 patients with idiopathic thrombocytopenic purpura unresponsive to treatment with steroids, immunosuppression or splenectomy, allogenic vincristine-loaded platelets were used. In 9 of them an increase of platelet number and regression of bleeding tendency was achieved. Two female patients with grade IV primary biliary cirrhosis, untractable pruritus, arthralgias and xanthomatous neuropathy in one case, underwent long term plasmapheresis for 30 months (on a continuous centrifugation cell separator) and 18 months (on capillary plasma filters). Nine people with cystic fibrosis (pwCF) were found to have isolated elevations in serum total bilirubin after starting elexacaftor/tezacaftor/ivacaftor (ETI) that were associated with Gilbert's Syndrome. In longitudinal examination, total bilirubin levels increased substantially after initiation of ETI without elevations in liver transaminases in those with this syndrome. CASE: An 11-year-old girl presented with fever and jaundice after arrival from India. Her blood culture was positive for Salmonella typhi. While on treatment with ceftriaxone, she had worsening of her jaundice with abrupt elevation of liver transaminases associated with coagulopathy.
