The Economic Impact of Loss to Follow-Up in Eosinophilic Esophagitis: A Model-Based Analysis from the Perspective of Italian National Health Service.

PURPOSE: Eosinophilic esophagitis (EoE) is a chronic immune-mediated disease requiring long-term management. Loss to follow-up (LTFU) is a significant issue, leading to increased complications and higher healthcare costs. One key factor contributing to LTFU is the lack of structured transition models for patients moving from pediatric to adult care. This study provides the first model-based economic evaluation of LTFU in EoE within the Italian National Healthcare Service (NHS) and aims to quantify the economic impact of LTFU as well as evaluate the potential cost reduction associated with impl

Psychological and medical care of gender nonconforming youth.

Gender nonconforming (GN) children and adolescents, collectively referred to as GN youth, may seek care to understand their internal gender identities, socially transition to their affirmed genders, and/or physically transition to their affirmed genders. Because general pediatricians are often the first point of contact with the health care system for GN youth, familiarity with the psychological and medical approaches to providing care for this population is crucial. The objective of this review is to provide an overview of existing clinical practice guidelines for GN youth. Such guidelines em

"I feel like a guest on the other side": youth-identified challenges and solutions to transitioning from adolescent to adult HIV services in Western Kenya.

Adolescents and young adults living with HIV (AYAH) face significant challenges when transitioning from adolescent to adult HIV services. Up to 50% of AYAH do not remain engaged in care after transition, which may result in virological failure. We sought to understand challenges to clinic transitions and youth-identified solutions to optimize health outcomes. We conducted three focus group discussions (FGDs) (n = 33) and a 4-day participatory, human-centered design (HCD) workshop as part of this qualitative sub study. AYAH recruited for FGDs between October and November 2020 were from health f

Neonatal integrated home care: nursing without walls.

A Neonatal Integrated Home Care Program was developed to cross-train NICU staff nurses to provide follow-up care for high-risk neonates in the home. Implementation required collaboration of the NICU and the Center for Home Care and Hospice. Initial target populations were premature infants in transition to oral feeds and oxygen-dependent neonates. Staff, parent, and insurer enthusiasm for this innovative program resulted in expansion to serve infants and families with many other care needs. Continuity of care provides an invaluable resource for families at home with their high-risk infants. Ou

Comorbidity, Pain, Utilization, and Psychosocial Outcomes in Older versus Younger Sickle Cell Adults: The PiSCES Project.

Background. Patients with SCD now usually live well into adulthood. Whereas transitions into adulthood are now often studied, little is published about aging beyond the transition period. We therefore studied age-associated SCD differences in utilization, pain, and psychosocial variables. Methods. Subjects were 232 adults in the Pain in Sickle Cell Epidemiology Study (PiSCES). Data included demographics, comorbidity, and psychosocial measures. SCD-related pain and health care utilization were recorded in diaries. We compared 3 age groups: 16-25 (transition), 26-36 (younger adults), and 37-64 (

Adult outcomes and lifespan issues for people with childhood-onset physical disability.

This paper aimed to discuss functioning, quality of life, (QoL) and lifespan care issues of adolescents and young adults with childhood-onset physical disability from a clinical, scientific, and personal perspective. We present a résumé of results of recently performed studies in rehabilitation-based samples of (young) adults with childhood-onset conditions such as cerebral palsy (CP) and spina bifida (SB), and different models of transition and lifespan care. The studies showed that many young adults with a childhood-onset disability experience health-related problems such as functional deter

[Transition in patients with cystic fibrosis and asthma].

Adolescence represents a period of significant transformation for individuals with chronic conditions such as cystic fibrosis (CF) and asthma. Due to substantial progress in highly effective cystic fibrosis transmembrane conductance regulator (CFTR) modulator therapy, nearly everyone with CF now reaches adulthood, necessitating ongoing interdisciplinary medical care. Conversely, approximately 70% of children with asthma experience a resolution of symptoms as they reach adulthood. Factors contributing to the persistence of asthma into adulthood include the presence of severe asthma, diminished

The transition from pre- to early pregnancy for women with type 2 diabetes - a qualitative study.

BACKGROUND: Pregnant women with type 2 diabetes and their offspring face an increased risk of obstetric complications, necessitating major changes in diabetes self-care during pregnancy. These changes include the initiation of insulin therapy, frequent blood glucose monitoring, carbohydrate counting and regular hospital visits. For many women, this transition is challenging and emotionally demanding. To improve care and support during this vulnerable period, gaining a deeper understanding of these women's experiences is essential. AIM: To explore how women with type 2 diabetes experience the

Navigating the Transition From Adolescence to Adulthood Among Young People With Severe Haemophilia: The Qualitative Phase of the TRANSHEMO Project.

INTRODUCTION: Haemophilia causes spontaneous or prolonged bleeding due to a deficiency in clotting factor VIII (haemophilia A) or IX (haemophilia B). Although substitutive therapies and regular follow-up can prevent severe haemorrhagic events, adherence to treatment remains a challenge. Transitioning from adolescence to adulthood and from paediatric to adult care is particularly complex for young people with severe haemophilia (PwSH), as it involves gaining autonomy in health management. OBJECTIVES: This study aimed to explore factors influencing the success of the transition process in young

Burden of disease and clinical targets in adult patients with X-linked hypophosphatemia. A comprehensive review.

UNLABELLED: X-linked hypophosphataemia (XLH) is a lifelong condition. Despite the mounting clinical evidence highlighting the long-term multi-organ sequelae of chronic phosphate wasting and consequent hypophosphatemia over the lifetime and the morbidities associated with adult age, XLH is still perceived as a paediatric disease. INTRODUCTION: Children who have XLH need to transition from paediatric to adult healthcare as young adults. While there is general agreement that all affected children should be treated (if the administration and tolerability of therapy can be adequately monitored), t

Implementation of an educational intervention to optimize self-management and transition readiness in young adults with sickle cell disease.

BACKGROUND: The transition from the pediatric setting to adult care is a well-described period of morbidity and mortality for persons with sickle cell disease (SCD). We sought to measure the feasibility and effectiveness of providing skill-based educational handouts on improving self-management and transition readiness in adolescents with SCD. METHODS: This was a single-center study in which participants completed a self-assessment, the Adolescent Autonomy Checklist (AAC), to assess transition readiness and self-management skills at baseline. After results were reviewed by the study coordinat

Transition of Intravenous Treprostinil to Oral Therapy in a Patient with Functional Class IV Chronic Thromboembolic Pulmonary Hypertension.

Chronic thromboembolic pulmonary hypertension (CTEPH) occurs when pulmonary emboli fail to resolve with anticoagulation. For patients with inoperable or residual CTEPH, riociguat is currently the only therapy approved by the United States Food and Drug Administration. However, some patients with CTEPH may require therapy beyond riociguat, such as intravenous prostacyclins, which can present significant administration challenges in patients with complex comorbid conditions. We describe a 42-year-old man with T12 paraplegia complicated by CTEPH (functional class IV with substantial right ventric

Current practices and experience of transition of young people on long term home parenteral nutrition (PN) to adult services - A perspective from specialist centres.

BACKGROUND: There has been an estimated fivefold increase in the number of children receiving parenteral nutrition (PN) at home in the past 10 years with some children approaching the age when they should be referred to adult services whilst still on treatment. Models of care for moving young people onto adult providers of PN at home are not yet well developed, and transition is a potentially dangerous time for young people with complex health needs. METHODS: A questionnaire to ascertain current experience and practices of transition in the context of home PN services was dispatched to 170 co

Tuberous sclerosis complex: Concerns and needs of patients and parents from the transitional period to adulthood.

INTRODUCTION: Transitioning into adulthood and from pediatric services to adult healthcare are both challenging processes for young adults with rare chronic disorders such as tuberous sclerosis complex (TSC) and their parents. Adult healthcare systems are often less family-oriented and lack multidisciplinary care and experience with TSC, which can result in increased health risks and morbidity. Patient-driven data on care needs are necessary to optimize support for this vulnerable patient group. AIM: The aim of this study was to explore the concerns and care needs of young adult patients with
