Global surgery in a postconflict setting--5-year results of implementation in the Russian North Caucasus.

BACKGROUND: Collaborations for global surgery face many challenges to achieve fair and safe patient care and to build sustainable capacity. The 2004 terrorist attack on a school in Beslan in North Ossetia in the Russian North Caucasus left many victims with complex otologic barotrauma. In response, we implemented a global surgery partnership between the Vladikavkaz Children's Hospital, international surgical teams, the North Ossetian Health Ministry, and civil society organizations. This study's aim was to describe the implementation and 5-year results of capacity building for complex surgery

Intracranial pressure response after pharmacologic treatment of intracranial hypertension.

BACKGROUND: The accepted treatment of increased intracranial pressure (ICP) in patients experiencing severe traumatic brain injury is multimodal and algorithmic, obscuring individual effects of treatment. Using continuous vital signs monitoring, we sought to measure treatment effect and ascertain the accuracy of manual data recording. METHODS: Patients older than 17 years, admitted and requiring ICP monitoring between 2008 and 2010 at a high-volume urban trauma center, were retrospectively evaluated. Timing and dose of ICP-directed therapy were recorded from paper and electronic medical recor

The dangers of being a "weekend warrior": a new call for injury prevention efforts.

BACKGROUND: Nonprofessionals routinely perform high-risk home maintenance activities otherwise regulated by the Occupational Health and Safety Administration when professionals perform the same work. Reducing the risks taken by these "weekend warriors" has not been the focus of injury prevention efforts. This study describes injury patterns and outcomes for nonprofessionals attempting home roof and tree maintenance. METHODS: We queried our trauma registry for all adult patients (age, ≥18 years) with injury codes for "fall-from-height" or "struck-by-tree" (2005-present) and reviewed charts to

Fecal incontinence: a clinical approach.

"Fecal incontinence" is defined as the involuntary loss of stool at any time of life after toilet training. It is a socially and psychologically devastating condition for patients and their families, and a topic which both patients and physicians are reluctant to approach. Although the true prevalence of fecal incontinence is unknown, studies have reported it to be as high as 2. 2% in the general population, with significantly higher rates among nursing home residents and hospitalized elderly. Risk factors include advancing age, female gender and multiparity. An understanding of pelvic floor a

Blood Pressure Variability and Outcome Predictors for Traumatic Brain Injury Patients with Diffuse Axonal Injury: A Retrospective Cohort Study.

BACKGROUND: Diffuse axonal injury (DAI), a feature seen in severe traumatic brain injury (TBI), is associated with substantial morbidity and mortality. Although blood pressure variability (BPV) has been shown to impact TBI outcomes overall, its relevance in DAI cases remains uncertain. We investigated whether 24-hour post-injury BPV and other clinical factors were linked to patient outcomes. METHODS: We conducted a retrospective analysis of Level I trauma center-admitted TBI patients with radiographic DAI diagnosis (computed tomography/magnetic resonance imaging). Hospital disposition (home,

Impact of a 'Catheter Bundle' on Infection Rates and Economic Costs in the Intensive Care Unit: A Retrospective Cohort Study.

INTRODUCTION: Catheter-related infections (CBRSIs) are a widespread problem that increase morbidity and mortality in intensive care unit (ICU) patients and management costs. OBJECTIVE: The main aim of this study was to assess the prevalence of CBRSIs in an intensive care unit following international literature guidelines for managing vascular lines in critically ill patients. These guidelines include changing vascular lines every 7 days, using needle-free devices and port protectors, standardising closed infusion lines, employing chlorhexidine-impregnated dressings, and utilising sutureless d

A Critical Review on the Relevance of Paracetamol for Procedural Pain Management in Neonates.

Effective and safe pain relief in neonates matters. This is not only because of ethical constraints or human empathy, but even more because pain treatment is an important and crucial part of contemporary medical, paramedical, and nursing care to improve the outcome in neonatal intensive care graduates. Paracetamol (acetaminophen) is likely one of the pharmacological tools to attain this, with data on prescription practices suggesting that paracetamol is somehow the "rising star" in neonatal pain management. Besides very rare topical clinical scenarios like peripartal asphyxia and subsequent wh

What role does laparoscopy play in the diagnosis and immediate treatment of infants with necrotizing enterocolitis?

PURPOSE: Laparotomy is the preferred option for the surgical treatment of necrotizing enterocolitis (NEC); however, laparoscopy may offer benefit, particularly when it is uncertain if the operation is necessary. Our aim was to review the current evidence for the use of diagnostic and therapeutic laparoscopy in NEC. MATERIALS AND METHODS: A systematic review of published literature (1990-July 2012) found in MEDLINE, EMBASE, the Cochrane Library Controlled Trials Register, and the Cumulative Index to Nursing and Allied Health reporting the use of laparoscopy for management of acute NEC was unde

Isolated cervical spine fractures in the elderly: a deadly injury.

BACKGROUND: Traumatic injury in the elderly is an increasing problem and studies have shown that elderly patients (>/=65 years old) with cervical spine fractures and spinal cord injury (SCI) carry a mortality rate of 21% to 30%. However, little has been described with regard to outcomes for elderly patients with isolated cervical spine fractures (ICSF). HYPOTHESIS: Outcomes for elderly patients with ICSF will be similar to elderly patients with cervical fractures and associated traumatic injuries (ATI) or SCI. METHODS: A 9-year retrospective analysis was performed on all patients >/=65 years

Tacks: a new technique for craniofacial fixation.

Biodegradable fixation in craniofacial surgery provides secure fixation while eliminating much of the concern over intracranial migration of metallic plates and screws. One limitation of present biodegradable systems, however, is the need for tapping the drill hole before screw insertion. Herein, a new method of rigid, biodegradable fixation with tacks (Macrapore, Inc., San Diego, CA) is described. The tacks are made of a 70:30 ratio of the L and DL form of polylactic acid (L,DL-PLA). Degradation times range from 18 to 36 months. Newer prototypes are nearly developed for more rapid dissolution

Barriers and facilitators to transitional care in older adults following hip fracture: A mixed-methods systematic review.

BACKGROUND: Hip fracture is a leading cause of disability and mortality in older adults, affecting an estimated 2.6 million people annually worldwide. Transitional care, the coordinated management of the shift from hospital to community, is central to functional recovery yet remains poorly systematised across health systems. AIM: To identify the barriers and facilitators to hospital-to-community transitional care in older adults (≥65 years) following hip fracture surgery. METHODS: A mixed-methods systematic review following the Joanna Briggs Institute (JBI) convergent integrated approach. Fo

Trends in Perioperative Urinary Catheter Utilization by Surgical Procedure Type in the United States, 2010-2017: Evidence From the Nationwide Inpatient Sample.

BACKGROUND: Urinary catheters are frequently used in perioperative care, yet their overuse contributes to iatrogenic complications including catheter-associated urinary tract infections (CAUTIs) and urethral trauma. In response, the Centers for Disease Control and Prevention (CDC) issued guidelines in 2009 to minimize inappropriate perioperative catheter use. This study evaluates national trends in perioperative indwelling urethral catheter utilization across various surgical specialties from 2010 to 2017 using the Nationwide Inpatient Sample database (NIS). METHODS: We conducted a retrospect
