High mortality rate associated with KPC-producing Enterobacter cloacae in a Brazilian hospital.

We describe a clonal dissemination of KPC-producing Enterobacter cloacae in a Brazilian hospital. Patients diagnosed with theses isolates showed high mortality rate (41.8%) and were associated with previous use of antibiotics and urinary catheterization. Therefore, infection control measures and use of stricter antibiotic policies are required to control the spread of these organisms.

Candida lusitaniae: a new opportunistic pathogen of the urinary tract.

A 78-year-old man with mild diabetes presented with dysuria and frequency of urination of 15 months' duration subsequent to urinary catheterization for cataract surgery. Multiple urine specimens revealed the presence of considerable quantities of yeast that were later identified as Candida lusitaniae. The patient responded well to irrigation of the bladder with amphotericin B.

Modified Young-Dees-Leadbetter bladder neck reconstruction in patients with successful primary bladder closure elsewhere: a single institution experience.

PURPOSE: Achievement of urinary continence in patients with the exstrophy-epispadias complex remains a challenge. We reviewed our experience with the modified Young-Dees-Leadbetter bladder neck repair in patients with bladder exstrophy who underwent primary bladder closure elsewhere. MATERIALS AND METHODS: We retrospectively reviewed exstrophy charts and database of 57 male and 11 female with classic bladder exstrophy who underwent bladder neck repair at our institute and successful primary bladder closure elsewhere during the last 2 decades. Osteotomy was performed at primary closure in 14 (

Evaluation of augmentation cystoplasty in childhood with reference to vesico-ureteric reflux and urinary infection.

OBJECTIVES: To assess the non-operative treatment of vesico-ureteric reflux in patients with abnormal bladder compliance, to determine whether reduction in storage pressure by augmentation leads to cessation of reflux and whether symptomatic urinary tract infection worsens or renal function deteriorates if reflux continues after augmentation and clean intermittent catheterization (CIC). PATIENTS AND METHODS: Thirty-nine children (17 boys, 22 girls) who underwent clam ileocystoplasty over a 5-year-period were evaluated. The mean age at operation was 9.7 years (range 1.5-17.5). Pre-operative as

Intermittent self-catheterisation: teaching the skill to patients.

Intermittent self-catheterisation (ISC) is not a new concept. However, it is a method that could be more widely used by patients to drain the bladder rather than having an indwelling catheter with a drainage bag and its associated problems. The author describes the procedure, patient assessment, and the training required by patients so that they can undertake ISC. Self-catheterisation promotes patient independence, improves quality of life and helps to avoid complications associated with indwelling urinary catheters.

A Case of Pyelonephritis Caused by Ureteral Obstruction due to a Urinary Catheter.

An 88-year-old man with long-term catheterization developed sepsis and obstructive pyelonephritis due to catheter migration into the left ureter. After recovery with treatment, recurrence was revealed following routine exchange. Clinicians should suspect ureteral migration in elderly, bedridden patients with long-term urinary catheters who present with peri-catheter leakage or renal dysfunction.

Purple urine bag syndrome: An unusual presentation of urinary tract infection: A case series and literature review.

RATIONALE: Purple urine bag syndrome (PUBS) is a rare but visually alarming condition associated with urinary tract infections (UTIs), typically occurring in debilitated elderly patients with long-term indwelling catheters. Awareness of PUBS is essential, as it can serve as an indicator of underlying infection. PATIENT CONCERNS: Two elderly male patients presented with striking purple discoloration of their urinary catheter bags. Caregivers expressed concern, although both patients denied fever, dysuria, or suprapubic pain. DIAGNOSES: PUBS secondary to UTI was diagnosed. Case 1 involved mixe

Norfloxacin-releasing urethral catheter for long-term catheterization.

Norfloxacin-releasing urethral catheters were prepared for the purpose of preventing urinary tract infections during long-term catheterization. The outer and inner surfaces of the catheters were coated with poly(ethylene-co-vinyl acetate) (EVA) and an amphiphilic multiblock co-polymer (PEO2kPDMS), composed of poly(ethylene oxide) and poly(dimethyl siloxane). Norfloxacin, a fluoroquinolone synthetic antibiotic, was impregnated into a coating layer. The in vitro drug release behavior was monitored for 30 days, the surface topography was investigated using scanning electron microscopy (SEM) and t

Bacterial colonization, species diversity and antimicrobial susceptibility patterns of indwelling urinary catheters from postpartum mothers attending a Tertiary Hospital in Eastern Uganda.

BACKGROUND: Postpartum urinary Catheter-Related Infections (CRIs) are a significant cause of maternal sepsis. Several studies done have reported the presence of mixed populations of bacteria with a significant increase in Extended-Spectrum Beta-Lactamase (ESBL) Enterobacteriaceae spps, Methicillin-Resistant Staphylococcus aureus (MRSA), Multi-Drug Resistant (MDR) bacteria in urine and blood cultures of catheterized patients despite the use of prophylactic antibiotics. This study aimed at determining the bacterial species diversity and susceptibility patterns of indwelling urinary catheters fro

Preventive activity of promethazine against fluconazole-resistant Candida spp. biofilms fixed on urinary catheters.

Catheter-associated urinary tract infections (CA-UTI) are among the most prevalent infections, affecting over 150 million individuals worldwide annually. Candida is the most common fungal genus causing urinary infections, representing a significant medical challenge, particularly in patients undergoing prolonged catheterization-a risk factor that exacerbates clinical conditions by promoting the formation of resistant biofilms. In this context, the coating of catheters with antimicrobial substances combined with drug repositioning is an innovative alternative. In this study, we evaluated the ac

Psychogenic urinary retention in children: a case report.

Psychogenic urinary retention occurs relatively infrequently in children and is less common than in adults. The influence of psychogenic factors on voiding generally results in an irritative syndrome, but rarely in urinary retention. A definitive diagnosis is established by excluding other pathological conditions. Evaluation includes urine culture, renal echography, spine magnetic resonance imaging, voiding cysto-urethrography, intravenous pyelography, and uroflowmetry. Here, we report on a 6-year-old girl with a 1-month history of voiding difficulty. Urology studies, including urine culture,

Role of sacral neuromodulation in modern urogynaecology practice: a review of recent literature.

Sacral neuromodulation (SNM) offers promise in the therapy of many pelvic floor disorders. This innovative treatment has slowly gained popularity. A review of recent literature is presented in relation to its efficacy and complications in various pelvic floor conditions: overactive bladder and urge urinary incontinence, chronic urinary retention, painful bladder syndrome, pelvic pain and double incontinence. It is a minimally invasive, completely reversible safe procedure with good long-term outcomes. However, the treatment is costly, the revision rate is high and patients require life-long fo

Urodynamic changes after endoscopic correction of vesicoureteral reflux.

Massive bilateral vesicoureteral reflux (VUR) in a 7-year-old girl with spinal scoliosis was successfully treated by endoscopic correction. She was admitted due to a febrile urinary tract infection and urinary incontinence. A cystometrogram demonstrated normal detrusor function during storage. The endoscopic subureteric injection of polytetrafluoroethylene (Teflon) was performed, resulting in the disappearance of the VUR. A postoperative cystometrogram demonstrated overactive detrusor function during storage, necessitating anticholinergic medication. She has been free of febrile urinary tract

Effect of promethazine against Staphylococcus aureus and its preventive action in the formation of biofilms on silicone catheters.

Urinary infections caused by Staphylococcus aureus are commonly associated with urinary catheterization and often result in severe complications. Given this problem, the objective of the study was to investigate the preventive action of promethazine (PMT) against the formation of methicillin-resistant Staphylococcus aureus (MRSA) biofilms when impregnated in urinary catheters. For this purpose, techniques such as broth microdilution, checkerboard, impregnation on urinary catheter fragments, flow cytometry assays and scanning electron microscopy were employed. PMT exhibited antimicrobial activi…
