OBJECTIVE: To evaluate the prospective effect of applying midline partial glossectomy with Uvulopalatopharyngoplasty in the treatment of OSAHS.
METHOD: The data of 40 patients with OSAHS operated with midline partial glossectomy with UPPP were analyzed.
RESULT: The follow-up polysomnography (6-month follow-up) revealed that the mean apnea and hypopnea index decreased from 62.39+/-21.68 to 18.60+/-5.01 (P<0.01), and the mean minimal oxygen saturation (SaO2) increased from 64.40%+/-14.96% to 88.39%+/-9.21% (P<0.01). Herbal medicine is a growing alternative for established medicine. Many plants and herbs are currently in use for a myriad of diseases and symptoms. However, there are many reports in the literature of life-threatening adverse effects of these drugs. We report a 39 years old male, that consulted for pain in the nostrils and severe nasal obstruction, that appeared two hours after instilling Ecballium elaterirum in the nostrils. On physical examination, uvular edema was observed. The patient was successfully managed with intravenous betametasone and chlorphenamine.

Laser-assisted uvulopalatoplasty (LAUP) enlarges the oropharyngeal airway by reshaping the uvula, soft palate, and tonsillar pillars. Cephalometry is useful as a screening test for anatomical abnormalities in patients having obstructive sleep apnea syndrome. Pseudomona vasculitis is an exceptional disease. Only a few cases have been reported, non with oropharyngeal involvement. The case of a 30-year-old, HIV-positive man who suddenly developed septicemia and necrotizing lesions with tissue destruction of the oropharynx is reported. Histological study confirmed vasculitis. Pseudomona aeruginosa was isolated in peripheral blood and in the biopsy of the palatal lesion. Antibiotic treatment produced satisfactory results.

Uvulopalatopharyngoplasty (U3P) has been advocated for treatment of snoring and sleep apnea/hypopnea syndrome (SAHS), but often it does not effect a cure, so that other therapy (CPAP) is often required. Velopharyngeal function, voice and speech changes following uvulopalatopharyngoplasty (UPPP) were studied prospectively and compared with those in an age and sex matched group undergoing tonsillectomy. The fundamental frequency of voice was significantly lower in those undergoing UPPP. All the other changes studied were not significantly different in the two groups.

The paper deals with the polysomnographic issues from 7 patients diagnosed of obstructive sleeping illness and treated with an UPPP. Tracings were recorded after 3 and 12 months. The comparison between pre- and postoperative findings showed the improvement, being the higher apneic index decrease 50 per cent, among 57 per cent of the cases.

We have brought together information on 864 affected individuals in 164 families (including three new pedigrees) reported in the 137 year period since 1845 when Demarquay first described a family with what was later called van der Woude syndrome (VWS).
