[Differential diagnosis "vertigo and dizziness"].

Vertigo and dizziness are symptoms of interdisciplinary dimension. However, the differentiation and classification of vertigo syndromes also require experience and multidisciplinary knowledge. Since the clinical syndrome is subjective, a detailed analysis of the complaints underlying is required. International disease definitions are an indispensable tool in the differential diagnosis of vertigo syndromes today. With simple diagnostic tools eye movement disorders and nystagmus can be examined and assigned to specific vestibular disorders today. Screening tests (e.g. head impulse test) are now

An adequate parameter evaluating the galvanic body sway test: comparison with the caloric test in patients with vestibular schwannomas.

It has been reported that the galvanic body sway test does not correlate with the caloric test. We evaluated the galvanic body sway test in patients with vestibular schwannomas using three parameters: the angle of deviation response onset, the maximum value of the deviation response, and the area of deviation. These parameters reflect velocity, position, and locus of the centre of pressure, respectively. Among these parameters, only the angle of deviation response onset showed unilateral weakness of the response correlating with the canal paresis value, which indicates that velocity is respons

Associations Between Cognitive Performance, Depression, Anxiety, and Stress in Patients With Chronic Vestibular Dysfunction: A Cross-Sectional Analysis.

BACKGROUND: The vestibular system is critical for sensorimotor function, balance control, and gaze stability. Recent research highlights its role in cognitive functions, including attention, memory, and visuospatial skills. The integration of this system is lateralized in the brain. These disorders represent a substantial burden on healthcare systems, and a negative impact on quality of life. OBJECTIVES: Previous research has shown that increased cognitive load can alter vestibular evoked myogenic potentials (VEMPs), suggesting that vestibular function is influenced by cognitive demands. Ther

[Simple method of differential diagnosis of peripheral and central vertigo--development of diagnostic method and studies of 178 cases].

In patients who complain of vertigo or who have equilibrium disorders, it is often difficult to determine the etiology of the disorder, that is to determine whether it is dependent on a peripheral or central vestibular disorder. To attempt to determine the etiology in these cases, we divised a new method, the caloric eye tracking pattern test (CETP-Test). Seventeen normal subjects and 161 patients were tested. The latter group included 33 with peripheral disorders such as Meniere's disease, benign paroxysmal positional nystagmus, and others, and 128 with central disorders such as vertebral bas

Vertigo and the enlarged vestibular aqueduct syndrome.

An enlarged vestibular aqueduct (EVA) is one of the most commonly identified inner ear bony malformations in children with sensorineural hearing loss of unknown cause. Most previous reports have focused on hearing loss, but individuals with EVA may also experience paroxysmal vertigo lasting minutes to hours. We report the clinical vestibular features and vestibular function testing of two children and one adult with EVA who had a history of sensorineural hearing loss and presented to our Neurotology Clinic for the evaluation of episodic vertigo. All the patients had an antecedent history of pr

A Case of Bilateral Temporal Bone Fractures Treated With Cochlear Implantation and Customized Vestibular Rehabilitation.

Bilateral temporal bone fractures (TBFs) involving the otic capsule are uncommon, but can result in simultaneous severe auditory and vestibular dysfunction. We present the case of a 59 year-old male patient who experienced bilateral hearing loss and vertigo following a backward fall from a height of 2 m. Temporal bone computed tomography scans revealed bilateral fractures involving the otic capsule with hemorrhage in both mastoid cavities. Audiologic evaluation confirmed bilateral profound sensorineural hearing loss, and vestibular testing indicated bilateral vestibular hypofunction. The patie

Unilateral vestibular loss impairs external space representation.

The vestibular system is responsible for a wide range of postural and oculomotor functions and maintains an internal, updated representation of the position and movement of the head in space. In this study, we assessed whether unilateral vestibular loss affects external space representation. Patients with Menière's disease and healthy participants were instructed to point to memorized targets in near (peripersonal) and far (extrapersonal) spaces in the absence or presence of a visual background. These individuals were also required to estimate their body pointing direction. Menière's disease p

The skull vibration-induced nystagmus test: A useful vestibular screening test in children with hearing loss.

INTRODUCTION: Skull-Vibration-Induced-Nystagmus Test (SVINT), a non-invasive first line examination test, stimulates both otolith and canal structures and shows instantaneously a vestibular asymmetry. This study aimed to analyze the SVINT results observed in children with hearing loss (HL) amplified with hearing aids (HA) or unilateral cochlear implant (uCI) and healthy children. MATERIAL AND METHODS: This case-control study compared the results of SVINT, caloric test (CaT) and video head-impulse-test (VHIT) in 120 controls to 30 children with HA and 30 with uCI, aged 5-18 years old. [The eye, vision and vertigo].

Although ocular alterations alone rarely cause vertigo, the ophthalmologist can play an important role in the interdisciplinary context by testing visual function. Assessment of ocular motility is the most important individual examination in the diagnostic evaluation of vertigo. Methods that analyze specific visual functions, like the dynamic visual acuity test, may play an important role in the future. In addition to neuro-ophthalmologic disorders, ocular alterations are also receiving increasing attention. They are postulated to be a key element in the development of multifactorial age-relat

The clinical significance of head-shaking nystagmus in the dizzy patient.

The clinical significance of horizontal head-shaking nystagmus (HSN) was evaluated in 85 patients who complained of dizziness and vertigo. This was done by comparison of the horizontal head-shaking test with routine rotatory and caloric vestibular testing. We found that HSN evoked by horizontal head-shaking is a highly sensitive way to detect unilateral vestibular hypofunction. Except in patients with additional central vestibular imbalance or in patients with Meniere's disease, the direction of horizontal HSN is highly significant in indicating the side of the lesion, with the fast phase beat

[Hearing disorders in Paget's disease].

The audiometric data of 41 patients with Paget's disease of the bone show that hearing loss exceeds the age-related presbycusis in patients with deformities of the temporal bone as demonstrated by X-ray (27%). The usual type of hearing loss is a sensorineural one. 80% of these patients showed hair cell damage, 32% of them a retrocochlear lesion, depending on the degree of deformity of the temporal bone. Conductive hearing loss of the otosclerosis type was found in only 20%. The Paget-related hearing loss can be regarded as a consequence of the deformation occurring both at the bone surrounding

[Bilateral caloric long-term irrigation as a method for the differentiation of tonal tinnitus].

In 307 subjects with tinnitus aurium, the parameters frequency, loudness, and masking levels did not correlate significantly with the basic diseases. As a new possibility in differentiation between tinnitus of central or peripheral origin, bilateral caloric long-term irrigation of the ear canal is presented. Using warm water leads to an increase, cold water to a decrease in the loudness of tinnitus. Such results were found in cases of Menière's disease and noise-induced hearing loss. An explanation of this phenomenon lies in the hypothesis of tinnitus genesis established by Tonndorf.

[Head trauma with resultant irreversible deafness on one side and reversible hearing impairment on the other side. A case report].

Following a severe head trauma, a 27-year-old man immediately suffered from bilateral hearing loss and impaired balance. On examination there was a complete loss of hearing on the right side. On the left side a subtotal hearing loss of sensorineural origin occurred and became total after a few days. The vestibular function was absent on both sides. Following treatment with prednisolone, hearing recovered in the left ear and reached an average tone threshold of 55 dB. The vestibular function remained lost from the onset of the trauma. At follow up two years later the subtotal hearing loss in th

Air as the caloric stimulus.

The use of air as the caloric medium was investigated and compared to that of water. The ability of air and water to generate surface temperatures close to the fluid outlet temperature was studied by irrigation onto a thermocouple. Results of caloric tests performed with air and water are compared by calculating the average nystagmus response for the hot and cold stimuli for two groups of sixty patients. Also, the number of canal paresis, directional preponderance, mixed, bi-lateral canal paresis and normal responses are compared for each group. Finally, the effect of performing a bi-thermal a

Comparison of two systems for the video head impulse test (vHIT) for the lateral semicircular canal: description of results from normal and pathological subjects.

BACKGROUND: The video head impulse test (vHIT) is a recent technique for functional evaluation of semicircular canals (SSCs). The vHIT examines eye movements at high frequencies of stimulation and provides an objective assessment of the functioning of the high-frequency domain of the vestibular system. OBJECTIVE: To describe the results from vHIT performed using two systems. METHODS: All subjects were evaluated through an audiological and otoneurological battery of tests and were diagnosed as normal or abnormal by an otorhinolaryngologist. The results from two systems: 1. ICS Impulse (Otomet
