[Changes in the compound action potential in patients with acoustic neuroma].

Thirty-eight patients with proven acoustic neuroma were divided into three groups: 13 patients with normal threshold on pure tone audiogram, 15 patients with a hearing loss of 40 dB-80 dB (HL) in the low and middle frequencies, and 10 totally or almost deaf patients. Results of the first two groups are reported, using intra-tympanic electrodes with alternating clicks of 90 dB (nHL) intensity and a stimulus rate of 20/s. A total of 500 sweeps were averaged. Widening of the CAP complex by a mean of 1.6 ms, compared to 1.1 ms in normal hearing subjects, was the most striking finding. Summation po

Acoustic nerve conduction abnormalities in Guillain-Barré syndrome.

We recorded brainstem auditory evoked potentials (BAEPs) in two patients with Guillain-Barré syndrome (GBS). One patient was acutely deaf with total absence of BAEP waveforms indicative of acoustic nerve conduction block. Hearing improved during early convalescence, and there were prolonged wave I latencies. Normal BAEPs were recorded on recovery. A second patient had bilaterally prolonged wave I latencies. These BAEP findings suggest that acoustic nerve conduction abnormalities from demyelination may occur in GBS.

[Calcium antagonists and damage to the organ of Corti in acoustic trauma].

The protective effect of Ca2+ antagonists in acoustic trauma was evaluated by morphometric analysis of the guinea pig cochlea. Damage to the outer and inner hair cells was significantly reduced. Ca2+ antagonists may exert a beneficial influence on the cellular membrane of the sensory cell or on the cochlear vasculature if given in advance and in the early post-traumatic phase.

Interaction of age and intersignal interval on acoustic reflex amplitude.

We studied the effect of intersignal interval (ISI) on the averaged acoustic reflex in nine young and nine elderly subjects. Over the range of ISIs from 1.5 to 9 sec, there was an interaction between ISI and age group, but the effect was unique to reflex mode. Results emphasize the importance of measuring both crossed and uncrossed reflex waveforms, in the study of aging, and of controlling the ISI in order to avoid overestimation of aging effects.

The treatment of acoustic neuroma and Menière's disease in the same patient.

Although total excision is accepted as the standard of care in the treatment of acoustic neuromas, for the elderly patient this approach is modified. Small tumors which cause only eighth nerve symptoms of hearing loss and tinnitus can be followed radiologically while larger tumors with brain stem compression may be subtotally excised through the translabyrinthine route. Our standard treatment for Menière's disease--that is refractory to medical management--has been vestibular nerve section through a retrolabyrinthine approach. In the event of bilateral Menière's disease or Menière's disease in

Hearing loss in the cervico-oculo-acoustic (Wildervanck) syndrome.

A 22-year-old woman with mixed hearing loss, Klippel-Feil anomalad (fused cervical vertebrae), and bilateral abducens palsy with retracted bulb (Duane's syndrome) is classified as having the cervico-oculo-acoustic syndrome or Wildervanck syndrome. In this syndrome, the unilateral or bilateral congenital hearing loss is usually severe and of neurosensory origin. To our knowledge, this is the third case of this syndrome with the classical triad and a mixed hearing loss. It is the second case in which an exploratory tympanotomy was performed. An ankylosis of the stapes was found. Stapedectomy and

An acoustic analysis of the effects of surgical therapy on voice quality.

An automated computer system was used to measure changes in vocal quality resulting from laryngeal surgery. Five acoustic parameters were computed and seven listeners made judgments on vowel samples recorded before and after surgery from 37 patients. Presurgery and postsurgery changes in the five acoustic measures were compared with the perception of change in vocal harshness. A multiple regression analysis showed that four of the five acoustic measures accounted for a large amount of the variance in the judgment of change of voice quality following surgery. The measures may provide a quick an

Quantitated determination of proteins in perilymph in patients with acoustic neuromas.

Perilymph, cerebrospinal fluid (CSF) and plasma from 19 patients with acoustic neuromas were examined for albumin, alpha 2-macroglobulin and IgG. Patients with otosclerosis were used as controls. A highly significant increase of the tested proteins was found in perilymph from patients with acoustic neuromas. Determination of the tau-transferrin, which is a specific CNS protein, could neither be demonstrated in plasma nor in perilymph. This is in favour of the assumption that perilymph is plasma-derived and not concentrated CSF. Various causes for the high protein concentrations are discussed,

[Effect of acoustic stress on morphology of rat sensorimotor cortex].

The structure and ultrastructure of the sensomotor cortex in pubertal rats were studied 7, 14 and 21 days after acoustic stress. Focal peripheral chromatolysis and pigment accumulation by lysosomes were found in neurons after 14 and 21 days of exposure to noise. The number of lysosomes in which pigment was accumulated, and multivesicular bodies appeared, increased in the glial cells. The cavities of granular reticulum widened because of the accumulation by them of muddy secretion. These phenomena indicate the diffusive character of structural changes in the cortex of the large hemispheres unde

Acoustic reflex crossover artifacts in infants and young children.

Crossover artifacts occur when the probe microphone of an impedance instrument directly responds to vibratory-acoustic energy that crosses the skull from the stimulus transducer on the contralateral ear. The resulting meter deflections are subjectively similar to and may be interpreted as actual acoustic reflexes (ARs) by unsuspecting examiners. In this study, infants and young children aged 10 days to 6 years 9 months were fit with earplugs to eliminate the AR and were examined for artifact incidence and threshold. Results indicate a high rate of occurrence of the crossover artifact in the yo

[Sphygmo-acoustic concordance. A valuable sign in cardiovascular examinations].

It is described an oscilloscopic-auscultatory sign named sphygmoaccoustic concordance (S-AC) which means that the changes of intensity of a cardiac or arterial murmur is concordant with the changes in the development of a pulse observed in a mechanical oscilloscope in an artery located near the site of murmur's origin. Sphygmo-accoustic discordance (S-AD) means incongruency of these phenomena. The cardiac murmurs was auscultated at the same time that the oscillation of the carotid artery was observed in a mechanical oscillometer, modify by the author, and the arterial murmur was examined durin

Acoustic and perceptual comparison of chronic and incipient spastic dysphonia.

Vocal symptoms of patients with chronic and incipient spastic dysphonia were compared on a number of acoustic and perceptual parameters. Patients with incipient spastic dysphonia displayed less severe strain-strangle phonation, effort, rhythm, and stress. Harshness was the only perceptual parameter on which incipient spastic dysphonics were rated higher than chronic spastic dysphonics. Changes in the acoustic measures of laryngealization, harmonic change, mean vowel duration, and duration ratio between stressed and unstressed vowels accompanied changes in listeners' perception of strain-strang

Acoustic trauma effects from doubling the exposure time.

Guinea pigs were used to test acoustic trauma effects from doubling the exposure time. Previously, two frequencies (20 kHz and 4 kHz) were used in conjunction with each other (ie double exposure) to see whether a prior exposure of the cochlea to one frequency would alter the extent of damage caused by a secondary frequency, when compared with damage caused by either frequency alone. A reduced area of damage was attained after double exposures. To complete the series, the same two frequencies were used separately, but the exposure time was doubled. The results from the 20 kHz series surprisingl
