Alcohol use disorders.

Alcohol use disorders consist of conditions characterised by compulsive heavy alcohol use and loss of control over alcohol intake. Alcohol use disorders are some of the most prevalent mental disorders globally, with higher prevalence in high-income countries and lower prevalence in low-income countries. The recent COVID-19 pandemic was associated with an increase in fully alcohol-attributable mortality, in part triggered by alcohol-specific interactions with stress. Despite their high prevalence, alcohol use disorders remain undertreated, even though there are scientifically established and co

Is there a beneficial dose of alcohol?

Souček M. Is there a beneficial dose of alcohol? Alcohol consumption has a significant impact on overall health burden and is one of the main causes of morbidity and premature mortality. On average, 10 liters of pure alcohol per capita are consumed annually in the Czech Republic. Based on recent studies, we are moving away from the concept of a beneficial dose of alcohol, and a minimal dose, ideally none, is recommended. This particularly applies to oncological diseases and liver diseases. From the perspective of cardiovascular diseases, the maximum tolerable dose is reported to be 20-30 g of

Alcohol and the cardiovascular system.

Mlíková Seidlerová J. Alcohol and the cardiovascular system The consumption of alcohol leads to a variety of direct and indirect effects on the cardiovascular system, as evidenced by numerous epidemiological studies on this topic. The effect of alcohol on the development of cardiovascular complications is influenced not only by the amount of alcohol consumed but also by the drinking pattern (binge drinking vs. regular consumption of moderate amounts of alcohol). This applies to the relationship between alcohol consumption and the rise in blood pressure or the development of arterial hypertensi

Alcohol and life expectancy.

The recent three leading risk factors for global disease burden in the world are, in descending order, high blood pressure, tobacco smoking including second-hand smoke, and alcohol use. Alcohol use increases the risk for many acute and chronic health consequences including cancer, road injury and suicide as well as alcohol use disorder. It is known that there is a U- or J-shaped relationship between alcohol consumption and all-cause mortality. The descending leg of the curve showing this relationship is best explained by a decrease in the risk of cardiovascular disease, especially ischemic hea

Alcohol and Cardiovascular Disease.

Alcohol's impact on cardiovascular health is biphasic: low-to-moderate intake may appear protective, but excessive or binge drinking causes significant harm. This review examines mechanisms linking overconsumption to cardiovascular disease. Acute heavy drinking can trigger "holiday heart syndrome," a transient atrial arrhythmia from electrophysiological instability, autonomic imbalance, and electrolyte shifts. Chronic excess contributes to alcoholic cardiomyopathy via oxidative stress, mitochondrial dysfunction, and impaired calcium handling. Alcohol also promotes atrial fibrillation and hyper

Alcoholic cardiomyopathy: an update.

Alcohol-induced cardiomyopathy (AC) is an acquired form of dilated cardiomyopathy (DCM) caused by prolonged and heavy alcohol intake in the absence of other causes. The amount of alcohol required to produce AC is generally considered as >80 g/day over 5 years, but there is still some controversy regarding this definition. This review on AC focuses on pathogenesis, which involves different mechanisms. Firstly, the direct toxic effect of ethanol promotes oxidative stress in the myocardium and activation of the renin-angiotensin system. Moreover, acetaldehyde, the best-studied metabolite of alcoh

Alcohol-related dementia.

Dementia is one of the leading causes of death both in the UK and worldwide. Approximately 1 million people have been diagnosed with this condition in the UK. Although there are many types of dementia, this article will focus on alcohol-related dementia. Alcohol has become a leading cause of death in the 50-69-year age group in England, and with consumption rising rapidly, there is an increased risk that young and middle-aged people will develop alcohol-related dementia in the future. The aim of this article is to review the evidence base and discuss whether alcohol-related dementia is a sub-c

[Alcohol and older women].

Frequently linked to social isolation (widowhood, estrangement from family and friends, etc.), alcoholism in elderly women is often invisible, with a high level of dependence. It potentiates the vulnerabilities and impact of aging. What approach to care and support can geriatric teams offer? What are the care objectives?

Infections in Alcoholic Hepatitis.

Severe alcoholic hepatitis (sAH) is defined by a modified discriminant function ≥32 or model for end-stage liver disease (MELD) >20. Patients with sAH are in an immunocompromised state attributed to cirrhosis-related immunoparesis and corticosteroid use. Individuals with sAH often develop severe infections that adversely impact short-term prognosis. Currently, the corticosteroid prednisolone is the only treatment with proven efficacy in sAH; however, the combination of corticosteroid treatment and altered host defense in sAH has been thought to increase the risk of acquiring of bacterial, oppo

Alcohol-Associated Cirrhosis.

Alcohol-associated cirrhosis (AC) contributes up to 50% of the overall cirrhosis burden in the United States. AC is typically a comorbid condition in association with alcohol-use disorder. AC is often coexistent with other conditions. Several noninvasive methods are available to assist in recognizing the presence of AC. The natural history of AC is governed by the patients continued drinking or abstinence. All treatment starts with abstinence. After decompensation, the progression to acute-on-chronic liver failure heralds death. When patients who have deteriorated are declined liver transplant

Alcoholic Hepatitis: A Review.

Alcoholic liver disease (ALD) represents a spectrum of injury, ranging from simple steatosis to alcoholic hepatitis to cirrhosis. Regular alcohol use results in fatty changes in the liver which can develop into inflammation, fibrosis and ultimately cirrhosis with continued, excessive drinking. Alcoholic hepatitis (AH) is an acute hepatic inflammation associated with significant morbidity and mortality that can occur in patients with steatosis or underlying cirrhosis. The pathogenesis of ALD is multifactorial and in addition to genetic factors, alcohol-induced hepatocyte damage, reactive oxygen

Acute Alcoholic Hepatitis.

Alcoholic hepatitis is a unique type of alcohol-associated liver disease characterized by acute liver inflammation caused by prolonged heavy alcohol use. Treatment is mostly supportive. The short-term prognosis of acute alcoholic hepatitis depends on liver recovery, and ranges widely from rapid improvement to grim multiorgan failure despite treatment. Refinement of scoring systems have enhanced prognostication to guide clinical decision making in alcoholic hepatitis. Recent advances in the treatment of alcoholic hepatitis have solidified corticosteroids as the cornerstone of treatment to enhan

Cycling injuries and alcohol.

BACKGROUND: Most of the cycling accidents that occur in Finland do not end up in the official traffic accident statistics. Thus, there is minimal information on these accidents and their consequences, particularly in cases in which alcohol was involved. The focus of the present study is on cycling accidents and injuries involving alcohol in particular. METHODS: Data on patients visiting the emergency department at North Kymi Hospital because of a cycling accident was prospectively collected for two years, from June 1, 2004 to May 31, 2006. Blood alcohol concentration (BAC) was measured on adm

[Alcoholic ketoacidosis – a review].

Alcoholic ketoacidosis - a review A chronic alcoholic with severe metabolic acidosis presents a difficult diagnostic problem in the emergency room. Over and above methanol- and ethylene glycol intoxication, alcoholic ketoacidosis is a common but less recognized etiology. The disorder occurs in alcoholics who have had a recent binge drinking followed by the abrupt cessation of alcohol consumption because of abdominal pain and vomiting, with resulting dehydration, starvation, and then a β-hydroxybutyrate dominated ketoacidosis. Laboratory results may be misleading as the common urine-ketone test…

Baclofen for alcohol withdrawal.

BACKGROUND: Baclofen shows potential for rapidly reducing symptoms of severe alcohol withdrawal syndrome (AWS) in people with alcoholism. Treatment with baclofen is easy to manage and rarely produces euphoria or other pleasant effects, or craving for the drug. This is an updated version of the original Cochrane Review published in 2015, Issue 4. OBJECTIVES: To assess the efficacy and safety of baclofen for people with AWS. SEARCH METHODS: We updated our searches of the following databases to March 2017: the Cochrane Drugs and Alcohol Group Specialised Register, CENTRAL, PubMed, Embase, and C
