[Anaphylactic shock caused by suxamethonium (Curacit)].

We describe five cases of anaphylactic shock following the short-acting muscle relaxant succinylcholine (suxamethonium). All the patients recovered uneventfully. Skin prick testing identified succinylcholine as the probable causative agent in four cases. The incidence of anaphylaxis to succinylcholine in our material appears to be 1:1,000. Treatment consisted of volume substitution, adequate oxygenation and adrenergic agents. We discuss the routine use of succinylcholine for intubation in elective surgery, and conclude that the anaesthesiologist must always be prepared for a possible anaphylac

Propofol as an intravenous anaesthetic agent in dogs.

Studies in dogs with an emulsion formulation of the intravenous anaesthetic, propofol, showed that induction of anaesthesia was smooth and it was possible to maintain anaesthesia by intermittent injection. The mean dose for induction of anaesthesia in unpremedicated dogs was 5.95 mg/kg body-weight. When no premedication was administered anaesthesia was maintained by a total dose of approximately 0.806 mg/kg/minute. Premedication with between 0.02 and 0.04 mg/kg of acepromazine reduced the mean induction dose by about 30 per cent and the maintenance dose by more than 50 per cent. In 68 unpremed

Present status and practice of electroconvulsive therapy at the Psychiatric Hospital, Enugu, Nigeria.

During the 12-month period, 1 January 1978 to 31 December 1978, 690 people received 4,194 electroconvulsive treatments at the Psychiatric Hospital, Enugu. The diagnostic classifications of patients receiving this form of treatment included severe psychotic depression (and masked depression), catatonic schizophrenia (with marked withdrawal, mutism, or excitement); mania which has become unresponsive to major tranquilizers, some selected cases of acute delirium and the collective group "puerperal psychosis". Bilateral ECT was generally used and the frequency of treatment was two ECT's per week.

Esketamine and neurocognitive disorders in adult surgical patients: a meta-analysis.

BACKGROUND: Prior meta-analyses have established the potential of intravenous ketamine in safeguarding against neurocognitive impairment, but the efficacy of intravenous esketamine for the prevention of perioperative neurocognitive disorders (PND) remains uncertain. The primary aim of this meta-analysis was to conduct a comprehensive evaluation of the effects of esketamine on PND in adult surgical patients undergoing general anesthesia. METHODS: We searched several electronic databases and clinical trial registries to find relevant trials. Randomized controlled trials of perioperative use of

Infectious Thoracoabdominal Aortic Aneurysm Repair in a Patient with Myasthenia Gravis: A Case Report.

Myasthenia gravis (MG) is an autoimmune neuromuscular junction disorder that rarely coexists with infectious thoracoabdominal aortic aneurysms (TAAA) requiring open repair. A 57-year-old patient with MG underwent elective thoracoabdominal aortic replacement. He was diagnosed with MG (Osserman classification II A). Extent IV thoracoabdominal aortic repair was performed under general anesthesia and maintained by total intravenous anesthesia. The patient was withdrawn from the ventilator on postoperative day 5 without spinal cord ischemia and myasthenic crisis. The management of infectious TAAA w

Chelonian Sedation and Anesthesia.

Anesthetic management of chelonians represents a unique challenge; the order Chelonia includes numerous species that display diverse anatomic features, habitats, body sizes, temperaments, and metabolic rates. Owing to their peculiar characteristics, safe and effective sedation and anesthesia may be more complicated than in other animals. For example, gas inductions are not indicated, and intravenous catheterization requires practice. The pharmacology of anesthetic drugs is severely impacted by body/environmental temperature, site of administration, and organ function. This review will summariz

Near ideal anesthetic technique for tracheal stenting in central airway obstruction with dexmedetomidine-ketamine infusion: a case report.

Central airway obstruction presents as an emergency with dyspnea and stridor. Anesthetic management of rigid bronchoscopy-guided tracheal stenting is highly stimulating procedure requiring general anesthesia. But it may lead to life threatening airway obstruction and cardiovascular collapse after induction. Total intravenous anesthesia based on propofol-remifentanil is an optimal anesthetic technique, but remifentanil is not available in many countries. Although dexmedetomidine-ketamine has been used for procedural sedation, its use for rigid bronchoscopy in the setting of central airway obstr

Complex Glycerol Kinase Deficiency (Xp21 Deletion Syndrome): A Case Report of a Contiguous Gene Disorder Necessitating Creative Anesthetic Planning.

We report a case of Xp21 deletion syndrome, a contiguous gene syndrome associating glycerol kinase deficiency, Duchenne muscular dystrophy, and congenital adrenal hypoplasia. This results in a contraindication to the use of all halogenated agents and of propofol. We used regional anesthesia combined with dexmedetomidine and ketamine. Previously, the patient had received inadvertently a propofol-based total intravenous anesthesia (TIVA) with no clinical side effects. We were unfortunately unable to document the metabolic consequences of this glycerol load. We suggest that if propofol is deemed

Not Everything That Shakes Is a Seizure: A Case Report.

Not everything that shakes is an epileptic seizure. We present a patient who repeatedly exhibited severe shaking at emergence from general anesthesia. Her nonepileptic myoclonus was mistaken for a refractory seizure and treated with benzodiazepines and intravenous anesthetics. The resulting depressed level of consciousness rendered myoclonus clinically indistinguishable from refractory seizures. Over the course of 6 procedures, we found that levetiracetam, a first-line antiepileptic drug, effectively suppressed her myoclonus. The episodic nature of perioperative anesthesia care presents a chal

[Anesthetic Management for a Patient with Fibromyalgia].

A 22-year-old female with fibromyalgia (FM) was scheduled for tonsillectomy under general anesthesia. Her medication included pregabalin 300 mg x day(-1) and dantrolene 50 mg x day(-1). Anesthesia was maintained with sevoflurane-remifentanil-fentanyl. Intravenous injection of fentanyl 20 μg x hr(-1) and droperidol 100 μg x hr(-1) was continued for 24 hours. On the first postoperative day, she reported that she had slept well and had no pain. There are some perioperative problems in a patient with FM. Therefore, anesthetic managements for a patient with FM is worth reporting.

Anesthesia in a patient with mucopolysaccharidosis type VI (Maroteaux-Lamy syndrome).

We report a case of anesthesia during surgery to enlarge the foramen magnum in a pediatric patient with an extremely rare form of mucopolysaccharidosis type VI (Maroteaux-Lamy syndrome). Airway control was unexpectedly easy, and intraoperative anesthetic management with total intravenous anesthesia went smoothly. However, the disease is progressive, with no guarantee that future anesthetic management of this patient will remain easy. If repeated surgery is required, thorough testing should be conducted over time to assess both airway and systemic complications. Nevertheless, we found that safe

[Anesthetic management of a patient with Bartter's syndrome].

A 48-year-old woman with Bartter's syndrome underwent right mastectomy under general anesthesia. Her operative course was uneventful. She was preoperatively complicated with severe hypokalemia but had no signs and symptoms of hypokalemia. In anesthetic care of patients with Bartter's syndrome, even when they have no symptoms of hypokalemia, the meticulous intravenous administration of potassium chloride is required in order to maintain the preoperative level of the serum potassium during anesthesia. In addition, attention should be paid to factors causing an additional reduction in the serum p
