The application of a neural network to predict hypotension and vasopressor requirements non-invasively in obstetric patients having spinal anesthesia for elective cesarean section (C/S).

BACKGROUND: Neural networks are increasingly used to assess physiological processes or pathologies, as well as to predict the increased likelihood of an impending medical crisis, such as hypotension. METHOD: We compared the capabilities of a single hidden layer neural network of 12 nodes to those of a discrete-feature discrimination approach with the goal being to predict the likelihood of a given patient developing significant hypotension under spinal anesthesia when undergoing a Cesarean section (C/S). Physiological input information was derived from a non-invasive blood pressure device (Ca

Dose-response relationship of prophylactic norepinephrine infusion for prevention of spinal anesthesia-induced hypotension during caesarean delivery: A comparative study of normal weight and obese parturients.

Obese parturients are at increased risk of spinal anesthesia-induced hypotension during cesarean delivery, but optimal prophylactic norepinephrine dosing remains unclear. In this randomized, double-blind, dose-response study, 200 parturients undergoing elective cesarean delivery were stratified into obese (BMI ≥30 kg/m²) and normal weight (BMI 18.5-24.9 kg/m²) groups. Within each group, patients received norepinephrine infusion at 0.02-0.06 µg·kg⁻¹·min⁻¹. Standardized combined spinal-epidural anesthesia with 16 mg hyperbaric ropivacaine was used, and infusion was started immediately after spin

Remifentanil-Based Monitored Anesthesia Care in a Paraplegic Patient With Severe Kyphoscoliosis, Non-palpable Spinal Landmarks, and Coagulation Abnormality Undergoing Ischiorectal Abscess Drainage: A Case Report.

Severe kyphoscoliosis with prior spinal surgery presents significant anesthetic challenges, including anticipated difficult airway, restrictive pulmonary physiology, and technical impracticality of neuraxial anesthesia. We report the anesthetic management of a 23-year-old paraplegic female patient with a massive dorsal gibbus deformity and completely non-palpable spinal landmarks who presented with recurrent ischiorectal abscess. Preoperative evaluation revealed moderate anemia, leukocytosis, mild hypokalemia, and prolonged international normalized ratio (INR). Due to inability to tolerate the

Knowledge, fear and acceptance rate of spinal anesthesia among pregnant women scheduled for cesarean section: a cross-sectional study from a tertiary care hospital in Karachi.

BACKGROUND: Worldwide, the cesarean section (c-section) rate is rising. Globally, regional anesthesia in the form of spinal anesthesia (SA) is considered the first choice in uncomplicated c-section cases for safe maternal and neonatal outcomes. This study aimed to ascertain knowledge, acceptance, and fears of SA among patients scheduled for c-section in a tertiary care hospital. METHODS: This cross-sectional study was performed in the Anesthesia Department, Hamdard University Hospital, Karachi, from April to September 2023. Knowledge was assessed using a self-designed questionnaire with a tot

Effect of crystalloid solution co-loading infusion rate on the dose requirements of prophylactic phenylephrine for preventing hypotension following combined spinal-epidural anesthesia for cesarean delivery.

BACKGROUND: Intravenous fluid administration and prophylactic vasopressor infusion are the primary methods for preventing spinal anesthesia-induced hypotension during cesarean delivery. However, evidence regarding the impact of different volumes of crystalloid solution on the phenylephrine infusion dosage for preventing this hypotension remains inconclusive. This study aimed to determine the effect of two IV fluid infusion rates (10 or 20 mL/kg/h) on phenylephrine requirement for preventing spinal anesthesia-induced hypotension. METHODS: Eighty healthy parturients undergoing elective cesarean

Background factors for intra-operative hypotension during hip fracture repair surgery in the elderly under spinal anesthesia managed by orthopedic surgeons: A retrospective case-control study.

BACKGROUND: Spinal anesthesia is used for femoral trochanteric fracture surgery, but frequently induces hypotension and the causative factors remain unclear. We examined background factors for the use of an intraoperative vasopressor in elderly patients receiving spinal anesthesia for femoral trochanteric fracture surgery. METHODS: We retrospectively analyzed 203 patients >75 years (mean age, 87.9 years) with femoral trochanteric fractures who underwent short nail fixation under orthopedically managed spinal anesthesia at our hospital between April 2020 and July 2023. Patients were divided in

Comparison of crystalloid and colloid co-load combined with norepinephrine prophylaxis on post-spinal anesthesia hypotension during cesarean delivery: a randomized sequential allocation dose-finding study.

BACKGROUND: Fluid loading is an essential component of treatment for reducing the incidence of post-spinal anesthesia hypotension and is necessary to maintain intravascular volume, perfuse tissues, and control spinal anesthesia hypotension after sympathetic blockade. We performed a randomized sequential allocation dose-finding study to compare the effects of 10 mL/kg crystalloid and 6% hydroxyethyl starch (130/0.4) co-load on the ED90 of prophylactic norepinephrine infusion for preventing post-spinal anesthesia hypotension during cesarean delivery. METHODS: Eighty patients were randomly alloc

Comparison of a Modified Regimen of Prophylactic Phenylephrine Boluses Versus Variable Rate Infusion During Elective Cesarean Delivery Under Spinal Anesthesia: A Noninferiority Randomized Double-Blind Study.

BACKGROUND: Prophylactic phenylephrine boluses have been found to be as effective as variable rate infusions during elective cesarean delivery but require a higher number of physician interventions to maintain blood pressure near baseline values. Therefore, there is a need to find a feasible regimen of bolus administration that is equally efficacious to the infusion regimen while at the same time requires a comparable number of physician interventions and is thus non-inferior to the infusion regimen. METHODS: Healthy pregnant women with term, uncomplicated, singleton pregnancies undergoing el

Association between intraoperative blood transfusion and postoperative delirium in elderly patients undergoing total hip arthroplasty with combined spinal-epidural anesthesia: an IPTW-adjusted retrospective cohort study.

BACKGROUND: Perioperative blood transfusion has been considered a potential risk factor for postoperative delirium (POD) in elderly patients. However, the association remains controversial, and prior studies have lacked a quantitative evaluation of the dose–response relationship. METHODS: This single-center retrospective cohort study included 610 patients aged 65 years or older who underwent their first total hip arthroplasty between January 2020 and April 2025. Patients were stratified into three groups according to intraoperative red blood cell transfusion volume: no transfusion, low-volume

Comparative Effects of Target-Controlled Infusion of Propofol Versus Spinal and Thiopental-Sevoflurane Anesthesia on Lipid Peroxidation in Elective Cesarean Section: A Prospective, Open-Label Study.

BACKGROUND: During pregnancy, physiological changes can increase oxidative stress (OS) in both mothers and fetuses. The use of anesthesia for cesarean sections (CSs) could exacerbate this stress due to its impact on the ischemia-reperfusion effect. Our study aimed to explore the effects of target-controlled infusion of propofol on OS during CSs, and to compare these effects with those of spinal and thiopental-sevoflurane anesthesia. METHODS: The study included ninety parturients undergoing elective CS, allocated into three groups: Group S (spinal) (n = 30), Group P (propofol) (n = 30), and Gr

Readiness for Discharge After Foot and Ankle Surgery Using Peripheral Nerve Blocks: A Randomized Controlled Trial Comparing Spinal and General Anesthesia as Supplements to Nerve Blocks.

BACKGROUND: Neuraxial anesthesia is often viewed as superior to general anesthesia but may delay discharge. Comparisons do not typically use multimodal analgesics and nerve blockade. Combining nerve blockade with general anesthesia may reduce pain, opioid consumption, and nausea. We hypothesized that general anesthesia (with nerve blocks) would lead to earlier readiness for discharge, compared to spinal anesthesia (with nerve blocks). METHODS: All patients underwent ambulatory foot and ankle surgery, with a predicted case duration of 1-3 hours. All patients received popliteal and adductor can

Failed spinal anesthesia in a pregnant patient under a "Cash-Before-Care" operating room model in a low- and middle-income country: a systems-ethics case report.

Safe anesthesia for non-obstetric surgery during pregnancy requires access to emergency drugs and airway equipment should a primary technique fail or a complication occur. In Cameroon, the "cash-before-care" model requires patients to pre-purchase specific anesthetic kits, which can significantly constrain contingency planning. A 31-year-old woman (G4P3) at 18 + 4 weeks' gestation presented for urgent wound debridement. An initial spinal anesthetic (hyperbaric bupivacaine 10 mg, fentanyl 25 μg, morphine 100 μg) failed to produce any motor or sensory block after 15 min. Although conversion to g
