[A case of moyamoya disease with a subarachnoid hemorrhage treated with endovascular technique].

We report a case of a moyamoya disease presenting with subarachonoid hemorrhage (SAH) due to a ruptured aneurysm. A 40-year-old woman presented with sudden onset of headache and vomiting. Computed tomography (CT) showed diffuse thick SAH localized around basal cistern. 3D-CT Angiography (3D-CTA) and digital subtraction angiography (DSA) demonstrated a saccular aneurysm at the bifurcation of the left superior cerebellar artery and basilar artery. In addition, the both carotid arteries were occluded at the terminal portion and the territory of both middle cerebral arteries were perfused by abnor

De-novo headache with transient vertebro-basilar symptoms: role of embryonic hypoglossal artery.

We report the case of a 56-year-old man with acute onset of de-novo stabbing, pulsating and diffuse headache with subsequent appearance (within few minutes) of posterior fossa symptoms (vomiting, postural instability, anisocoria, incoordination, dysarthria, retropulsion) lasting 9-12 h. Recurrent hypertensive crises were detected during the acute observation in the Emergency Room, even in the absence of previous history of hypertension. Once subarachnoid hemorrhage and focal lesions (vascular and non-vascular) were excluded, brain computerized tomography-angiography and digital subtraction ang

Imaging and radiological interventions of portal vein thrombosis.

Portal vein thrombosis (PVT) is diagnosed by imaging methods. Once diagnosed by means of ultrasound, Doppler ultrasound can be performed to distinguish between a benign and malignant thrombus. If further information is required, magnetic resonance angiography or contrast-enhanced computed tomography is the next step, and if these tests are unsatisfactory, digital subtraction angiography should be performed. Many papers have been published dealing with alternative methods of treating PVT, but the material is fairly heterogeneous. In symptomatic non-cavernomatous PVT, recanalization using local

Acute subarachnoid haemorrhage: 3D time-of-flight MR angiography versus intra-arterial digital angiography.

To evaluate the efficacy and reliability of 3D time-of-flight MR angiography (TOF MRA) as a noninvasive procedure, 27 patients with acute subarachnoid haemorrhage (SAH) were studied with MRA immediately before or after intra-arterial digital subtraction angiography (DSA).3DTOF MRA was performed with an axial slab of 60 mm centered on the circle of Willis and isotropic voxels. DSA showed 22 aneurysms and 1 dural arteriovenous fistula in 21 patients; the aneurysms ranged in size from 2 to 8mm. MRA failed to show 2 small aneurysms, at the origin of the posterior and anterior communicating arterie

Magnetic resonance angiography: current clinical applications.

Magnetic Resonance Angiography (MRA) is the newest non-invasive procedure for the evaluation of the vascular system. The basis to obtain images of vessels from Magnetic Resonance (MR) data rely upon the specific signal alteration determined by flowing blood. By using special techniques only these signals are evidenced, with cancelling of the surrounding stationary tissues. Clinical applications are reviewed in this paper. They include: Evaluation of intracranial vessels with important adjunct information to those obtained by conventional MR images; Definition of cerebrovascular disease includi

7T magnetic resonance angiographic imaging of basilar artery perforator aneurysms - initial experience of a non-invasive alternative to DSA.

BackgroundPerforator aneurysms of the basilar artery (PABA) are rare causes of subarachnoid haemorrhage (SAH) and challenging to diagnose. We present two cases of SAH caused by PABA diagnosed by cone beam computed tomography angiography (CBCTA) and a novel non-invasive method - 7T magnetic resonance imaging (7T MRI).MethodsTwo patients with SAH, diagnosed with PABA, were imaged on day 9 and 13 after onset, respectively, with CBCTA and 7T MR angiography (MRA) performed on the day after and at follow-up at 3 months.ResultsAll four 7T MRI examinations in the two patients were technically successf

Giant intradural cervical spine arteriovenous malformations - A case and review of literature.

INTRODUCTION: Spinal arteriovenous malformations (SAVMs) are very rare and can be very challenging to treat since none of the therapeutic options does provide a definitive cure to these lesions. We believe that incorporation of intraoperative angiography during surgery in a hybrid theatre can help achieve a better cure. CASE PRESENTATION: We present a 45 years old woman with three (3) years history of weakness and ten (10) days' history of acute pain in right upper extremity. Magnetic resonance angiography (MRA) of the cervical segment of spinal cord revealed tortuous vascular masses from for

[Gadolinium digital cavography with ultrasound guidance in renal failure: report of 2 cases].

Iodine-induced renal failure is a major concern when considering angiography in patients with renal insufficiency. Previous studies have shown that gadolinium can perform well as a radiographic contrast agent and is not nephrotoxic at doses < or = 0.4 mmol/kg. We report high-quality images of the inferior vena cava, obtained by digital subtracted venography using power-injected gadodiamide through the common femoral vein. This technique, in combination with ultrasound, appears as a good alternative for insertion of vena cava filters in patients with renal failure or those with prior episodes o

Atypical presentation of subclavian steal syndrome mimicking transient ischemic attack: successful endovascular management with angiographic correlation-a case report.

BACKGROUND: Subclavian steal syndrome is an uncommon vascular disorder caused by stenosis or occlusion of the subclavian artery proximal to the vertebral artery origin. It classically presents with vertebrobasilar symptoms such as vertigo, syncope, or diplopia; however, atypical transient neurological presentations may delay diagnosis. CASE PRESENTATION: A 71-year-old patient presented with recurrent slurred speech and left upper limb weakness without vertigo or diplopia. Examination revealed absent measurable blood pressure and absent radial pulse in the left arm, carotid bruits, and mild di

High-resolution magnetic resonance vessel wall imaging provides new insights into Moyamoya disease.

Moyamoya disease (MMD) is a rare condition that affects the blood vessels of the central nervous system. This cerebrovascular disease is characterized by progressive narrowing and blockage of the internal carotid, middle cerebral, and anterior cerebral arteries, which results in the formation of a compensatory fragile vascular network. Currently, digital subtraction angiography (DSA) is considered the gold standard in diagnosing MMD. However, this diagnostic technique is invasive and may not be suitable for all patients. Hence, non-invasive imaging methods such as computed tomography angiograp

Interventional recanalization as a treatment of carotid stump syndrome caused by right internal carotid artery occlusion: A case report.

RATIONALE: Carotid stump syndrome is a cerebral infarction caused by an embolus formed subsequent to the vortex of blood flow from the occluded stump, which then moves through the collateral vessels into the brain. The covered stent and stent-assisted coil embolization stump are the effective interventions for the carotid artery stump. PATIENT CONCERNS: A 71-year-old man twice experienced left limb weakness; diffusion weighted imaging confirmed the diagnosis of cerebral infarction. Cervical computed tomography angiography, intracranial magnetic resonance angiography, and digital subtraction a

Current optimal morphological evaluation of peripheral arterial diseases.

Imaging plays a central role in the management of peripheral arterial diseases (PAD), emphasized by the continuously increasing use of modern cross-sectional imaging and the corresponding decline of digital subtraction angiography (DSA) in the diagnostic setting. Imaging is sometimes used as a screening tool in high-risk individuals. In symptomatic patients, imaging is mandatory to make an accurate positive diagnosis, grade the severity and the extent of the abnormalities, carefully choose and plan the best therapeutic option and provide adequate follow-up. Non-invasive diagnosis of PAD can be

Peripheral arterial disease: the evolving role of non-invasive imaging.

Peripheral arterial disease is usually secondary to stenotic or occlusive atherosclerosis and is both common and increasing in western society. The majority of symptomatic patients have intermittent claudication and only a minority (<2% and typically those with diabetes mellitus or renal failure) progress to critical limb ischaemia, heralded by the onset of rest pain and/or tissue loss. Imaging is largely reserved for patients with disabling symptoms in whom revascularisation is planned. In these patients, accurate depiction of the vascular anatomy is critical for clinical decision making as t

Carotid-artery imaging in the diagnosis and management of patients at risk of stroke.

Carotid atherosclerotic disease is one of the major preventable causes of ischaemic strokes. In clinical practice, decision making with regard to carotid endarterectomy or stenting is still primarily based on the extent of luminal stenosis. In most centres worldwide, luminal stenosis is now mainly assessed with non-invasive techniques, such as Doppler ultrasound, magnetic resonance angiography, and CT angiography, either alone or in combination. Although intra-arterial digital subtraction angiography remains the historical gold standard, it has now mostly been replaced by these non-invasive te
