AIM: To assess feasibility, complications and efficacy of induction chemotherapy followed by standard chemoradiotherapy in patients with bulky anal canal cancer.
PATIENTS AND METHODS: Patients with squamous cell carcinoma of the anal canal, staged bulky tumor with or without nodal involvement were prospectively enrolled. Before standard chemoradiotherapy, patients received induction chemotherapy with 3 cycles of 75 mg/m2 cisplatin and 750 mg/m2 5-fluorouracil. Patients were followed-up routinely until recurrence or death.
RESULTS: Seven patients with bulky anal canal cancer were evaluable fo

OBJECTIVE: The bulboclitoris (clitoris and vestibular bulbs) is the primary organ responsible for female sexual arousal and orgasm. Effects of radiotherapy on the bulboclitoris are unknown, as its structure/function has yet to be described in radiotherapy, and it overlaps only partially with the external genitalia structure. Our aim was to: describe bulboclitoris structure, function and delineation; compare volume of and dose delivered to the bulboclitoris vs external genitalia; and, compare bulboclitoris-sparing IMRT (BCS-IMRT) to standard IMRT (S-IMRT) to determine reoptimization feasibility

PURPOSE: Needle aspiration of an acute simple perianal abscess may be an alternative to conventional incision drainage with potential advantages in wound healing, functional outcome, and quality of life. The aim and objectives of the study are to compare the outcome of needle aspiration and postoperative antibiotics with that of conventional surgical incision drainage of acute perianal abscess. The primary outcome was abscess recurrence. Secondary outcomes were fistula formation, wound healing, quality of life, and fecal continence.
METHODS: This is a three-center randomized controlled trial,

BACKGROUND: Anal canal carcinoma is relevant because it commonly occurs in high-risk groups, and its incidence has been increasing.
OBJECTIVE: This study evaluated the accuracy of anal cytology in the screening of precursor lesions of anal cancer, compared with histopathologic examination as the reference, in all subjects and in men who have sex with men, HIV-infected men and women, and men who have sex with men and HIV-infected subgroups.
DATA SOURCES: The data included studies identified in the MEDLINE, Latin American and Caribbean Health Sciences, Cochrane Library, and Embase electronic d

The treatment of anal cancer has evolved remarkably in the past 30 years. Definitive chemoradiotherapy is the standard of care, allowing organ preservation and maintenance of continence for most patients. This article reviews recent advances in radiotherapy planning and delivery that have resulted in improvements in treatment-related toxicity. Most notably, the advent and wide adoption of intensity-modulated radiotherapy provides a superior toxicity profile compared with older techniques, while maintaining similar oncologic outcomes. Current areas of active research include optimizing and indi

An 82-year-old man who complained of constipation, anal pain, and bleeding was diagnosed with internal hemorrhoid in another hospital 6 months previously. At the time of our consultation, a tumor was observed in the anal canal. Computed tomography (CT) imaging revealed that the tumor extended to the front of the coccyx with abscess formation. Histological examination of biopsy specimens confirmed a diagnosis of adenocarcinoma. Since the patient refused to undergo surgery, we recommended chemoradiotherapy for relieving pain. A total of 60 Gy of irradiation was administered to the lesser pelvis,

69-year-old woman underwent resection of a tumor diagnosed as squamous cell carcinoma of the anal canal. After 4 years and 5 months, computed tomography(CT)showed abdominal and pelvic lymph node swelling. Chemotherapy consisting of biweekly docetaxel(35mg/m2)+nedaplatin(35mg/m2)achieved a partial response. After 20 cycles of chemotherapy, the patient was treated with radiotherapy because of a pelvic lymph node recurrence. Since then, no recurrence has occurred for 16 months. The adverse event was Grade 2 in leucopenia and neutropenia. This case suggested that chemoradiotherapy consisting of do

The patient was a 74-year-old woman. She visited a dermatology clinic with a complaint of discomfort in the anal region. Erosion was observed in the anal region, and biopsies were performed. She was diagnosed with Bowen's disease and was referred to the dermatology department of our hospital for treatment. At our department, an endoscopic examination was performed for assessing the extent of Bowen's disease in the rectum. A retroflexed view of the anal canal revealed a slightly raised lesion with a faded color and an irregular surface. Narrow-band imaging (NBI) revealed a whitish lesion with a

This case report is on a very rare case of giant anal canal ulcer with rectal prolapse causing total faecal incontinence. An elderly patient in her eighties presented to the emergency department with complaints of a mass protruding thorough anus along with fresh rectal bleeding. Examination revealed a large ulcer measuring 6 cm x 8 cm involving the entire anal canal with rectal prolapse. Here, we discuss the management of this patient with a rare cause of giant anal canal ulceration.

A 53-year-old man, who was diagnosed as having an anal canal cancer associated with anal fistula, was introduced to our hospital. By CT and MRI examination, fluid collections were detected in the perirectal space and fistula was connected to bladder. Total pelvic exenteration was performed under the consideration of keeping the margin for the large skin defect. We reconstructed the perianal skin defect using a VY advancement of bilateral gluteus maximus musculocutaneous flaps. After the surgery, no sign of recurrence has been observed in 1 year and 4 months. This constructive technique was eff

Buschke-Löwenstein tumour is classified as a verrucous carcinoma. It presents like an exophytic tumour of the genital or peri-anal area, with ulceration and sometimes fistulae and sinuses. It is preferentially seen in men and immunocompromised patients. Histological appearance is not far from condyloma acuminata, but with a tendency to compress and displace deeper tissues, without basement membrane disruption. HPV types 6 or 11 are regularly found in association with this tumour. Other STI have to be searched. Physical examination and precise imagery are useful to chose the right treatment reg

BACKGROUND: Malignant degeneration of anal fistula is a rare occurrence in the course of a common disease. Abdominoperineal resection offers the only hope of cure.
CASE REPORT: A 41-year-old patient who was operated 14 years earlier for anal fistula presented recurrence treated by fistulectomy. Histology revealed a well-differentiated squamous-cell carcinoma. Radiotherapy (45 Gy) was added postoperatively. Five months later, local recurrence was treated by chemotherapy and abdominoperineal resection. Two months later the patient died from cachexia with local recurrence.
DISCUSSION: Biopsies

OBJECTIVE AND IMPORTANCE: We present and illustrate an unusual case of the complete familial Currarino triad (an association between a bony sacral defect, a presacral mass, and an anorectal malformation) in which the teratoma arose from the conus medullaris and contained mature neurons, glia, and branching ependymal canals that were in communication with a terminal syrinx. The embryogenic implications are discussed.
CLINICAL PRESENTATION: The patient was a term neonate when discovered to have imperforate anus. Further workup revealed lumbosacral dysraphism with a presacral mass, a rectovagina

The rectal "inflammatory cloacogenic polyp" is a recognized pathologic entity in adulthood. We report three cases in children aged between 8 and 12 years. This disorder should be included in the differential diagnosis in patients with hematochezia and tenesmus. These polyps are usually located at the anorectal junction and so may be overlooked at colonoscopy unless an endoscopic retroflexion maneuver is performed.
