In 57 aphakic second eyes of 88 patients who had previously suffered an aphakic detachment in the first eye, fifteen patients have so far suffered a retinal detachment in the second eye. However, of these fifteen patients, only two have suffered any reduction of central vision associated with detachment of the macula, reflecting an apparent change in the pattern of aphakic detachment disease in the second eye. In view of this observation, we do not think an energetic policy of prophylaxis involving 360 degrees cryotherapy should be pursued at the present time provided that follow-up facilities

Explanations are given for the physiological behavior of the cornea in the phakic and aphakic eye and for a cornea covered by a contact lens that causes epithelial anoxia. The normal, constant hydration of the cornea is a result of active ion transport by the endothelial layer. The hypoxic epithelium under a contact lens produces lactate ion that migrates to the posterior portion of the cornea where it modifies the normal stromal balance between osmotic and inhibition pressures. The purpose of this study is to calculate the longitudinal spherical aberration and thickness, and consequently the mass of aphakic corneal contact lenses in air to determine optimum lens designs. The longitudinal spherical aberration has been calculated by the ray tracing method through the aspheric surfaces. Geometrically based formulas for lathing aphakic epikeratophakia lenses have been simplified to enable easy calculation of the radii of cut for the optical and the wing zones. The formulas compensate for postoperative thinning of the lens on host cornea and allow for any desired amount of pre-lathing dehydration. Clinical results to date show that over 80% of lenses are made within 3 diopters of the expected power.

Of 650 eyes with posterior uveal melanomas that were accessioned at the Armed Forces Institute of Pathology from 1975 to 1983, 28 were aphakic and eight were pseudophakic. In ten of these 36 cases, the cataract had been unilateral. Only two patients had been examined with preoperative A-scan ultrasonography to obtain the axial length of the eye. None of the patients had been examined for possible intraocular tumors by either A- or B-scan ultrasonography. Smith and Atchison (1983) give an optical model of a "zonal aspheric" aphakic lens, but note that lenses mass-produced to this design philosophy do not show similar optical properties to the model. This paper offers an explanation for this discrepancy, supported by lens measurements.
