Trends in incidence and neonatal outcome following low Apgar scores (1 min Apgar score < 6) were prospectively studied during the years (1981, 1983, 1986 and 1988. The incidence of birth asphyxia was 7.6% of live births during the study period; it was 5.8% in 1981, increased to highest of 8.9% in 1986 with slight reduction to 7.2% in 1988. At one high-risk perinatal centre over a 9-year period, 83.1% (103/124) sets of liveborn twins with gestational ages less than 33 weeks were delivered vaginally. Mortality in vaginal births was 26.7% (55/206), almost double that of Caesarean births of 14.3% (6/42), a non-significant difference. The function of the sympathetic-adrenal system was studied by measuring umbilical cord blood catecholamines in 31 full-term newborns with perinatal asphyxia and evidence of hypoxic-ischemic encephalopathy (HIE) and in 20 normal newborns. A case of extremely severe birth asphyxia is reported. In spite of successful resuscitation the arterial pH was still only 6.6 at the age of 1 h. Assessment at 3 years 10 months revealed that the child was not only completely normal, but was of superior intelligence. This case demonstrates the advisability of maintaining an optimistic approach even when very severe fetal acidosis is detected around the time of birth.

In order to specify the influence of pre- and perinatal factors in the etiology of the different forms of congenital cerebral palsy the anamnestic data of 178 children born between 1962 and 1976 were analysed. The main adverse factors for the whole group were perinatal asphyxia (40%), prematurity (37%) and bleeding in the first trimenon (11%). As expected asphyxia was of foremost importance in spastic tetraplegia and prematurity in spastic diplegia. A 2-week-old premature child with congenital glaucoma secondary to anterior cleavage syndrome was treated with timolol maleate and cyclocryotherapy. The patient had apneic spells of up to 30 seconds that stopped soon after timolol maleate therapy was discontinued. No apnea was seen before timolol maleate administration, and no further spells were noted after subsequent cyclocryotherapy without timolol maleate treatment. Possible central nervous system toxicity of timol maleate or its metabolic by-products in neonates with immature blood-brain barriers was noted.

Biochemical and hormonal effects of oral calcium supplementation in premature and asphyxiated neonates during the first few days of life are described. Eight pairs of infants were matched for gestational age and one-minute Apgar score.
