Causes and Consequences of Lack of Transparency In Iran's Healthcare System: A Qualitative Interview Study.

Lack of transparency in healthcare systems is a critical challenge that undermines accountability, fuels corruption, and erodes public trust. While transparency has been shown to generate multiple positive outcomes, the Iranian healthcare system continues to suffer from significant deficits in this area. To investigate this issue, a qualitative approach was employed, which involved conducting 35 in-depth interviews with experts and social activists engaged in healthcare system transparency at all levels, recruited through purposive stratified sampling method. This was followed by a thematic an

Parents (of minors) are not surrogates: acknowledging (finally) the unique moral space of parents.

A common conflation happens both in everyday discussions in medicine as well as in the medical literature-namely, equating 'parents of minors' with 'surrogate decision makers.' It is important for pediatric clinicians and ethicists to stop using language that confuses and obscures the important difference between surrogates and parents in ways that affect the ethics of decision-making for minors. Specifically, parents of minor children are morally different than surrogates, and by using surrogate language, a misunderstanding arises that distorts the moral space and relevant obligations that pe

Survey of OB/GYN Residents Knowledge and Practice of Trauma Informed Care.

BACKGROUND: There is a high prevalence of a history of traumatic experiences among those seeking obstetric/gynecologic care. OB/GYN clinicians must use trauma-informed care practices to avoid re-traumatization and to provide supportive care. The objective of this study was to describe the knowledge and use of trauma-informed care practices by OB/GYN residents in the outpatient setting. METHODS: A descriptive research design was used. In February 2022, an email with a link to an electronic survey was sent to US OB/GYN residency program directors via a listserv. OB/GYN program directors were as

Confugenics - East Asian culture favors uptake of human cognitive enhancement and IVF genetic technologies amid demographic challenges.

This study examines the declining fertility rates in East Asian Confucian societies, focusing on the unique childrearing practices and how future advancements in human enhancement and reprogenetic technologies may further accelerate the demographic decline. The focus is on the obsession with "child perfectionism" driven by the pursuit of academic credentialism and hypercompetitive social norms. This phenomenon has roots in the historical imperial examinations of China and has evolved into modern college entrance exams. Recent growth in knowledge-based and technology-driven economies in East As

Will Human-Animal Chimeras Cause Moral Confusion? Exploring Public Attitudes.

Recent medical research involving human-monkey chimeras, human brain organoids in rats, and the transplantation of a gene-edited pig heart and gene-edited pig kidneys in living human beings have intensified the debate about whether we should create human-animal chimeras for biomedical purposes and, if so, how we should treat them. Influential views in the debate frequently appeal to assumptions regarding how people will react to such chimeras. It has, for example, been argued that the most important objection against creating such chimeras is that this will result in inexorable moral confusion

Family Presence in Pediatric Cardiac Procedural Settings: A Qualitative Study of Clinicians, A Key Stakeholder Group.

BACKGROUND: With increased emphasis on healthcare transparency, parents are increasingly asking to be present for procedures performed on their children, especially in high-acuity contexts like care of children with congenital heart disease (CHD), where procedures may inform critical care decisions. In addition, observations of complex care may better communicate clinical knowledge and benefit grieving after adverse events. We examined clinicians' views on current family presence (FP) efforts and on the expansion of FP to include the observation of operative procedures. METHODS: This is a qua

What is pregnancy and what is disease? a critique of Smajdor and Räsänen's "is pregnancy a disease? a normative approach".

Anna Smajdor and Joona Räsänen argue that pregnancy should be classified as a disease (In: Is Pregnancy a Disease? A Normative Approach, vol 51, 2025). But their argument faces a problem not yet raised by other critics (see In: Pregnancy, Pain and Pathology: A Reply to Smajdor and Räsänen, vol 51, 2025; In: No, Pregnancy Is Not a Disease, vol 51, 2025; In: Failing to Deliver: Why Pregnancy Is Not a Disease, vol 51, 2025). To classify a phenomenon, e.g., pregnancy, as belonging to a category, e.g., disease, one must characterize the category as well as the phenomenon. But Smajdor and Räsänen do

Ethical Issues and Recommendations in Psychedelic Research and Practice: A Scoping Review.

The rapid growth in psychedelic research raises novel ethical challenges for both research and psychedelic-assisted therapy. Despite these challenges, there is no consensus among researchers, clinicians, patients, and regulators on how these ethical issues may be avoided or managed. This study aimed to identify key ethical issues in psychedelic research and practice in the literature. A scoping review was performed, identifying fifty-one relevant articles. Content analysis revealed five main ethical concerns in the literature: 1) standards of practice, 2) equity, 3) integrity, 4) cultural appr

Recognizing Humanity, Perceiving Ableism: Supporting Parents of Children with Down Syndrome.

This commentary reflects on twelve narratives authored by parents of children with Down syndrome. While the narratives represent a diverse set of experiences, the commentary identifies some repeated narrative aims, unified under the goal of recognition. For example, multiple authors explicitly or implicitly seek recognition of their children beyond stereotypes while others hope to make ableist values and practices more visible to clinicians and other readers. Through their rich descriptions of parenting, the authors also show themselves to be in need of support both in terms of material and bu

Should Australia Adopt a Groningen Like Protocol?

Based on an analysis of a case study from a Victorian case of neonatal hypoxic ischaemic encephalopathy (HIE), I want to propose that in some circumstances it would be desirable to have well-regulated legal provisions, such as the Groningen Protocol (GP), to hasten the death of newborns with catastrophic health conditions in order to reduce suffering. My argument will take the following form: (1) Neonatal euthanasia in form of withholding all (including natural) nutrition and hydration is already practiced in Victoria and considered legal in circumstances of palliative care. (2) In cases where

Mandated Disposal and the Management of Foetal Death: A State-by-State Analysis of Foetal Disposition Laws in the United States Post-Dobbs.

Across the United States, every state establishes their own requirements for the disposition of foetal remains, including laws that address the disposal and reporting of foetal remains after a foetal death has occurred. Most states discard foetal remains as biohazard material, processed by healthcare facilities through medical incineration or sterilization. However, there are variations among state laws, with some states imposing specific requirements for the disposal of foetal remains and treating foetuses as persons. To examine these differences, I reviewed U.S. state statutes on foetal disp

Mpox and the Ethics of Outbreak Management: Lessons for Future Public Health Crises.

Mpox, first identified in captive monkeys in 1958 and recognized in humans by 1970 in the Democratic Republic of Congo, was historically confined to sporadic zoonotic outbreaks in Central and West Africa. These outbreaks, often driven by rodent-to-human transmission in resource-limited settings, reflect persistent systemic health disparities. In recent years, mpox has also been reported in high-income countries (including the United States, United Kingdom and Europe), underscoring its global health implications beyond traditionally endemic regions. This article examines the mpox outbreak throu

Historical perspectives in medical humanities: two views on vaccination during the plague pandemic in Calcutta.

The aim of this article is to complicate and rethink histories of non-Western responses to vaccination, as well as to see how a historical perspective may contribute to contemporary discussions of vaccines within the medical humanities. This is done through a case study approach. The focus of this article is a collection of essays written by a Bengali prophet-astrologer and published during the plague pandemic in Calcutta, British India, at the turn of the twentieth century (1899). While one essay in the collection was critical of the plague vaccine, another essay, in a later section of the sa

Should Patients Be Allowed to Pay Out of Pocket? The Ethical Dilemma of Access to Expensive Anti-cancer Treatments in Universal Healthcare Systems: A Dutch Case Study.

With the increasing prices of newly approved anti-cancer treatments contributing to rising healthcare costs, healthcare systems are facing complex economic and ethical dilemmas. Especially in countries with universal access and mandatory health insurance, including many European countries, the organizing of funding or reimbursement of expensive new treatments can be challenging. When expensive anti-cancer treatments are deemed safe and effective, but are not (yet) reimbursed, ethical dilemmas arise. In countries with universal healthcare systems, such as the Netherlands, this gives rise to a r

Donation After Circulatory Death following Withdrawal of Life-Sustaining Treatments. Are We Ready to Break the Dead Donor Rule?

A fundamental criterion considered essential to deem the procedure of vital organ procurement for transplantation ethical is that the donor must be dead, as per the Dead Donor Rule (DDR). In the case of Donation after Circulatory Death (DCD), is the donor genuinely dead? The main aim of this article is to clarify this uncertainty, which primarily arises from the fact that in DCD, death is determined based on cardiac criteria (Circulatory Death, CD), rather than neurological criteria (Brain Death, BD), and that to allow the procurement procedure, physicians reperfuse the organs in an assisted m

Smoking & autonomy: the generational tobacco endgame.

New Zealand and Malaysia have abandoned plans to introduce a generational smoking ban because of concerns that such a policy is incompatible with liberal democracy as it undermines autonomy. This paper challenges this claim by showing that smoking is not an autonomous act. Autonomy requires a deliberation of preferences, wills and inclinations. This does not occur in smokers because of three related factors: nicotine addiction, cognitive biases and psychosocial development in addiction. Nicotine addiction results in strong physical and psychological desires to seek pleasure and to avoid withdr

Jewish Ethics of Inmate Vaccines Against COVID-19.

PURPOSE: The COVID-19 pandemic broke out at the end of 2019, and throughout 2020 there were intensive international efforts to find a vaccine for the disease, which had already led to the deaths of some five million people. In December 2020, several pharmaceutical companies announced that they had succeeded in producing an effective vaccine, and after approval by the various regulatory bodies, countries started to vaccinate their citizens. With the start of the global campaign to vaccinate the world's population against COVID-19, debates over the prioritization of different sections of the pop
