The development of West Nile virus safety policies by Canadian blood services: guiding principles and a comparison between Canada and the United States.

To address the emerging threat of West Nile virus (WNV) to the blood supply, Canadian Blood Services (CBS) made a series of policy decisions that were either similar to those adopted in the United States or more stringent than policies formulated in the United States at the same time. More stringent Canadian policies included the development of an in-house WNV RNA assay, the stockpiling of frozen plasma components in the winter for transfusion in WNV-affected areas in the summer, a special recruitment campaign for red blood cell collections before the start of the 2003 WNV season, and an inven

Feasibility of collecting, processing and storing umbilical whole cord blood as a source for neonatal red blood cell transfusions (Whole-Cord).

BACKGROUND AND OBJECTIVE: The recent BORN trial showed that the transfusion of red blood cells collected from umbilical cord blood (UCB; rich in HbF), represents a physiologically safe option. This study evaluated the feasibility of collecting and storing UCB as whole blood and assessed the hematological and biochemical changes over storage. MATERIALS AND METHODS: In this prospective in-vitro study, UCB was collected from term vaginal deliveries (in utero) after 1-minute delayed cord clamping using CPD bags and stored at 2-6°C for 14 days. Volume collected, HbF levels, plasma hemoglobin, supe

Normalization of blood clotting characteristics using prothrombin complex concentrate, fibrinogen and FXIII in an albumin based fluid: experimental studies in thromboelastometry.

BACKGROUND: Colloid fluids supplemented with adequate combinations of coagulation factor concentrates with the capability to restore coagulation could be a desirable future treatment component in massive transfusion. METHODS: Starting from a coagulation factor and blood cell-free albumin solution we added Prothrombin Complex Concentrate, Fibrinogen Concentrate and Factor XIII in different combinations and concentrations to analyze their properties to restore thromboelastometry parameters without the use of plasma. Further analysis under the presence of platelets was performed for comparabilit

Pre-storage leucoreduction of platelet concentrates reduced biological response modifier concentrations and TRALI incidence in an in vivo sheep model.

Transfusion-related acute lung injury (TRALI) is a potentially fatal transfusion reaction. It can be caused by the transfusion of proteins, lipids, and extracellular vesicles that accumulate in blood components during routine storage and are collectively referred to as biological response modifiers (BRMs). It has been suggested that leucoreduction might reduce TRALI risk by limiting the accumulation of leucocyte-derived BRMs; however, there are conflicting data from haemovigilance reports and animal models regarding this. We harvested supernatant (SN) from leucoreduced buffy coat pooled platel

[Blood donors, risk factors and sexual orientation].

In order to reach high-quality blood-components, it is necessary that people who wish to be a blood donor shall observe a series of conditions that guarantee the least possible risk for the recipient and themselves. According to international standards, an adequate and reliable supply of safe blood can be assured by a stable base of regular, voluntary, unpaid blood donors (altruistic), which is a goal set by the World Health Organization; unfortunately in our country it only represents around 3%, despite the efforts of different institutions. In addition, the population is unaware of risk fact

Utility of point-of-care platelet aggregation testing for transfusion prediction.

Early identification and initiation of therapy for life-threatening hemorrhage is essential to minimize patient morbidity and mortality. In primary hemostasis, platelet function is integral to reach this goal, but major hemorrhage leads to impaired platelet mechanical activation and aggregation. Current devices for measuring platelet function are cumbersome or not promptly available for clinical decision making in this setting. Within this manuscript we prospectively evaluate a novel, rapid assay utilizing measures of platelet aggregation to predict hemorrhage. In this prospective cohort study

[Determination of anti-HTLV-I/II antibodies: Experience in 28,897 blood donations in Buenos Aires].

PURPOSE: The first human retrovirus, HTLV-I, was isolated in 1980; HTLV-II was described later. The former is endemic in southwestern Japan, the Caribbean and equatorial Africa; whereas the latter prevails in intravenous drug addicts, being also endemic in American indian populations. Both viruses are either sexually transmitted, from mother to child mainly by breast-feeding, by blood transfusion or by sharing contaminated needles. With regard to transmission, since they are intracellular viruses, it occurs only when whole blood or cellular components are transfused; this is not the case when

[The course of parvovirus B19 infection during pregnancy after kidney transplantation].

Having a tropism for erythroid progenitor cells, parvovirus B19 may cause partial red cell aplasia and thrombocytopenia. Early diagnosis of parvovirus B19 infection in immunocompromised patients is needed for timely antiviral therapy. A high-risk group for parvovirus B19 infection includes patients with blood diseases who receive multiple transfusions of blood components; those who have undergone donor organ transplantation and are long taking immunosuppressive drugs; and pregnant women. These patients require careful virological monitoring for major blood-borne viral infections. This paper de

Not as simple as ABC: Tools to trigger massive transfusion in pediatric trauma.

BACKGROUND: Early and accurate identification of pediatric trauma patients who will receive massive transfusion (MT) is not well established. We developed the ABCD (defined as penetrating mechanism, positive focused assessment with sonography for trauma, shock index, pediatric age-adjusted [SIPA], lactate, and base deficit [BD]) and BIS scores (defined as a combination of BD, international normalized ratio [INR], and SIPA) and hypothesized that the BIS score would perform best in the ability to predict the need for MT in children. METHODS: Pediatric trauma patients (≤18 years old) admitted to

Successful recovery after amniotic fluid embolism in a patient undergoing vacuum-assisted vaginal delivery.

Amniotic fluid embolism (AFE) is a rare, catastrophic emergency that requires prompt recognition and treatment. Despite early recognition and supportive therapy, the morbidity and mortality remain high. We report a case of AFE after vacuum-assisted vaginal delivery resulting in hemodynamic collapse and subsequent multiorgan failure. Management included mechanical ventilation, extracorporeal membrane oxygenation, and continuous veno-venous hemodialysis. The patient was able to make a full recovery with minimal sequelae. In AFE with multiorgan failure, extracorporeal membrane oxygenation and con

Design, development and implementation of a new transfusion safety system at the Banc de Sang i Teixits in Catalonia (Spain).

BACKGROUND AND OBJECTIVES: Although blood component transfusion is a safe procedure, it remains vulnerable to preventable errors related to patient and product identification, protocol adherence and staff training. In Catalonia, significant heterogeneity persists in transfusion safety systems (TSS), spanning from basic procedures to advanced technologies, resulting in process variability and potential risks to patient safety. In response, the Banc de Sang i Teixits (BST) prioritized the development of a unified, proprietary TSS. This study aimed to design, evaluate and implement a robust, reli…

A Narrative Review of the Use of Thrombopoietin and Thrombopoietin Analogues in Trauma and Critical Illness.

Introduction: Studies exploring the use of thrombopoietin (TPO) in trauma and hemorrhage-associated thrombocytopenia are limited. Transfusion of platelets, whether as part of whole blood or component therapy, is a cornerstone of survival in trauma patients. Adjuncts to transfusion may prove useful in the pre-hospital setting or in other settings where the logistics or risks of transfusion may not prove feasible. The purpose of this review is to evaluate the literature for the applications of TPO in critical illness and trauma. Methods: We conducted a review of relevant literature to provide a
