Asthma and allergy patterns over 18 years after severe RSV bronchiolitis in the first year of life.

BACKGROUND: An increased prevalence of asthma/recurrent wheeze (RW), clinical allergy and allergic sensitisation up to age 13 years has previously been reported in subjects hospitalised with respiratory syncytial virus (RSV) bronchiolitis in their first year of life compared with matched controls. A study was undertaken to examine whether these features persist into early adulthood, to report longitudinal wheeze and allergy patterns, and to see how large and small airway function relates to RSV infection and asthma. METHODS: Follow-up at age 18 years was performed in 46 of 47 subjects …

T - helper2 /T - helper1 imbalance in respiratory syncytial virus bronchiolitis in relation to disease severity and outcome.

The present study included 80 Egyptians infants with acute bronchiolitis; age ranged from 2 to 22 month and consisted of 60 males and 20 females. In addition, 30 apparently healthy infants of matched age and sex were also included as controls. Besides taking history, thorough clinical examination and routine laboratorial investigations, arterial oxygen saturation, RSV tissue culture of nasopharyngeal lavage, indirect immnoflourescence for serum IgM against RSV, serum IL-4 and IFN-gamma were done. Results obtained showed that viral bronchiolitis is predominant in male around 6 month of age and …

Severe and recurrent episodes of bronchiolitis obliterans organising pneumonia associated with indolent CD4+ CD8+ T-cell leukaemia.

The present study describes an adult male who has had recurrent episodes of pulmonary infiltrates with severe acute respiratory failure over a period of 10 yrs. Clinical and pathological characteristics revealed bronchiolitis obliterans with organising pneumonia (BOOP) that responded dramatically to prednisone. BOOP is characterised by inflammation of the bronchioles and surrounding tissue in the lungs. It can mimic infectious pneumonia but diagnosis is suspected when there is no response to multiple antibiotic treatment, and blood and sputum cultures are negative for microorganisms. A high pr…

Severe respiratory syncytial virus bronchiolitis in infants is associated with reduced airway interferon gamma and substance P.

BACKGROUND: Severe human respiratory syncytial virus (hRSV) bronchiolitis in previously well infants may be due to differences in the innate immune response to hRSV infection. AIM: to determine if factors mediating proposed mechanisms for severe bronchiolitis differ with severity of disease. METHODOLOGY/PRINCIPLE FINDINGS: 197 infants admitted to hospital with hRSV bronchiolitis were recruited and grouped according to no oxygen requirement (n = 27), oxygen dependence (n = 114) or mechanical ventilation (n = 56). We collected clinical data, nasopharyngeal aspirate (NPA) and if ventilated bron…

Bronchial responsiveness to methacholine and adenosine 5'-monophosphate (AMP) in young children with post-infectious bronchiolitis obliterans.

AIM: Bronchial hyperresponsiveness (BHR) is a characteristic feature of asthma, but it is also frequently demonstrated by children and adults with chronic obstructive lung diseases. BHR is usually measured by bronchial challenges using direct or indirect stimuli. The aim of this study was to compare these two types of bronchial challenge in young children with post-infectious bronchiolitis obliterans (BO). METHODS: Methacholine and adenosine 5'-monophosphate (AMP) bronchial challenges were performed on preschool children with post-infectious BO (n=18), those with asthma (n=23) and in controls…

Racemic epinephrine compared to salbutamol in hospitalized young children with bronchiolitis; a randomized controlled clinical trial [ISRCTN46561076].

BACKGROUND: Bronchiolitis is the most common cause of lower respiratory tract illness in infancy, and hospital admission rates appear to be increasing in Canada and the United States. Inhaled beta agonists offer only modest short-term improvement. Trials of racemic epinephrine have shown conflicting results. We sought to determine if administration of racemic epinephrine during hospital stay for bronchiolitis improved respiratory distress, was safe, and shortened length of stay. METHODS: The study was a randomized, double-blind controlled trial of aerosolized racemic epinephrine compared to s…

Discrepancy between cytokine production from peripheral blood mononuclear cells and nasal secretions among infants with acute bronchiolitis.

BACKGROUND: Many studies have measured cytokine production derived from peripheral blood mononuclear cells (PBMCs) to evaluate the immune response in acute bronchiolitis (AB), but no previous reports have examined the association between PBMC release of cytokines and concomitant airway immune response. OBJECTIVE: To determine whether interferon-gamma (IFN-gamma), interleukin 4 (IL-4), and IL-10 levels from PBMCs are associated with concurrent cytokine release in the airways of infants with AB. METHODS: Infants with acute viral-associated first episode of wheezing who required hospitalization…

Admission to the intensive care unit for respiratory syncytial virus bronchiolitis: a national survey before palivizumab use.

OBJECTIVE: Preterm infants, especially those with chronic lung disease (CLD), are considered more susceptible to severe respiratory illness from respiratory syncytial virus (RSV) infection than healthy term infants, and are therefore targeted for prophylactic administration of immune globulins. The impact of this practice on the more severe cases of bronchiolitis (i.e., pediatric intensive care unit [PICU] admission, mechanical ventilation, mortality) has not been reported to date. The aim of this study was to evaluate PICU admissions, need for mechanical ventilation, and mortality attributabl…

[Three cases of respiratory bronchiolitis-associated interstitial lung disease (RB-ILD): a study of HRCT-pathologic correlation].

Respiratory bronchiolitis-associated interstitial lung disease (RB-ILD) is a clinicopathologic entity occurring rarely in smokers. We report three cases of RB-ILD diagnosed pathologically by surgical lung biopsy. Cough was observed in all cases, sputum in one case and dyspnea on exertion in another. Reduction of diffusing capacity was observed in all three cases. No abnormality was found in the chest radiographs of any case. However, in high-resolution computed tomography (HRCT), ground-glass opacities and centrilobular nodules were observed in all three cases, emphysema in one case, intralobu…

Bronchiolitis obliterans organizing pneumonia syndrome in breast-conserving therapy for early breast cancer: radiation-induced lung toxicity.

PURPOSE: Breast-conserving surgery and postoperative radiotherapy have played important roles in the treatment of early breast cancer. Bronchiolitis obliterans organizing pneumonia (BOOP) syndrome has recently been reported to be one of the complications of adjuvant radiotherapy. The purpose of this study was to determine the incidence of and risk factors for BOOP syndrome in breast cancer patients. METHODS AND MATERIALS: Between January 1996 and December 1998, 157 patients with breast cancer underwent radiotherapy after breast-conserving surgery. The criteria used for the diagnosis of BOOP s…

Long-term effects of respiratory syncytial virus (RSV) bronchiolitis in infants and young children: a quantitative review.

UNLABELLED: One of the major questions regarding long-term side effects of bronchiolitis by respiratory syncytial virus (RSV) is whether or not it induces asthma in later life. In this quantitative review, the data of 10 controlled studies are analysed. METHODS: Follow-up studies of RSV bronchiolitis published between January 1978 and December 1998 were identified through a MEDLINE search. Studies were selected if (i) postnatal age at the time of the initial illness was below 12 mo, (ii) all children were hospitalized for RSV bronchiolitis, (iii) the diagnosis RSV was virologically confirmed …

Persistent high BAL fluid granulocyte activation marker levels as early indicators of bronchiolitis obliterans after lung transplant.

The major cause of mortality in the long-term in lung transplant recipients is chronic rejection. This is a fibroproliferative process in the small airways leading to obliterative bronchiolitis and progressive loss of lung function, both constituting the clinical entity bronchiolitis obliterans syndrome (BOS). Granulocyte activation has been implicated as one factor behind BOS. Granulocyte markers in bronchoalveolar lavage (BAL) fluid were prospectively and longitudinally studied in order to identify possible association with BOS. BAL fluid from 266 bronchoscopy procedures performed in twelve …

[Clinicopathologic study of various lung diseases with bronchiolitis obliterans organizing pneumonia (BOOP) pattern in open lung biopsy].

The histologic findings of BOOP are nonspecific, and the diagnosis of idiopathic BOOP is one of exclusion. We studied the clinicopathologic features of various lung diseases with histopathological appearance of bronchiolitis obliterans organizing pneumonia (BOOP) pattern in open lung biopsy specimens. The 17 patients with BOOP pattern studied included idiopathic BOOP (n = 7), unclassified interstitial pneumonia (n = 1), collagen vascular disease (n = 3, RA 1, PM/DM 2), hypersensitivity pneumonitis (HP, n = 2), eosinophilic pneumonia (EP, n = 1), multiple lung abscesses (n = 1), limited form of…

Azithromycin for acute wheezing and bronchiolitis in children younger than six years: A systematic review and meta-analysis.

BACKGROUND: Azithromycin is often prescribed off-label for acute wheezing and bronchiolitis in young children, despite guideline advice against routine antibiotics. We synthesised the randomised evidence and tested whether benefit depends on care setting. METHODS: We searched four databases and two registries to June 2026 for double-blind, placebo-controlled trials of azithromycin versus placebo in children younger than six years with acute wheezing or bronchiolitis. The acute episode was pooled as the ratio of means and binary outcomes as risk ratios (random-effects models). RESULTS: Ten tr…

Respiratory Support in Bronchiolitis: A Narrative Review of the Evidence for and Against High-Flow Nasal Cannula (HFNC) Use.

Bronchiolitis is a leading cause of pediatric hospitalization, and the use of high-flow nasal cannula (HFNC) as respiratory support has increased substantially over the past decade despite ongoing uncertainty about its clinical benefits. This narrative review evaluates the evidence supporting and questioning HFNC use in infants and young children with bronchiolitis. Early observational studies suggested reductions in intubation rates and improved clinical parameters with HFNC; however, more recent randomized controlled trials show no consistent benefit in length of stay, duration of oxygen the…

Necrotizing Enterocolitis as a Rare Complication of Severe RSV Bronchiolitis in a Term Neonate: A Case Report With Narrative Review.

Respiratory syncytial virus (RSV) bronchiolitis is a leading cause of hospitalization in early infancy, whereas necrotizing enterocolitis (NEC) is rare in term neonates without risk factors. We report a previously healthy, full-term 24-day-old neonate admitted with severe RSV bronchiolitis who developed sudden deterioration with abdominal distension, metabolic acidosis, septic shock, and imaging consistent with advanced NEC, ultimately resulting in death despite intensive medical and surgical management. To contextualize this association, we conducted a narrative review of case reports and sma…

Findings From Long-Term Follow-up in Children Hospitalized for Bronchiolitis in Pisa: Is It Time for a Standardized Protocol?

BACKGROUND: Children with bronchiolitis in early life are at increased risk of wheezing and asthma in the following years, though its exact role in these respiratory sequelae remains unclear. OBJECTIVES: This study aimed to determine the long-term outcomes of children previously hospitalized with bronchiolitis and to evaluate the need for a standardized follow-up in the following years. METHODS: Among 346 patients hospitalized for bronchiolitis at the Pediatrics Unit of the University Hospital of Pisa between 2010 and 2019, 234 were reached by phone and 102 agreed to participate to our follo…
