Necrotizing Faciitis after shoulder mobilization and intra-articular infiltration with betametasone.

Necrotizing Fasciitis is a rapidly progressive, potentially fatal infection of superficial fasciae and subcutaneous tissue, usually resulting from an inciting trauma to the skin. Medical literature refers few cases of necrotizing fasciitis related to intra-articular infiltrations, that often lead to patients death. This report describes the clinical events on a 55 year-old diabetic patient who developed upper extremity Necrotizing Fasciitis, 18 days after shoulder mobilization and intra-articular infiltration, due to Staphylococcus epidermidis. An early surgical debridement was performed and a

Adhesive capsulitis of the hip: a review.

Adhesive capsulitis of the hip (ACH) is a rare clinical entity. Similar to adhesive capsulitis of the shoulder, ACH is characterized by a painful decrease in active and passive range of motion as synovial inflammation in the acute stages of the disease progresses to capsular fibrosis in the chronic stages. Once other diagnoses have been ruled out, management of ACH is tailored to reduce inflammation in the acute stages with NSAIDs, intra-articular steroid injections, and targeted physical therapy while biomechanical dysfunction in the spine, hip, sacroiliac joint, or lower limb joints is addre

Effect of humeral head defect size on glenohumeral stability: a cadaveric study of simulated Hill-Sachs defects.

BACKGROUND: Hill-Sachs lesions are often present with recurrent shoulder instability and may be a cause of failed Bankart repair. HYPOTHESIS: Glenohumeral joint stability decreases with increasingly larger humeral head defects. STUDY DESIGN: Descriptive laboratory study. METHODS: Humeral head defects, 1/8, 3/8, 5/8, and 7/8 of the humeral head radius, were created in 8 human cadaveric shoulders, simulating Hill-Sachs defects. Testing positions included 45 degrees and 90 degrees of abduction and 40 degrees of internal rotation, neutral, and 40 degrees of external rotation. Testing occurred a

The antecubital fasciocutaneous island flap for elbow coverage.

Soft-tissue defects in the area of the periolecranon may be a source of concern to the reconstructive surgeon who aims for durable protection with a minimum of drawbacks. Lamberty and Cormack described the antecubital fasciocutaneous flap both as a local transposition and as a free flap. The island version of this flap enables a single-stage transfer of thin, pliable, sensitive skin into the region of the periolecranon without further scarring around the defect. In general, most of the donor site can be closed primarily together with a small, full-sheet, split-thickness skin graft on the remai

Arthroscopic subacromial decompression versus open acromioplasty. A two-year follow-up study.

In a prospective study, 20 patients were randomly selected for either open acromioplasty according to Neer or for arthroscopic subacromial + decompression (ASD). All suffered from shoulder impingement for which conservative treatment had been unsuccessful. Functional results after two years were assessed. All patients were assessed radiographically to visualize recurrence of subacromial osteophytes. Functional results in the arthroscopic group are good and similar to those after open surgery. Both methods seem to result in adequate subacromial decompression, including bone resection. The arthr

Impingement syndrome in athletes.

Athletes, particularly those who are involved in sporting activities requiring repetitive overhead use of the arm (for example, tennis players, swimmers, baseball pitchers, and quarterbacks), may develop a painful shoulder. This is often due to impingement in the vulnerable avascular region of the supraspinatus and biceps tendons. With the passage of time, degeneration and tears of the rotator cuff may result. Pathologically the syndrome has been classified into Stage I (edema and hemorrhage), Stage II (fibrosis and tendonitis), and Stage III (tendon degeneration, bony changes, and tendon rupt

Calf pain mimicking thrombophlebitis.

Calf pain is a common complaint in the primary care setting. An understanding of the relationship of calf pain to the popliteal cyst and pseudothrombophlebitis syndrome and a systematic approach to evaluation may avoid a mistaken diagnosis of thrombophlebitis and unnecessary anticoagulation therapy.

Distal biceps tendon injuries: A clinically relevant current concepts review.

Distal biceps tendon (DBT) conditions comprise a spectrum of disorders including bicipitoradial bursitis, partial tears, acute and chronic complete tears. In low-demand patients with complete DBT tears, non-operative treatment may be entertained provided the patient understands the potential for residual weakness, particularly in forearm supination. Most acute tears are best treated by primary repair using either single-incision or double-incision techniques with good clinical outcomes.Single-incision techniques may carry a higher risk of nerve-related complications, whereas double-incision tech…

Helpful tips for performing musculoskeletal injections.

Injections are valuable procedures for managing musculoskeletal conditions commonly encountered by family physicians. Corticosteroid injections into articular, periarticular, or soft tissue structures relieve pain, reduce inflammation, and improve mobility. Injections can provide diagnostic information and are commonly used for postoperative pain control. Local anesthetics may be injected with corticosteroids to provide additional, rapid pain relief. Steroid injection is the preferred and definitive treatment for de Quervain tenosynovitis and trochanteric bursitis. Steroid injections can also …

Imaging findings of complications affecting the upper extremity in intravenous drug users: featured cases.

Infections generally occur in intravenous drug abuse (IVDA) patients, most commonly affecting the spine and proximal joints. Numerous serious musculoskeletal complications of IVDA may involve the upper extremity, however. Soft-tissue complications in the upper extremity of IVDA patients include cellulitis, ulceration, abscess, pyomyositis, septic bursitis, tenosynovitis, and necrotizing fasciitis. Foreign bodies in soft tissue due to needle fragments are common findings. Primary bone and joint IVDA complications include osteomyelitis (acute and chronic) and septic arthritis. Other IVDA complic…

Forefoot pain involving the metatarsal region: differential diagnosis with MR imaging.

Many disorders produce discomfort in the metatarsal region of the forefoot. These disorders include traumatic lesions of the soft tissues and bones (eg, turf toe, plantar plate disruption, sesamoiditis, stress fracture, stress response), Freiberg infraction, infection, arthritis, tendon disorders (eg, tendinosis, tenosynovitis, tendon rupture), nonneoplastic soft-tissue masses (eg, ganglia, bursitis, granuloma, Morton neuroma), and, less frequently, soft-tissue and bone neoplasms. Prior to the advent of magnetic resonance (MR) imaging, many of these disorders were not diagnosed noninvasively, …
