The mechanism of action of calcium channel modulators, a class of drugs that includes 3 chemical groups--1,4-dihydropyridines, phenylalkylamines and benzothiazepines--has been extensively reviewed. The best known representatives of these 3 groups are nifedipine, verapamil and diltiazem, respectively. These drugs bind reversibly, stereospecifically and with high affinity to both the membrane-bound and the purified receptor complex. Non-dihydropyridines allosterically regulate dihydropyridine binding. This has been shown by using (-) [3H]202-791 and (+) [3H]PN200-110 as labeled ligands. The puri…

Calcium absorption from salts and foods intrinsically labeled with 45Ca was determined in the rat model. Calcium bioavailability was nearly 10 times greater for low oxalate kale, CaCO3 and CaCl2 than from CaC2O4 (calcium oxalate) and spinach (high in oxalates). Extrinsic and intrinsic labeling techniques gave a similar assessment of calcium bioavailability from kale but not from spinach.

Microsomal fractions prepared from porcine thyroid glands by differential centrifugations and sucrose density gradient centrifugation showed an ATP-dependent Ca2+ uptake. Electron microscopy and the study of marker enzyme activities suggested that the fractions consisted mainly of endoplasmic reticulum. The amount of transported Ca2+ increased four times in the presence of 20 mM oxalate owing to the precipitation of calcium oxalate, which was detected inside the microsomal vesicles by electron microscopy. Ca2+ was released rapidly when the calcium ionophore, A-23187, was added. The Ca2+ concen…

The demonstration that the calcium channel blockers reduce pulmonary vasomotor tone in experimental models of pulmonary hypertension has served as the rationale for their use in the management of primary pulmonary hypertension. The three currently available agents, verapamil, nifedipine, and diltiazem, have varying degrees of pulmonary vasoactivity. Experience with their use is reviewed.

It is not surprising that calcium-channel blocking agents, which have numerous effects on various physiologic systems, have been employed for several "unapproved" uses. This manuscript reviews reports that have appeared within the last two years describing unapproved cardiovascular and noncardiovascular uses of the three available calcium-channel blocking agents. The cardiovascular uses discussed include hypertensive emergencies, pulmonary hypertension, congestive heart failure, aortic insufficiency, Raynaud's phenomenon, migraine headaches, antiplatelet effects and cardiac surgery. Areas of n…

Calcium metabolism during the menstrual cycle was studied in seven women from whom fasting blood samples were drawn daily or every other day throughout ovulatory cycles. Total calcium (Ca), ionic calcium (Ca++), magnesium (Mg), phosphorus (P), and immunoreactive parathyroid hormone (PTH) and calcitonin (CT) were measured. LH levels were used to date each cycle and progesterone levels were used to confirm ovulation. Plasma estradiol was measured in two of the subjects. In six subjects with cycle lengths of 27-31 days, PTH levels rose progressively through the follicular phase to a peak at or sl…

The concentration of calcium in the pancreatic juice is lower than in plasma. Two calcium fractions occur in the juice, the one associated with the enzyme protein and the other entering the juice via diffusion. In chronic pancreatitis the calcium concentration of the juice is increased in post-secretin periods. Hypercalcemia stimulates enzyme secretion and elevates calcium concentration in the juice. Hypocalcemia inhibits secretion of enzymes and fluid. Calcium is an important mediator substance for the secretion of pancreatic hydrolases at the intracellular level. In primary hyperparathyroidi…

Tetany occurs with hypocalcemia and alkalosis or both. The interrelationship of calcium and acid-base balance necessary for inducing tetany, the role of the central nervous system, and the rate of development of hypocalcemia have been investigated. Tetany occurred in less than 50 percent of one group of dogs made alkalotic by hyperventilation or made hypocalcemic by infusion of ethylene glycol-bis(beta-amino ethyl ether) N, N'-tetraacetate. In contrast, hypocalcemia combined with hypocapnic alkalosis always produced tetany. Slowly evolving hypocalcemia was achieved inanother group of dogs by t…

Objective: Combining calcium with citrate clinically prevents the formation of calcium oxalate (CaOx) stones. In this study, we developed a new method for measuring calcium citrate (CalCit) precipitated out of urine samples. Method: CalCit in urine was formed by challenging with excessive calcium and precipitated out by ethanol. The precipitated CalCit (pCalCit) was redissolved and measured by an indicator displacement assay (IDA) using murexide as an indicator. This method is called the murexide-based IDA for measurement of precipitated CalCit (miDA-pCalCit). Results: Urea, creatinine, ammoni…

Short QT syndrome is a heritable arrhythmia disorder linked to sudden cardiac death. We recently identified that individuals with alternating hemiplegia of childhood (AHC), a rare neurodevelopmental disorder, can exhibit shortened corrected QT intervals and elevated risk for ventricular fibrillation. This is especially true for patients with AHC heterozygous for the recurrent ATP1A3-D801N variant, though the underlying cardiac mechanism remains unclear. We hypothesized that the D801N missense impairs Na+/K+-ATPase function, causing Ca2+ overload, shortened action potential duration (APD), and

Hypertensive rodents often exhibit bone loss and osteopenia with impaired bone formation. We hypothesized that spontaneously hypertensive rats (SHR) could compensate for osteopathy through certain mechanisms, particularly intestinal calcium absorption. However, this calcium transport may be suppressed after long-term exposure to high luminal calcium levels during calcium supplementation through a molecular mechanism known as calcium-dependent inactivation of calcium channels. Herein, we designed calcium-rich formulations-that is, 14 mM CaCl2 + 12 mM l-Proline +20 µM sodium zinc chlorophyllin (…

OBJECTIVE: An undetectable postoperative serum cortisol has been regarded as a definition of cure in Cushing's disease. However, we noted disease recurrence amongst patients with Cushing's disease despite undetectable postoperative cortisol levels, and this led us to audit our data. We have also previously assessed surgical outcome for acromegaly and microprolactinoma for a single surgeon. The aims of this study were two-fold: (i) to investigate the treatment and surgical outcome of patients with Cushing's disease. In particular, we wished to compare the data with outcome for other pituitary t
