Evaluation and Treatment Considerations for Children and Adolescents Presenting With Psychosis Symptoms in Primary Care.

Pediatricians and all other pediatric primary care clinicians are likely to encounter youth who report symptom concerns of psychosis, such as auditory or visual hallucinations. It is essential for clinicians to be comfortable evaluating and triaging these symptoms, including assessment of co-occurring safety concerns, to ensure access to appropriate treatment. Due to limited specialty mental health resources nationwide, pediatricians may also be tasked with initiating medications or referring directly to evidence-based psychosocial interventions in the community. This article reviews different

Wildfire mental health support programs.

Global climate change has increased the risk of wildfires, which pose serious short and long-term mental health problems. Emotional well-being and access to specialized health services are among the most challenging health concerns of those affected by wildfires. In this overview, I discuss the mental health burdens of wildfires and the need for programmatic solutions and resources for developing mental health support infrastructure, including access to care, Skills for Psychological Recovery training programs, and digital health tools. These specialized programs and tools have the potential t

Multi-Disciplinary Management of the Mangled Lower Extremity: Part II.

The management of mangled extremities has long produced contention among the orthopedic surgery community due to the complexity of injuries and difficulty predicting postoperative outcomes for limb reconstruction and amputation. This review discusses reliable prognostic indicators as well as socioeconomic factors that can drastically alter the feasibility and burden of various treatments for mangled lower extremity patients and treating surgeons. This review emphasizes an integrated, multidisciplinary approach to mangled extremity management, allowing for efficient utilization of resources and

Promoting Access and Equity: A Historical Perspective of Healthcare Access for People With Disabilities.

People with disabilities represent a large and often under-recognized minority population in the United States. Historically, negative healthcare provider perceptions and limited critical social determinants of health (including community living and education) have resulted in inequitable healthcare and access for this vulnerable group. Within the last 40 years, there have been some advances in legislation to improve access and support for those with disabilities. Since then, advances in accommodations have enabled better access to critical health-related resources and care. Continued forward

Improving medical education about gender-variant youth and transgender adolescents.

Gender minority children and adolescents present to a wide variety of health professionals for gender-related care and other care. However, few professionals may be prepared to meet their needs. Unprepared clinicians risk developing insufficient therapeutic rapport, missing salient information, and inadvertently contributing to risk. In this article the authors outline ways to address these gaps at all training levels to meet the needs of gender minority children and adolescents. They provide practical resources for colleagues interested in expanding education opportunities in their own communi

Diagnostic use of Burkholderia pseudomallei selective media in a low prevalence setting.

Routine use of selective media improves diagnosis of Burkholderia pseudomallei, but resources may be limited in endemic developing countries. To maximise yield in the relatively low-prevalence setting of Kuala Lumpur, Malaysia, B. pseudomallei selective agar and broth were compared with routine media for 154 respiratory specimens from patients with community-acquired disease. Selective media detected three additional culture-positive specimens and one additional melioidosis patient, at a consumables cost of US$75. Burkholderia pseudomallei was not isolated from 74 diabetic foot ulcer samples.

Births, deaths and medical emergencies in the district: a rapid participatory appraisal in Nepal.

Two qualitative rapid appraisal techniques were used in a community-based study to investigate health beliefs, attitudes and practices in a rural district in Nepal. Pregnancy and childbirth remain non-medical activities managed at home. Deaths may be avoided by better access to hospital emergency services. Health awareness of some problems, e.g. oral rehydration and diarrhoeal disease, is high; but of others, e.g. association of haemoptysis with tuberculosis, is poor. These methods require few resources, are efficient in time and manpower required, and generate useful relevant information on a

Nursing care of patients with ocular manifestations of human immunodeficiency virus infection.

With the rising incidence of AIDS in the United States, a larger number of patients with HIV-related infectious and noninfectious ocular conditions will inevitably occur. This necessitates that ophthalmic nurses have knowledge of HIV disease; resultant ophthalmic manifestations; treatment protocols, including unapproved therapies; community resources for persons with vision loss; social issues related to HIV; psychologic responses to the disease; and associated losses for patients and their families. This knowledge base will enable the nurse to recognize disease earlier, to educate patients an…

Emergency department presentation of type A botulism.

In October 1983, 28 people became ill with type A botulism in a common-source, food-borne outbreak. Thirteen of these patients presented to an emergency department for initial medical evaluation. The records of these thirteen patients were examined retrospectively in an attempt to characterize the initial presenting symptoms. All patients complained of at least three of four symptoms (generalized weakness, diplopia, dry mouth, and difficulty speaking). Emergency physicians can play a pivotal role in limiting the extent of common-source botulism outbreaks by early recognition of the disease and

Culturally-informed strategies to address the differential impacts of climate change-related changing environmental conditions on mental health in regional Australia.

Australians living in regional, rural, and remote (RRR) communities face unique climate change impacts that exacerbate both financial and mental health challenges. These communities are disproportionately affected due to their dependence on natural resources for economic well-being and their geographical isolation, which limits access to services. Additionally, poorly designed policies and systems often overlook the specific needs of RRR communities, compounding their vulnerability. This paper highlights the importance of developing culturally responsive and context-specific interventions, co-

Functional properties improve predictions of long-term productivity invariability but not resistance to drought.

Identifying the factors mediating the resilience and invariability of plant communities and their associated ecosystem functions is critical to understand the ecological impacts of climate change. Prior work has explored how classic "community properties" (characterizing a community by species composition) regulate the resilience and invariability of ecosystem functioning. Mechanistically, community properties influence resilience and invariability via species' traits, and as a result, "functional properties" (characterizing a community via functional traits) might better predict these qualiti

Divergent successional responses and environmental drivers of soil bacterial and fungal communities in subalpine forests of the Eastern Qilian Mountains.

INTRODUCTION: Soil microbial communities and extracellular enzyme activities play critical roles in regulating carbon (C), nitrogen (N), and phosphorus (P) cycling in forest ecosystems. However, whether bacterial and fungal communities exhibit similar or divergent successional trajectories, and how environmental factors regulate these responses during long-term subalpine forest succession, remain poorly understood. METHODS: We investigated a natural successional sequence from broadleaved mixed forest to mature Picea wilsonii forest in Xinglong Mountain, Eastern Qilian Mountains, using high-th

PacBio HiFi reads data sets for the ATCC-authenticated isolate of Candida albicans SC5314 (ATCC MYA-2876).

Candida albicans is a yeast of major medical importance. The genome sequence for the reference isolate SC5314 has been available for many years and is displayed as a phased diploid assembly through a public community resource. As DNA sequencing technologies continue to advance, new high-quality data sets can be generated to augment existing genome resources. Here, we present Pacific Biosciences (PacBio) HiFi data sets with exceptional read length and sequencing depth. The data sets were generated from C. albicans SC5314 newly obtained from the American Type Culture Collection, thereby authenti

An Evaluation of the Literacy Demands of Online Natural Disaster Preparedness Materials for Families.

OBJECTIVE: Natural disasters are becoming increasingly common, but it is unclear whether families can comprehend and use available resources to prepare for such emergencies. The objective of this study was to evaluate the literacy demands of risk communication materials on natural disasters for US families with children. METHODS: In January 2018, we assessed 386 online self-directed learning resources related to emergency preparedness for natural disasters using 5 literacy assessment tools. Assessment scores were compared by information source, audience type, and disaster type. RESULTS: One-

Winter Caching of Artemisia frigida Is Associated With Host Physiological and Gut Microbial Variation in Brandt's Voles.

Seasonal variation in food resources represents a major ecological challenge for wild herbivores, yet how naturally selected dietary resources become associated with host and microbial responses remains poorly understood. During winter, Brandt's voles (Lasiopodomys brandtii) cache large amounts of Artemisia frigida, suggesting that this plant may have ecological significance beyond its nutritional value. However, the biological basis underlying this seasonal food preference remains unclear. Here, using Brandt's voles as a model system, we investigated whether the naturally selected winter food

The contribution of residents who cooperate with ring-vaccination measures against smallpox epidemic.

OBJECTIVES: Establishing containment measures against the potential spread of the smallpox virus has become a major issue in the public health field since the 2001 anthrax attacks in the United States. The primary objective of the study was to investigate the relationship between the level of activity of public health agencies and the voluntary cooperation of residents with ring-vaccination measures against a smallpox epidemic. METHODS: A discrete-time, stochastic, individual-based model was used to simulate the spread of a smallpox epidemic that has become a more pressing topic due to 9/11 a
