BACKGROUND: Glenohumeral osteoarthritis (GHO) is a common cause of shoulder pain and dysfunction in middle-aged and older adults. Anatomic total shoulder arthroplasty (aTSA) is the preferred surgical treatment for patients with an intact rotator cuff. This video focuses on technical considerations in aTSA using a lesser tuberosity osteotomy (LTO).
INDICATIONS: aTSA is indicated in patients with advanced GHO who experience persistent pain, mechanical symptoms, and functional limitations despite prolonged conservative treatment. The rotator cuff should be intact.
TECHNIQUE DESCRIPTION: The pat

BACKGROUND: Osteochondral allografting is a well-established, durable treatment option for focal patellar cartilage defects, particularly in young, active patients with symptomatic grade 3 and 4 lesions. Clinical outcomes show success rates of 77% to 87% when grafts are accurately placed and properly integrated. Precise cartilage-to-graft congruity is critical, as malalignment may lead to persistent pain, graft failure, or cartilage degeneration. Arthroscope-assisted techniques enhance visual accuracy, improving fit and load distribution while reducing the risk of prominent or unstable grafts.

BACKGROUND: A thorough understanding of posterior hip anatomy is essential for safe and effective posterior hip endoscopy. The sciatic nerve runs laterally to the ischium and hamstring origin. While sciatic nerve injury during proximal hamstring repair is uncommon, the increasing use of endoscopic techniques has led to a rise in iatrogenic injuries. This highlights the need for a structured approach to identifying and protecting the sciatic nerve.
INDICATIONS: Surgical intervention is indicated for complete proximal avulsions involving all 3 hamstring tendons, partial avulsions involving ≥2 t

BACKGROUND: Chondral pathology of the knee-specifically involving the femoral condyles and trochlea-is commonly observed during knee arthroscopies. This condition may necessitate supplementary interventions, such as a concomitant tibial tuberosity osteotomy (TTO). Approaching this patient population with a straightforward, less invasive arthroscopic technique may offer an advanced solution.
INDICATIONS: Arthro matrix-induced autologous chondrocyte implantation (MACI; autologous cultured chondrocytes on a porcine collagen membrane) targets symptomatic chondral lesions in the knee that are cons

BACKGROUND: Extensor mechanism tendon ruptures, including the quadriceps and patellar tendons, are devastating injuries that most often require acute surgical repair to restore knee mechanics and function. There are 2 well-described categories of quadriceps repair techniques, including superior pole suture anchor repairs and transosseous repairs, with recent studies reporting similar overall outcomes. However, failure rates have been reported to be as high as 18% and 21%, respectively.
INDICATIONS: Extensor mechanism ruptures.
TECHNIQUE DESCRIPTION: The described technique utilizes 2 transos

BACKGROUND: Chronic exertional compartment syndrome (CECS) is characterized by pain related to exercise that resolves with a period of rest. Along with these symptoms, CECS is commonly diagnosed based on an increase in intercompartmental pressure during exercise. A negative magnetic resonance imaging (MRI) is helpful to rule out commonly presenting symptoms. While fasciotomy remains the gold standard for patients who require more than conservative treatment, sex-specific outcome data require further investigation.
INDICATIONS: This case presents a 17-year-old female adolescent with a 2-year h

BACKGROUND: Bucket-handle medial meniscal (BHMM) tears are a complex injury commonly found in young athletes. The literature reports that, on average, 85% of BHMM repairs are successful 1 year postoperatively. When failure occurs, a revision repair is optimal to prevent long-term arthritic changes and further stress on other knee ligaments.
INDICATIONS: A revision repair of BHMM tears in adolescents is preferred compared with a meniscectomy due to a higher likelihood of the revision fully healing compared with older patients, and a higher chance of preventing the effects of increased joint co

BACKGROUND: Iliopsoas tendinopathy is an established cause of persistent anterior groin pain after total hip arthroplasty (THA), with an incidence of 2% to 6%. Although frequently attributed to acetabular component malposition and anterior prominence, symptomatic iliopsoas irritation can also occur in cases with appropriately positioned implants. Ultrasound-guided corticosteroid injection targeted to the iliopsoas tendon can be diagnostic; however, a subset of patients remains refractory to nonoperative management. In these cases, arthroscopic intervention may be indicated. We present a case h

BACKGROUND: As the number of hip arthroscopies continues to rise, revision procedures have become increasingly common, with reported rates as high as 13.2%. Persistent functional limitation after primary hip arthroscopy is most often related to unresolved bony impingement, capsular insufficiency, or recurrent labral pathology.
INDICATIONS: Revision hip arthroscopy is indicated when symptoms persist after primary surgery and an intra-articular source of the symptoms-such as inadequate or excessive bony resection, capsular mismanagement, or iatrogenic cartilage or labral injury-has been identif

INTRODUCTION: Severe pectus excavatum (PE) may impair cardiopulmonary and physical function. The effectiveness of surgical treatment to correct PE and restore physical function is widely debated due to a lack of high-quality comparative evidence. The RESTORE trial aims to determine the clinical and cost-effectiveness of corrective surgery for severe PE compared with conservative management for the first time in a randomised controlled trial (RCT).
METHODS AND ANALYSIS: RESTORE is a pragmatic, multicentre, RCT with an embedded observational cohort. 200 participants aged ≥12 years with severe P

BACKGROUND: The incidence of anterior cruciate ligament (ACL) injuries has rapidly increased, and patients with ACL injuries frequently have concomitant meniscal pathologies. Given the advantages in restoring patient anatomy, retaining proprioceptive fibers, and eliminating risks of donor site morbidity, ACL repair has reemerged as a treatment option for indicated patients. For proximal tears, augmentation of the native ACL with an internal brace has been thought to reinforce ligament strength, facilitate improved ligamentization, and have a protective effect as a secondary stabilizer. As such

BACKGROUND: Isolated medial compartment knee osteoarthritis (OA) presents a challenging problem to treat for knee surgeons, with a multitude of options from conservative management, including injections and unloader braces, meniscal procedures, osteotomies, and unicompartmental knee arthroplasty (UKA). A new medial implantable shock absorber (MISHA) allows for offloading 142 N of the medial compartment during stance phase of gait.
INDICATIONS: US Food and Drug Administration approval was obtained on April 10, 2023, with the following indications: isolated medial knee OA (Kellgren-Lawrence gra

BACKGROUND: Tibial tubercle osteotomy (TTO) is a well-established procedure that improves patella-trochlea contact area and reduces aberrant forces causing maltracking or compression of the patellofemoral joint (PFJ). Contemporary TTO surgery often involves anteriorization, medialization, distalization, or combinations like anteromedialization (AMZ). The classic Fulkerson AMZ is a time-tested standard, but the technique does not allow for isolated planar adjustments such as straight anteriorization and may not be viable for subsets of patients with medial patellofemoral cartilage lesions due t

BACKGROUND: Anterior cruciate ligament reconstructions (ACLRs) are performed to restore knee biomechanics, increase knee stability, and slow the progression of osteoarthritis. After ACLRs, many patients still have residual anterolateral instability which is a risk factor for ACL graft failure. An anterolateral ligament reconstruction (ALLR) attempts to restore the native function of the anterolateral complex to augment the ACL. Performing an ALLR with an ACLR has been reported to reduce symptoms of instability and improve clinical outcomes.
INDICATIONS: While no definitive indication for an A

BACKGROUND: Anterior cruciate ligament (ACL) repairs, once widely abandoned due to historically high failure rates, have recently regained interest with the development of the bridge-enhanced ACL repair (BEAR) implant, a novel arthroscopic technique that uses a resorbable protein-based implant combined with autologous blood to primarily repair a midsubstance ACL tear. This technical note presents a step-by-step surgical method for performing an isolated midsubstance ACL repair using the BEAR implant.
INDICATIONS: The BEAR implant is indicated for skeletally mature patients at least 14 years o

BACKGROUND AND OBJECTIVES: In the era of precision medicine, genetic test results have become increasingly relevant in the care of patients with Parkinson disease (PD). While large research consortia are performing widespread research genetic testing to accelerate discoveries, debate continues about whether, and to what extent, the results should be returned to patients. Ethically, it is imperative to keep participants informed, especially when findings are potentially actionable. However, research testing may not hold the same standards required from clinical diagnostic laboratories and hold

BACKGROUND: Snapping biceps femoris syndrome (SFS) represents a rare clinical entity in which the biceps femoris subluxates over the fibular head in deep flexion. Two primary pathophysiologies have been described including a prominent or abnormal fibular head morphology. Others have implicated an anomalous biceps femoris insertion. The diagnosis is made clinically, with operative and nonoperative intervention strategies available for treatment.
INDICATIONS: SFS often results in audible snapping and associated pain at the lateral fibular head. When recalcitrant to nonoperative management, surg

BACKGROUND: Recently, there has been an increase in interest in all-inside anterior cruciate ligament (ACL) reconstruction (ACLR). Regarding graft maturity, graft contact with the bone tunnel is important. All-inside ACLR with short femoral and tibial sockets might allow to increase the contact between the graft and the socket (ie, no interference screw and contact between the graft and bottom of the socket) and can improve graft maturing.
INDICATIONS: All-inside ACLR with short sockets technique is indicated in patients with primary ACL rupture or ACL graft re-rupture.
