Association between preoperative functional status and timing of surgery in tuberculous destroyed lung patients.

BACKGROUND: Tuberculous destroyed lung (TDL) results in severe pulmonary impairment, significantly affecting patients’ functional status and quality of life. The timing of surgical intervention for TDL remains controversial, with debate surrounding the benefits of early versus delayed surgery. This study aimed to evaluate the impact of surgical timing on functional recovery, mortality, morbidity, and cost-effectiveness in TDL patients. METHODS: Patients with TDL were allocated to early surgery (ESG, ≤ 14 days after initial optimization) or delayed surgery (DSG, after 6–8 weeks of structured pr

The experience of caregiving for children with rare musculoskeletal conditions: a qualitative study in arthrogryposis multiplex congenita.

BACKGROUND: Arthrogryposis multiplex congenita (AMC) is a group of rare musculoskeletal conditions that is associated with complex healthcare needs and long-term follow up. The literature reports significant direct, indirect, and psychosocial costs for caregivers of children with neuromuscular conditions. Due to mobility limitations and frequent hospital visits, caring for a child with AMC is complex. Other challenges experienced by caregivers include financial strain, job changes, changes in interpersonal relationships and abandonment. This study was aimed at exploring the lived experience of

Examining the effects of low back pain and mental health symptoms on healthcare utilisation and costs: a protocol for a population-based cohort study.

INTRODUCTION: Low back pain (LBP) is a leading cause of disability associated with high healthcare utilisation and costs. Mental health symptoms are negative prognostic factors for LBP recovery; however, no population-based studies have assessed the joint effects of LBP and mental health symptoms on healthcare utilisation. This proposed study will characterise the health system burden of LBP and help identify priority groups to inform resource allocation and public health strategies. Among community-dwelling adult respondents of five cycles of the Canadian Community Health Survey (CCHS) in Ont

Cost-effectiveness of microendoscopic discectomy versus conventional open discectomy in the treatment of lumbar disc herniation: a prospective randomised controlled trial [ISRCTN51857546].

BACKGROUND: Open discectomy is the standard surgical procedure in the treatment of patients with long-lasting sciatica caused by lumbar disc herniation. Minimally invasive approaches such as microendoscopic discectomy have gained attention in recent years. Reduced tissue trauma allows early ambulation, short hospital stay and quick resumption of daily activities. A comparative cost-effectiveness study has not been performed yet. We present the design of a randomised controlled trial on cost-effectiveness of microendoscopic discectomy versus conventional open discectomy in patients with lumbar

The Efficiency and Cost-Effectiveness of 3D-Printed Patient-Specific Guide Plate for Patients Undergoing Open-Wedge High Tibial Osteotomy: A Multicentered Randomized Controlled Trial.

PURPOSE: Open-wedge high tibial osteotomy (OWHTO) is established for young, active patients with medial knee osteoarthritis. Patient-specific 3D-printed guide plates have been introduced to improve surgical precision and efficiency, but evidence of clinical and economic benefit is limited. We aimed to determine whether a 3D-printed patient-specific guide plate improves efficiency, functional outcomes, and cost-effectiveness compared to standard OWHTO. METHODS: In this multicenter randomized trial, patients scheduled for OWHTO were allocated to either conventional planning (control) or surgery

Allocative efficiency of opioid overdose prevention strategies for people incarcerated in New Jersey.

BACKGROUND: People with opioid use disorder (POUD) who are incarcerated are disproportionately impacted by the overdose crisis. We sought to identify overdose policies that allocate resources with maximal efficiency to reduce mortality among POUD in the New Jersey (NJ) Department of Corrections. MAIN OUTCOMES/MEASURES: We created a probabilistic state-transition model of a simulated cohort of POUD incarcerated in NJ to simulate maximizing medication for opioid use disorder (MOUD) during incarceration and/or post-release in the community and naloxone in the community. We estimated how maximizi

Health care costs associated with chronic hepatitis C virus infection in Ontario, Canada: a retrospective cohort study.

BACKGROUND: High-quality estimates of health care costs are required to understand the burden of illness and to inform economic models. We estimated the costs associated with hepatitis C virus (HCV) infection from the public payer perspective in Ontario, Canada. METHODS: In this population-based retrospective cohort study, we identified patients aged 18-105 years diagnosed with chronic HCV infection in Ontario from 2003 to 2014 using linked administrative data. We allocated the time from diagnosis until death or the end of follow-up (Dec. 31, 2016) to 9 mutually exclusive health states using

Research Gaps and Priorities for aHUS in the Gulf Cooperation Countries (GCC): Findings from a Payer-Focused Expert Meeting.

Atypical hemolytic uremic syndrome (aHUS) is a rare, life-threatening complement-mediated thrombotic microangiopathy (TMA) associated with high morbidity, mortality, and substantial healthcare resource utilization (HCRU). Although complement inhibitors such as eculizumab and ravulizumab have dramatically improved outcomes, aHUS continues to impose significant economic and operational burden on healthcare systems. In the Gulf region, payers and health systems face increasing pressures driven by rising costs, delayed diagnosis, fragmented access, and limited real-world evidence necessary for inf

Paying family caregivers: self-direction in medicaid personal care.

INTRODUCTION: As people age, many require help with personal care-often provided by family members or friends or direct care workers. Over the last three decades, driven by the disability rights movement, direct care worker shortages, and caregiver burden, self-direction has emerged as a Medicaid option that allows individuals to hire and pay their own caregivers, including family members. METHODS: We use 2021 TMSIS Medicaid claims data linked to Medicare Master Beneficiary Summary File to identify dually eligible beneficiaries 65+ receiving personal care. We describe the demographic characte

Unlocking Relief: Insights into Physiotherapy-Led Spinal Telehealth in Correctional Facilities.

The global prison population has increased by 32% since 2000, driving higher demand for health care services within correctional facilities. Musculoskeletal disorders are prevalent among incarcerated people, with 52% diagnosed with chronic conditions. In-person treatments outside facility walls incur high costs and are further hindered by stigma. Telehealth services for musculoskeletal care offer a promising alternative to improve access, reduce costs, and enhance care quality. This article explores the impact and perspectives of physiotherapy-led telehealth consultations for spinal disorders

My grief app for prolonged grief in bereaved parents: a randomised waitlist-controlled trial.

A minority of bereaved adults experiences prolonged grief disorder, depression, and/or posttraumatic stress disorder, with heightened risks observed among bereaved parents. Cognitive-behavioural therapies, both face-to-face and online, have demonstrated efficacy in treating post-loss mental health problems. Mobile phone applications potentially offer an efficient and cost-effective way to deliver self-help to bereaved adults, yet controlled effectiveness studies are lacking. Therefore, we examined the short-term efficacy of the My Grief app, based on cognitive-behavioural therapy, in 248 berea

Research on the control strategy of DC microgrids with distributed energy storage.

As a supplement to large power grids, DC microgrids with new energy access are increasingly widely used. However, with the increasing proportion of new energy in DC microgrids, its output fluctuations directly affect the overall stability of the microgrids. Distributed energy storage can smooth the output fluctuation of distributed new energy. In this paper, an AC-DC hybrid micro-grid operation topology with distributed new energy and distributed energy storage system access is designed, and on this basis, a coordinated control strategy of a micro-grid system based on distributed energy storag

Costs and effects of screening and treating low risk women with a singleton pregnancy for asymptomatic bacteriuria, the ASB study.

BACKGROUND: The prevalence of asymptomatic bacteriuria (ASB) in pregnancy is 2-10% and is associated with both maternal and neonatal adverse outcomes as pyelonephritis and preterm delivery. Antibiotic treatment is reported to decrease these adverse outcomes although the existing evidence is of poor quality. METHODS/DESIGN: We plan a combined screen and treat study in women with a singleton pregnancy. We will screen women between 16 and 22 weeks of gestation for ASB using the urine dipslide technique. The dipslide is considered positive when colony concentration ≥105 colony forming units (CFU)

Systematic care management: a comprehensive approach to catastrophic injury management applied to a catastrophic burn injury population--clinical, utilization, economic, and outcome data in support of the model.

The new standard for successful burn care encompasses both patient survival and the burn patient's long-term quality of life. To provide optimal long-term recovery from catastrophic injuries, including catastrophic burns, an outcome-based model using a new technology called systematic care management (SCM) has been developed. SCM provides a highly organized system of management throughout the spectrum of care that provides access to outcome data, consistent oversight, broader access to expert providers, appropriate allocation of resources, and greater understanding of total costs. Data from a

Investing 101: Foundational Rules for Building Long-Term Wealth.

There is limited financial education during medical training, despite physicians attaining high lifetime incomes and facing complex financial decisions. As a result, many radiologists enter practice with limited knowledge in personal investing and wealth management. This article presents a practical, evidence-informed framework for long-term investing tailored to physicians. We propose four core principles. First, limit exposure to higher risk assets such as individual stocks and cryptocurrencies to a small portion of the portfolio, given the risks of concentration and higher volatility and ra

Decarbonizing Basic Chemicals Production in North America, Europe, Middle East, and China: a Scenario Modeling Study.

The chemicals industry accounts for about 5% of global greenhouse gas emissions today and is among the most difficult industries to abate. We model decarbonization pathways for the most energy-intensive segment of the industry, the production of basic chemicals: olefins, aromatics, methanol, ammonia, and chlor-alkali. Unlike most prior pathways studies, we apply a scenario-analysis approach that recognizes the central role of corporate investment decision making for capital-intensive industries, under highly uncertain long-term future investment environments. We vary the average pace of decarb

Transformer-based word level Bangla sign language recognition using relative quantization encoding.

Sign language recognition (SLR) for low-resource languages like Bangla suffers from signer variability, viewpoint variations, and limited annotated datasets. In this paper, we present BdSLW401, a large-scale, multi-view, word-level Bangla Sign Language (BdSL) dataset with 401 signs and 102,176 video samples from 18 signers in front and lateral views. To improve transformer-based SLR, we introduce Relative Quantization Encoding (RQE), a structured embedding approach anchoring landmarks to physiological reference points and quantizes motion trajectories. RQE improves attention allocation by decr
