BACKGROUND: A relationship between lambdoid craniosynostosis and Chiari deformations has been suggested, but the true extent of this association remains uncertain. The authors reviewed a single center's experience treating lambdoid synostosis to further elucidate this relationship, examine surgical outcomes, and identify associations that might impact future treatments.
METHODS: A retrospective chart review was performed of all patients treated for lambdoid craniosynostosis, excluding the syndromic craniosynostoses. Operative data, scans, hospitalization, and subsequent surgical procedures we

The results of colonic resections were investigated in 356 patients in a retrospective analysis. Special attention was directed to the effectiveness of a transrectal tube, leading to decompression of the anastomosis for the first five days. Under elective conditions, the overall complication-rate was 12.7%. In emergency cases, these complications were 51.5% (one third of these cases had a decompression-tube). The clinical relevant leakage-rate under elective conditions ranged to 1.7%. Postoperative mortality related to surgical complications turned out to be 1.5% under elective conditions. The

There are various approaches and surgical techniques with the objective of nerve root decompression, restrict mobility, and fusion of the listhesis. Among the techniques, posterior interbody fusion combines direct and indirect root decompression with the fusion between vertebral bodies, placing an autologous bone graft between transverse apophysis and vertebral bodies. Transforaminal lumbar and posterior interbody fusion, on the same way, look to decompress and fuse but with a different approach to the spine. The anterior approach for interbody fusion provides a better fusion rate. Lateral lum

From 2005 to 2011, 23 of 178 (12.9%) patients with venoarterial (VA) extracorporeal membrane oxygenation (ECMO) had left atrial (LA) decompression to help improve left ventricular (LV) function, LA/LV dilatation, and/or lung edema. LA decompression was achieved with LA cannulation (n = 16), surgically created adjustable atrial septal defect (n = 3), or balloon atrial septostomy (n = 4). Sixteen (70%) patients had LA decompression at the time of ECMO initiation and all had LA decompression within 12 hours of ECMO initiation. ECMO duration was 5.9 ± 4.5 days and 16 (70%) patients were successful

BACKGROUND AND IMPORTANCE: Basilar artery (BA) dolichoectasia is a rare vascular anomaly characterized by elongation, dilatation, and tortuosity of the BA. A dolichoectatic BA can cause neurovascular compression of the trigeminal nerve, resulting in classical trigeminal neuralgia (TN). Diagnosis and surgical management are often challenging due to the complex anatomy and the frequent presence of multiple neurovascular conflicts.
CLINICAL PRESENTATION: We report the case of a 70-year-old man with multiple comorbidities and an 8-year history of right-sided TN refractory to pharmacological and i

A four-level thoracic disc prolapse is extremely rare, especially when surgical intervention is required. The level of the disc and the herniation's direction determine the surgical approach of selection. A 46-year-old male presented with a two-week history of severe, sudden back pain and weakness in his lower extremities. Clinical presentation was suggestive of thoracic radiculopathy and myelopathy. Magnetic resonance imaging confirmed four-level disc prolapses at the levels of T9-T10, T10-T11, and T11-T12, which were central and in both directions. Decompression and fixation were done throug

We analyzed 124 papers published in the English language literature to define the indications and timing of surgery in spinal TB and to evaluate the outcome of various surgical procedures for kyphosis and neural outcome. Surgery in spinal tuberculosis is indicated for diagnostic dilemma, neural complications, and prevention of kyphosis progression. Up to 76% canal encroachment is compatible with a normal neurologic state as the spinal cord tolerates gradually developing compression. Patients with relatively preserved cord size, but with edema/myelitis and predominantly fluid compression on MRI

OBJECTIVE: Chronic cluster headache occurs in less than 10% of cluster headache sufferers, but remains an intractable medical problem. Surgical treatments have also been limited in their effectiveness. The authors describe their experience with attempted surgical amelioration of chronic cluster headache.
DESIGN: Twenty-eight patients, including two with bilateral cluster headache, underwent 39 operations for microvascular decompression of the trigeminal nerve, alone or in combination with section and/or microvascular decompression of the nervus intermedius. Follow-up averaged 5.3 years.

The concept of enlarging the orbital space in some patients with Graves' ophthalmopathy is usually acceptable. Controversial, however, are 1. the indications for enlarging the space, 2. the sequence of therapy that is most successful in restoring the eyes to near normal, and 3. the value of enlarging the orbital space and its relationship to non-surgical alternatives. Since 1669 a personal experience with more than 600 eyes in over 300 patients using the transantral route to orbital decompression has accumulated. From the surgical experience plus exposure to other non-surgically treated patien

Lumbar foraminal stenosis is a common cause of intractable radiculopathy that may require surgical treatment when conservative management is ineffective. Transforaminal endoscopic lumbar foraminotomy (TELF) has been developed as a minimally invasive alternative to conventional open surgery, offering reduced tissue disruption and faster recovery. However, conventional TELF techniques may provide limited decompression in complex cases involving severe bony stenosis, fibrosis, or postoperative anatomical changes. This protocol describes circumferential TELF (C-TELF), an advanced endoscopic decomp

BACKGROUND: There are currently no intraoperative neurophysiological tools to assess the effectiveness of trigeminal nerve decompression during microvascular decompression surgery for drug-resistant trigeminal neuralgia. In microvascular decompression surgery for hemifacial spasm, an abnormal electromyographic activation of facial muscles after stimulation of the offending vessel was identified and named 'Z-L response'.
METHODS: We adapted a neurophysiological protocol to elicit a Z-L response during microvascular decompression surgery for trigeminal neuralgia and applied it to a prospective

BACKGROUND: Postoperative new onset diplopia can be a disadvantage for surgical orbital decompression in patients with exophthalmos in thyroid eye disease. The various modifications of decompression (number and combination of walls) differ in their influence on the postoperative squint angle. We report on postoperative diplopia in a modified 2 wall decompression strategy (lateral wall and floor).
METHODS: This study was a retrospective analysis of 36 consecutive 2-wall decompressions performed between 2006-2010 in 24 patients with 6 months of stable exophthalmos in thyroid eye disease after m

OBJECT: Several surgical procedures have been developed to treat thoracic OPLL (ossification of the posterior longitudinal ligament). However, favorable surgical results are not always achieved, and consistent protocols and procedures for surgical treatment of OPLL in this region have not been established. Beak-type OPLL in the thoracic spine is known to be the most complicated form of OPLL to treat surgically. In this study, the authors examine the clinical outcomes after anterior decompression via a posterolateral approach for beak-type OPLL in the thoracic spine and address the gradual spin
