Hand eczema continues to bedevil both patients and physicians. While appropriate and judicious intervention minimizes aggravation, discomfort, and inconvenience, recurrences are frequent. Treatment with corticosteroids is often effective for irritant dermatitis. Efforts to avoid the irritant, protect the hands, and use emollients are vital in preventing recurrence. Protection and avoidance are also the key techniques to teach patients with allergic dermatitis. In the case of pompholyx, experts now believe the condition is caused by stress or emotional turmoil, so the most helpful intervention …

PURPOSE: This study aimed to describe the ocular surface disorder in patients with atopic dermatitis (AD).
DESIGN: A prospective case-controlled study.
PARTICIPANTS: A total of 44 patients with active AD seen at Kobe University School of Medicine, Department of Ophthalmology, during 1994 through 1996 and 22 normal control subjects were studied.
INTERVENTION: The subjects underwent routine ophthalmic examinations, tear film break-up time (BUT), Schirmer test, and conjunctival impression cytology.
MAIN OUTCOME MEASURES: Patients and control subjects were compared for tear function parameters…

We examined the frequency and types of ocular complications that developed in 560 eyes of 280 patients with atopic dermatitis at the Department of Dermatology, Tokyo Medical College Hospital. Mild ocular involvement included blepharitis in 294 eyes (52.5%), conjunctivitis in 221 eyes (39.5%), and superficial punctate keratopathy in 65 eyes (11.6%). More serious ocular complications leading to decreased visual function occurred in 133 eyes (23.8%) that developed lens opacities, and 34 eyes (6.0%) that developed retinal break or retinal detachment. Because of the presence of abnormalities in the…

We have studied the occurrence of associated diseases in a cohort of 305 patients with dermatitis herpetiformis (DH) followed up for a mean of 10 years. The results were compared with those from 383 patients with coeliac disease (CD). Twenty-nine (9.5%) patients with DH and 73 (19.1%) with CD had concomitant endocrine or connective tissue disorders. The following associations were found: auto-immune thyroid disease (4.3% of DH patients and 6.0% of CD patients), insulin dependent diabetes (1.0% DH and 5.5% CD), lupus erythematosus (1.3% DH and 0.3% CD), Sjögren's syndrome (1.0% DH and 2.9% CD),…

Although pyoctanin is widely used in medicine, there are hardly any reports of contact allergy to it. Most reports of side-effects refer to irritant reactions after topical application. We describe 2 patients who developed pyoctanin contact dermatitis after long-term application on damaged skin. Positive patch test results down to concentrations of 0.0025% and 0.05% aqueous pyoctanin solution, respectively, were found.

Atopic dermatitis (AD) is the most common chronic skin disorder in infancy and childhood and is the main hallmark of atopic constitution. The disease is multifactorial, and although genetic predisposition is certainly a prerequisite, a number of environmental factors modulate the phenotypic expression of AD. The majority of affected children shows IgE sensitisation towards a large variety of foods and aeroallergens. Since at least 1600, it has been recognized that patients with AD have a high predisposition to develop asthma. Recent epidemiological studies show that AD is commonly seen in indi…

The aim of this study was to investigate the frequency of sensitization to fatty alcohols in a group of patients with suspected cosmetic or medicament contact dermatitis. From May 1992 to September 1995, we patch tested a series of 5 fatty alcohols on 146 patients. These included 108 females and 38 males aged from 13 to 72 years (mean age 42.5). These patients, who had previously been tested with the GIRDCA standard series, were selected because their clinical lesions or histories indicated topical preparations as the possible source of their contact dermatitis. High-grade fatty alcohols (> 99…

A case of occupational contact dermatitis to gold is presented. This is uncommon as gold is relatively insoluble. Allergic contact dermatitis to gold is confirmed by patch testing with gold sodium thiosulfate 0.5% in petrolatum. Occupational contact dermatitis in the gold industry can be reduced by providing workers with more protective gear and better ventilation.

Dermatitis herpetiformis (DH) and coeliac disease (CD) are gluten-sensitive diseases which have a common immunogenetic background, with the histocompatibility locus antigen (HLA) alleles DQ A1*0501 and B1*0201 in the short arm of chromosome 6. CD is well known to cluster in families whereas DH has not been generally regarded as a familial disease. To study the familial incidence of DH, a prospective study was started in 1969 at the Department of Dermatology of Helsinki University Hospital and in 1976 at the Department of Dermatology of Tampere University Hospital. A total of 1018 patients with…

Perianal dermatitis caused by Streptococcus pyogenes (beta-haemolytic Lance-field group A) was diagnosed in four children, three boys aged I, 6 and 8 years and a girl aged 2 years. Perianal itching in children is most frequently attributed to enterobiasis (Enterobius vermicularis); fissures due to scratching can complicate this situation. Perianal streptococcal dermatitis can effectively be treated with oral penicillin, in combination with local antibiotic application (fucidin, mupirocin).
