An 8-year retrospective review of patch testing with rubber allergens: the Mayo Clinic experience.

BACKGROUND: Allergic contact dermatitis from rubber chemicals is increasingly recognized. OBJECTIVE: To review the results of patch testing with rubber allergens from January 1, 2000, through December 31, 2007. METHODS: Patients who underwent patch testing with a specialized series of rubber allergens were identified. RESULTS: In total, 773 patients (64.2% female; mean age, 48.6 years) were patch-tested with a rubber series (27 allergens), and 739 (95.6%) were concomitantly patch-tested with a standard allergen series. Commonly affected sites of dermatitis were the hand (49.7%), foot (15.9%

Occupational contact allergy to components of polyester resin systems.

BACKGROUND: Allergic contact dermatitis caused by polyester resin is considered to be rare. OBJECTIVES: To describe 11 new patients diagnosed with occupational contact allergy to components of polyester resin systems in 1994-2009. METHODS: Data on occupations, patch test results, symptoms and exposure to polyester products were retrieved from patient files, covering 1994 to February 2015. RESULTS: Five patients reacted to unsaturated polyester resin putties. In 3 of these cases, putties were considered to be the main cause of the dermatitis. Four patients had cobalt allergy resulting from c

A scoping review to identify strategies that work to prevent four important occupational diseases.

BACKGROUND: Despite being largely preventable, many occupational diseases continue to be highly prevalent and extremely costly. Effective strategies are required to reduce their human, economic, and social impacts. METHODS: To better understand which approaches are most likely to lead to progress in preventing noise-related hearing loss, occupational contact dermatitis, occupational cancers, and occupational asthma, we undertook a scoping review and consulted with a number of key informants. RESULTS: We examined a total of 404 articles and found that various types of interventions are report…

Occupational relevance of positive standard patch-test results in employed persons with an initial report of an occupational skin disease.

OBJECTIVES: It is commonly accepted that the standard screening tray is an essential diagnostic test in patch testing supporting the diagnosis of contact dermatitis, the most common type of occupational skin disease (OSD). In this study standard patch-test results of employed persons with an initial report of an OSD were analyzed within 24 occupational groups. METHODS: An evaluation was made of employed persons recorded in the Register of Occupational Skin Diseases in Northern Bavaria (Berufskrankheitenregister Haut-Nordbayern; BKH-N) between 1990 and 1999, catering for those standard screeni

Skin diseases in musicians.

Instrumental musicians are a risk group for skin diseases. A systematic review was performed on Pubmed database and in the musical literature. Most publications on dermatoses in musicians are case reports. The exact prevalence of skin diseases in musicians is unknown but high rates have been reported. The most at-risk musicians are percussionists, string and wind instrumentalists. Repeated physical trauma is a frequent cause of skin conditions in musicians (callosities, fiddler's neck syndrome…). The allergens most often reported in musicians' allergic contact dermatitis are metals (nickel, di

Unintended cutaneous reactions to CS spray.

CS spray (2-chlorobenzylidene malononitrile 5% w/v in methyl isobutyl ketone) has been used by the police force in the UK as an incapacitant for nearly a decade. It causes a number of well-recognized cutaneous reactions, which are generally regarded as short-lived. These include skin burning, erythema and blistering. However, a range of unpredictable cutaneous reactions to CS spray may also occur. We have found contact allergy, leukoderma, initiation or exacerbation of seborrhoeic dermatitis, and aggravation of rosacea following CS spray exposure in 6 police officers and 1 doorman. These skin

Managing Contact Dermatitis Without Patch Testing.

Allergic contact dermatitis (ACD) is a common skin condition caused by contact with an exogenous agent that elicits an inflammatory response. Patch testing (PT) is considered the gold standard for diagnosing ACD. Unfortunately, PT may not be available to some patients due to insurance and financial limitations, contributing to health care disparity and leaving patients with undiagnosed, incompletely managed dermatitis that can have further detrimental health and occupational effects. For other patients, PT is precluded by lack of availability of specialist/expert care, comorbid medications, or

Essential Oils, Part IV: Contact Allergy.

Nearly 80 essential oils (including 2 jasmine absolutes) have caused contact allergy. Fifty-five of these have been tested in consecutive patients suspected of contact dermatitis, and nine (laurel, turpentine, orange, tea tree, citronella, ylang-ylang, sandalwood, clove, and costus root) showed greater than 2% positive patch test reactions. Relevance data are generally missing or inadequate. Most reactions are caused by application of pure oils or high-concentration products. The clinical picture depends on the responsible product. Occupational contact dermatitis may occur in professionals per

Isothiazolinone derivatives and allergic contact dermatitis: a review and update.

Allergic contact dermatitis (ACD) from isothiazolinones has frequently been described in the literature. Following an epidemic of sensitization to methylchloroisothiazolinone/methylisothiazolinone (MCI/MI) in the 1980s, and more recently to MI, the Scientific Committee on Consumer Safety of the European Commission banned their use in leave-on products, while restricting that in rinse-off cosmetics. Despite a decreasing prevalence of ACD from MCI/MI and MI, cases caused by occupational exposure and non-cosmetic isothiazolinone sources are on the rise. Moreover, sensitization to newer and lesser

How irritant is water? An overview.

Water is a skin irritant which deserves attention because of its ubiquity. During the Vietnam war, soldiers suffered from painful swollen feet, so-called tropical immersion foot. In occupational dermatology, the importance of water as a skin irritant is especially appreciated. The irritancy of water has been demonstrated by occlusion experiments; occlusion with either closed chambers or water-soaked patches has been shown to produce clinical and histopathological inflammation. Functional damage, as revealed by increased transepidermal water loss, has also been shown. Repeated water exposure wi

[Prospective study of the importance of constitutional parameters in the development of eczema in hairdressers and construction workers].

The considerable rate of professional hand eczema in hairdressers, especially in young ones, emphasizes the necessity of practicable methods for early diagnosis of risk and manifestation of eczema. Therefore, we investigated different constitutional parameters of the skin (hyperhidrosis, dyshidrosis and acrocyanosis of the hands, seborrhea, sebostasis, white dermographism) in a prospective study of 210 hairdressers' and 95 bricklayers' apprentices over 3 years. In addition, the alkali resistance and the permeability of the horny layers (by nitrazine yellow) were examined every year in each of

Occupational allergic diseases among harvesting fishermen on the open sea: A systematic review.

BACKGROUND: Nearly 60 million people work in the fishing and aquaculture sectors worldwide and are exposed to specific allergens. Some reviews have been published in occupational allergic diseases in seafood workers but none in fishermen. OBJECTIVE: To describe the morbidity and main causal agents of allergic diseases among harvesting fishermen. METHODS: A protocol with predefined objectives and inclusion criteria was used in accord with the Preferred Items for Reporting Systematic Reviews and Meta-analyses-Protocols statement. Population was defined as harvesting fishermen, and the conditio

Skin protection in the metal industry.

In the metal industry, workplaces with a high skin risk used to be frequent. This has changed in recent years in big plants, where technological progress and automatization have eliminated many hazards. However, in many small and middle-sized companies without supervision by an occupational physician, high-risk workplaces may still prevail. Therefore skin protection plays an important role in this industry, with its focus on irritant exposures. From our own studies and campaigns, the following conclusions can be drawn: prework occupational examination and counselling of high-risk groups, espec

The importance of occupational skin diseases in the United States.

Occupational skin diseases and disorders (OSDs) are the most commonly reported non-trauma-related (acute or cumulative) category of occupational illnesses in the United States. This factor, along with their potential chronicity, their effect on an individual's vocational and avocational activities, and the fact that they are preventable, point out the public health importance of OSDs. It can be difficult to obtain accurate epidemiological data for OSDs in the US, and all sources have their limitations. OSD cases that result in days away from work are important categories to study, since days a

Hand eczema: a review of clinical, dermoscopic and histological features.

INTRODUCTION: Hand eczema (HE) is a prevalent inflammatory skin condition, impacting 15-20% of individuals, with a notable incidence rate of approximately 7.3 cases per 1000 person/years. This condition exhibits significant gender-based variability, with a higher prevalence in females. The clinical presentation of HE is characterized by pruritic erythematous, edematous, weeping plaques, vesicles, and/or bullae, showcasing considerable heterogeneity. EVIDENCE ACQUISITION: A literature search was conducted across multiple databases, including Medline, Pubmed, Scopus, and the Cochrane Library. T

[Work-related diseases of the skin].

Work-related diseases of the skin are aetiologically completely different diseases. The common denominator is that they are all triggered by occupational activity. Allergic and cumulative irritant contact eczema occur the most frequently. Early reporting of occupational dermatosis to the accident insurance carrier should first and foremost lead to adequate preventive measures at the workplace. Early recognition, systematic therapy and rapidly implemented prevention should enable the affected person to continue to work in his or her occupation. To implement appropriate preventive measures, the
