The magnetic resonance appearance of extraperitoneal bowel perforation: a case report.

The use of magnetic resonance imaging (MRI) for abdominal and gastrointestinal imaging has been quite limited, when compared to its utilization in other organ systems. We present an interesting case of extraperitoneal bowel perforation, which was diagnosed by magnetic resonance (MR). Extraperitoneal bowel perforation is frequently a confusing and difficult diagnosis to make. The multiplanar imaging capability and unique sensitivity of MR to fluid enabled a specific diagnosis to be made in this challenging case.

The initial management of left lower quadrant peritonitis.

The majority of patients admitted to the hospital with left lower quadrant peritonitis and suspected acute diverticulitis are treated empirically without colon radiography either for the duration of their hospital course, or until such time as it is deemed safe to perform a barium enema to confirm this diagnosis. Unlike barium, a water-soluble contrast agent such as Gastrografin can be used safely and accurately immediately on hospitalization. Early establishment of an accurate diagnosis can help eliminate complications, morbidity, mortality, and lengthy hospitalizations. Seventy-one patients

Single-incision Laparoscopy Versus Multiport Laparoscopy for Colonic Surgery: A Multicenter, Double-blinded, Randomized Controlled Trial.

OBJECTIVE: To compare outcome of single-port laparoscopy (SPL) and multiport laparoscopy (MPL) laparoscopy for colonic surgery. SUMMARY OF BACKGROUND DATA: Benefits of SPL over MPL are yet to be demonstrated in large randomized trials. METHODS: In this prospective, double-blinded, superiority trial, patients undergoing laparoscopic colonic resection for benign or malignant disease were randomly assigned to SPL or MPL (NCT01959087). Primary outcome was length of theoretical hospital stay (LHS). RESULTS: One hundred twenty-eight patients were randomized and 125 analyzed: 62 SPL and 63 MPL, in

'Pointing' in the wrong direction--a case of diverticulitis presenting at the hip.

We report the case of a 73-year-old lady who was seen with an infected sinus leaking from the wound of a recently inserted right total hip joint prosthesis. Plain radiographs revealed gas shadows in the region of the wound. An intra-operative sinogram showed the sinus to be coming from a pelvic diverticular abscess and the hip to be an innocent bystander. Our case emphasises the importance of having a clear preoperative plan with relevant investigations before a radical operative procedure is undertaken, to avoid unnecessary risk to a non-infected hip prosthesis.

Conservative treatment of selected patients with colovesical fistula due to diverticulitis.

Thirty patients with colovesical fistula due to diverticulitis were encountered in the past ten years. Six patients did not receive any of the standard operative therapies. Four of these patients were observed for three to 14 years with nonoperative management, with little inconvenience and without significant complications. Five of the 24 patients in the surgical group died in the postoperative period. Nonoperative therapy seems to be a viable option in selected patients.

Laparoscopy decreases anastomotic leak rate in sigmoid colectomy for diverticulitis.

BACKGROUND: Early studies comparing laparoscopic and open operations for diverticulitis failed to show any advantages of the laparoscopic approach. Our study compared the 30-day postoperative outcomes of laparoscopic and open sigmoid colectomy for diverticulitis by surgeons who had performed 20 or more laparoscopic colectomies before the study period. HYPOTHESIS: Patients who undergo an elective laparoscopic operation for diverticulitis have reduced postoperative complications compared with patients who have a traditional open operation. DESIGN: Retrospective analysis. SETTING: Academic med

Reversal of Hartmann's procedure after surgery for complications of diverticular disease of the sigmoid colon is safe and possible in most patients.

INTRODUCTION: Febrile neutropenia is one of the most serious complications in patients with hematological malignancies and chemotherapy. Channa striata is a freshwater fish belonging to the family Channidae. This study aims to determine whether the administration of channa striata extract can increase neutrophil count, neutrophil function and prevent incidence of febrile neutropenia in acute myeloid leukemia (AML) patients receiving chemotherapy. METHOD: A randomized, single-blind, controlled study compared a standard nutrition from hospital with a channa striata extract-supplemented per oral

Laparoscopic sigmoid colectomy after acute diverticulitis: when to operate?

BACKGROUND: Laparoscopic sigmoid colectomy has become an acceptable method of surgical treatment for diverticulitis. However, an optimal waiting period before attempting elective laparoscopic colectomy has not been established. We sought to evaluate the relationship between the time interval from an acute episode of diverticulitis to laparoscopic colectomy and surgical outcomes. METHODS: All patients undergoing laparoscopic colectomy during a period of 10 years in a single institution were studied. Retrospectively collected data included patient demographics, American Society of Anesthesiolog

Gastrointestinal disorders: the role of diet in cause and management.

Diet has a definite role in certain gastrointestinal disorders, and dietary manipulation often is beneficial. In other cases, such as peptic ulcer, traditional dietary therapy cannot be justified by available evidence.

Acute diverticulitis management: evolving trends among Italian surgeons. A survey of the Italian Society of Colorectal Surgery (SICCR).

Acute diverticulitis (AD) is associated with relevant morbidity/mortality and is increasing worldwide, thus becoming a major issue for national health systems. AD may be challenging, as clinical relevance varies widely, ranging from asymptomatic picture to life-threatening conditions, with continuously evolving diagnostic tools, classifications, and management. A 33-item-questionnaire was administered to residents and surgeons to analyze the actual clinical practice and to verify the real spread of recent recommendations, also by stratifying surgeons by experience. CT-scan remains the mainstay

Diverticular disease at colonoscopy in Lagos State, Nigeria.

BACKGROUND: The upsurge in the reported cases of diverticular disease (DD) has led to a re-appraisal of the earlier held views that it was a rare entity in Nigeria. The advent of colonoscopy has contributed in no small way to this change. We sought to determine the clinical characteristics, indications for colonoscopy, and intra-procedural findings among these patients. MATERIALS AND METHODS: A retrospective cross-sectional study was carried out on the colonoscopy records from four private endoscopy units based in Lagos State, Nigeria. The records were drawn from a 5-year period (August 2010

[Treatment of colovesical fistula: predictive factors of the maintenance of long-term digestion continuity].

AIM OF THE STUDY: Colovesical fistulas (CVF) may occur in inflammatory or tumoral pelvic diseases. The aim of this study was to analyze the surgical management of patients with a CVF in order to define predictive factors of good results and long term digestive continuity. PATIENTS AND METHODS: From 1989 to 1999, this retrospective study included 37 patients, 19 men and 18 women, mean age: 69 years (range 37-93 years). Main etiologies were colonic diverticulitis (n = 22) in 60% of the patients, cancer (n = 6) in 16%, previous radiotherapy (n = 5) in 14%. Treatment was a diverting colostomy in

Smoking and alcohol consumption as risk factors for colonic diverticulosis and its complications: Systematic review and meta-analysis.

BACKGROUND: Colonic diverticulosis (CD) is a prevalent condition marked by small pouches (diverticula) in the colon wall, particularly in those over 50. Many studies have investigated various risk factors, but the relationship between smoking, alcohol consumption, and CD remains to be decided. This meta-analysis analyses the association between these lifestyle factors and the risk of CD and its complications. METHODS: A comprehensive literature search was conducted using PubMed, Embase, and Google Scholar to find relevant studies. Inclusion criteria were based on predefined eligibility standa
