Aspiration of the elbow joint for treating radial head fractures.

BACKGROUND: Radial head fracture is the most common fracture of the elbow. It usually results from a fall onto an outstretched arm. In 1954, Mason classified these fractures into type 1 (undisplaced), type 2 (simple displaced), and type 3 (comminuted fractures). Aspiration of the elbow joint aims to relieve pressure in the elbow joint and has been used as an initial treatment option for radial head fractures. However, it is an invasive technique with the potential for complications such as infection and injury to nerves and vessels. OBJECTIVES: To assess the effects (benefits and harms) of el…

[Hinged external fixation in orthopaedic and trauma surgery of the elbow].

PURPOSE OF THE STUDY: A hinged external fixator of the elbow provides stable fixation of the joint while maintaining the range of its motion. The aim of the study was to evaluate a group of patients in whom an external fixator was used to manage severe injuries to the elbow, namely, traumatic unstable dislocation, unstable fracture-dislocation, and elbow fractures not permitting management by primary osteosynthesis. This also involved assessment of early elbow mobilisation and a comparison of this group with a group of patients treated conservatively for less severe elbow injuries. MATERIAL A…

Traumatic instability of the elbow - anatomy, pathomechanisms and presentation on imaging.

UNLABELLED: Traumatic instability of the elbow is a common problem among patients presenting to emergency departments. As recurrent instability is more common than previously thought and the knowledge of the most frequent underlying causes increases, treatment of elbow instability is becoming increasingly sophisticated. Therefore, it is mandatory for the radiologist to be familiar with the important osseous and soft-tissue stabilisers of the elbow joint and to know the pathomechanisms that lead to a spectrum of joint instability. This article discusses the relevant anatomy in elbow stability w…

Treatment of ulnar neuropathy at the elbow: cost-utility analysis.

PURPOSE: The choice of surgical treatment for ulnar neuropathy at the elbow (UNE) remains controversial. A cost-utility analysis was performed for 4 surgical UNE treatment options. We hypothesized that simple decompression would emerge as the most cost-effective strategy. METHODS: A cost-utility analysis was performed from the societal perspective. A decision analytic model was designed comparing 4 strategies: (1) simple decompression followed by a salvage surgery (anterior submuscular transposition) for a poor outcome, (2) anterior subcutaneous transposition followed by a salvage surgery for…

Laxity of the ulnar nerve during elbow flexion and extension.

PURPOSE: To evaluate the dynamic anatomy of the ulnar nerve at the elbow. METHODS: We studied 11 fresh cadavers. We placed metal clips on the ulnar nerve at three locations: at the medial epicondyle (point A), 3 cm proximal to the epicondyle (point B), and 14 cm proximal to the epicondyle (point C). The distances from the medial epicondyle to points A, B, and C on the ulnar nerve and between each pair of points were measured in full elbow extension and flexion. RESULTS: With full elbow flexion, there was no movement of the ulnar nerve at point A (adjacent to the medial epicondyle). Point A a…

Posttraumatic elbow arthritis in the young adult: evaluation and management.

Degenerative joint disease following trauma to the elbow is difficult to manage in any patient. However, this condition becomes substantially more challenging in the young, active population. Increased activity demands and limited functional capacity of total elbow arthroplasty mean that joint arthroplasty should be regarded as a salvage procedure. The primary goal of treatment is to restore a pain-free or minimally painful functional joint while preserving future surgical options. This requires accurate assessment of the primary patient complaint, be it terminal pain and stiffness or pain alo…

[Orthopaedic treatment of floating elbow in a child: a case report].

The floating elbow in the child is rare and serious. The authors report a traumatic case of floating elbow without vascular and nervous lesions associated in a 6-year-old child. We treated these both fractures of the distal forearm and the supracondylar humeral by respectively cast plaster and a collar and cuff with an anatomical reduction. The radiological and clinical results were excellent with the 29 months follow-up.

The triceps-flexor carpi ulnaris (TRIFCU) approach to the elbow.

INTRODUCTION: Intra-articular fractures of the distal humerus frequently require internal fixation. Several approaches have been described, … ing rapid recovery of extensor function. Additionally, because of the natural carrying angle of the elbow, repositioning of the reflected triceps aponeurosis is easy.

Surgical treatment of posterolateral rotatory instability of the elbow in children.

Posttraumatic instability of the elbow in children can be a very challenging problem to diagnose and treat. Posterolateral rotatory instability (PLRI) has been well described with regard to diagnosis and treatment in adults but best clinical practices are less clear for pediatric cases. Children with PLRI can present with unusual signs and symptoms and poorly documented, vague trauma. The need for and safety of formal reconstruction of the lateral ulnar collateral ligament in children has been somewhat controversial in the past; however, with careful consideration the diagnosis of PLRI in chil…

Clinical concepts for treatment of the elbow in the adolescent overhead athlete.

Injuries to the adolescent elbow are common because of the repetitive overuse inherent in many overhead sport activities. The management of these patients is greatly facilitated through a greater understanding of the demands placed on the upper extremity kinetic chain during these overhead activities as well as a detailed examination and rehabilitation for the entire upper extremity kinetic chain. Particular emphasis on improving rotator cuff strength and muscular endurance, along with scapular stabilization, is a critical part of elbow rehabilitation in these patients. In addition, the use of…

Posteromedial dislocation of the elbow with lateral condyle fracture in children.

Posteromedial dislocation of the elbow with lateral condyle fracture is a rare injury, and only a few cases have been reported in the literature. We report 3 children who had posteromedial dislocation of the elbow with Milch type II lateral condylar fracture of the distal humerus. Major complications included malunion, nonunion of the fracture, heterotophic calcification and elbow subluxation. From our experience and a review of the literature, open anatomical reduction and internal fixation of the lateral condyle fracture is the mainstay of treatment for these children. Poor reduction of the …

Elbow arthroplasty for the treatment of bicolumn distal humeral fractures.

Distal humeral fractures are difficult to treat in elderly patients. Recent advances in precontoured and locked plates have made open reduction and internal fixation easier, but rigid and anatomic reconstruction is required for a successful result. In elderly patients with poor bone quality and an unreconstructable articular surface, total elbow arthroplasty can be a successful alternative to open reduction and internal fixation.
