Standardized sonographic examination of pediatric elbow injuries is an effective screening method and improves diagnostic efficiency.

INTRODUCTION: Positive role of ultrasound in the diagnosis of pediatric elbow injuries were confirmed by many papers but no comprehensive, standardized method has been developed for daily clinical practice. The aim of our prospective diagnostic study was to prove the efficacy of a five point sonographic point of care method for detecting different pediatric elbow fractures or dislocations. METHODS: Between 2016 January and 2017 March 365 children (age 1-14) with suspected closed elbow injury were enrolled in our study. Sonographic point of care examination was carried out by a properly traine…

Approach to pediatric patients with elbow injuries in the emergency department.

Pediatric elbow injuries are a common complaint presenting to emergency departments. The unique anatomy and skeletal immaturity in children can make the elbow particularly difficult to evaluate and manage. Additionally, many healthcare institutions have limited or no access to pediatric orthopedic specialists, leaving management decisions to the clinician at the bedside. This review highlights the nuances of the pediatric elbow examination, addressing both acute and subacute (overuse) injuries. Common fracture types are reviewed, and key principles of x-ray interpretation are demonstrated, inc

Total elbow arthroplasty for primary osteoarthritis.

BACKGROUND: Primary osteoarthritis of the elbow is a less common indication for total elbow arthroplasty (TEA). Higher complication rates in younger, active patients may offset short-term improvements in pain and function. The purpose of this study was to determine pain relief, functional outcomes, complications, and survival of TEA in this population. METHODS: Between 1984 and 2011, 20 consecutive TEAs were performed for primary elbow osteoarthritis. Two patients died before the 2-year follow-up. Mean age at surgery was 68 years (range, 51-85 years). Outcome measures included pain, motion, M

[Comparison of two techniques for lateral collateral ligament complex repair in treatment of terrible triad of elbow].

OBJECTIVE: To compare the effectiveness of transosseous tunnel fixation and drilling fixation for repair of lateral collateral ligament complex (LCLC) in treatment of terrible triad of elbow (TTE). METHODS: A clinical data of 50 patients with TTE between June 2012 and January 2018 were retrospectively analyzed. The LCLC was repaired with transosseous tunnel fixation in 22 patients (transosseous tunnel fixation group) and with drilling fixation in 28 patients (drilling fixation group). There was no significant difference between the two groups ( P>0.05) in gender, age, fracture side, time from

Simultaneous ipsilateral distal radius and radial head fractures: Two case reports of radius bipolar fracture.

RATIONALE: Distal radius fracture with simultaneous ipsilateral radial head fracture is a very rare pattern of injury. This type of injury is referred to as 'radius bipolar fracture'. Treatments for this injury pattern can be challenging because both the wrist and elbow need to be considered. There are currently no guidelines for the treatment of this specific type of injury. We report two cases of this unusual pattern of injury treated in our hospital. PATIENT CONCERNS: Case 1 was a 78-year-old female patient and case 2 was a 19-year-old female patient who visited our emergency department wi

Functional outcomes and complications of distal humerus hemiarthroplasty performed in acute versus salvage cases: a systematic meta-analysis.

BACKGROUND: Distal humeral hemiarthroplasty (DHH) is a treatment option for comminuted joint fractures of the elbow, particularly when osteosynthesis is not possible or has failed. This meta-analysis aims to compare the clinical and functional outcomes of DHH performed for distal humerus fractures acutely versus as salvage after failure of osteosynthesis (open reduction and internal fixation [ORIF]). METHODS: A systematic review was conducted according to Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines. Studies reporting cases of DHH for complex distal humerus fr

Bilateral Monteggia fracture in adults.

INTRODUCTION: In 1814 Giovanni Monteggia first described two cases of fractures of the proximal third of ulna with dislocation of the radial head. These fractures are more common in children than in adults, and mutual Monteggia fracture is a rare complication. This study presents a treatment course of a patient with bilateral Monteggia fracture. CASE REPORT: A 55-year-old patient was injured by falling in the yard. Radiography showed bilateral Monteggia fracture type II (by the Badon classification). Operative treatment of fracture was done by a compression plate on the right side and by the

Engagement of the Radial Head in Posterior Capitellum Fracture: Defining the Critical Point.

INTRODUCTION: Elbow dislocation is usually treated conservatively with a good outcome. When there is associated injury to the capitellum, "Osborne-Cotterill Lesion," then the clinical course is less predictable and controversial. In this article, we attempt to define the critical point on the capitellum surface that, when breached, forebodes potential engagement of the radial head in the defect and imminent dislocation. CASE REPORT: We present two cases of fracture and fracture dislocation of the elbow with "Osborne-Cotterill lesion." They were treated with fracture reduction and fixation. In

Monteggia and equivalent lesions. A study of 41 cases.

Forty-one patients who suffered from a Monteggia fracture and were treated between 1984 and 1993 were reviewed retrospectively using a new motion score of the elbow joint. There were 14 children and 27 adults of whom 34 were available for review. The results in children were significantly better than those of the adults. Among the adults, the final results of the patients who had a Bado Type 1 equivalent injury were significantly worse than those of the other groups, especially when associated with a fracture of the radial head. The type of fracture, open or closed, the presence of a fracture

Fracture Risk After Corticosteroid Injections in Non-geriatric Patients With Upper Extremity Osteoarthritis: A Retrospective Analysis.

BACKGROUND: Corticosteroid injections (CSIs) are commonly used for symptomatic management of upper extremity osteoarthritis (OA). While systemic corticosteroids are associated with impaired bone health, the fracture risk of intra-articular CSIs in younger patients remains unclear. This study aimed to determine whether intra-articular CSIs of the shoulder, elbow, or wrist increase short-term fragility fracture risk in non-geriatric patients with upper extremity OA. METHODS: We retrospectively reviewed patients aged < 65 years who received intra-articular CSIs in the shoulder, elbow, or wrist b

Surgical approaches for open reduction and internal fixation of intra-articular distal humerus fractures in adults: A systematic review and meta-analysis.

BACKGROUND: A number of surgical approaches have been described for open reduction and internal fixation (ORIF) of intra-articular distal humerus (IDH) fractures in adults. However, there is no consensus as to which approach is better in terms of functional outcomes and complications. The purpose of this study was to determine whether the functional outcomes and types and rates of complications are influenced by the choice of surgical approach for ORIF of IDH fractures (AO/OTA types 13 B & C). METHODS: A systematic review of literature was performed using the PubMed, EMBASE and Cochrane Datab

Rehabilitation of Paraplegia Secondary to Giant cell Tumor of Bone Case Report.

INTRODUCTION: 180 °rotated and displaced fracture neck of radius with displacement is rare case in pediatric age as the cartilaginous head absorbs the force and transmits it to the weaker physis or metaphysis of the neck. These fractures characteristically produce an angular deformity of the head with the neck. CASE REPORT: This case report describes a rare case of 180° rotated and displaced fracture neck of radius with displacement. A 12-year-old female presented to OPD with history of self-fall over right elbow while playing and started complaining of pain over right elbow for about 2 h. Sh

MR Imaging of Pediatric Elbow Trauma.

Magnetic resonance imaging (MRI) of the pediatric elbow is important because it allows delineation of fracture type in a joint with complex anatomy, made more complex by the variable appearance of ossification centers of the distal humerus.1-4 Acute and chronic injuries can be evaluated with MRI using several classification systems. Accurate diagnosis of pediatric elbow trauma with MRI has management implications. For example, a Salter-Harris II fracture can be differentiated from a Salter-Harris IV physeal fracture by delineating intra-articular fracture extension and displacement, which requ

Correlation between inner epiphysis ulna radius length ratio and prognosis of Monteggia fractures in children: a multicenter study.

The forearm function in children is influenced by several factors, with the length relationship between the ulna and radius being particularly important. However, due to the underdevelopment of cartilage in children, assessing this length relationship through imaging techniques is challenging. This study aims to identify a reliable method for evaluating the lengths of the ulna and radius in children, establish normal values for the ratio of the epiphyseal plate lengths of these bones, examine this ratio in children with Monteggia fractures, and investigate its role in determining forearm funct

Ultrasound assistance in treatment with elastic stable intramedullary nail fixation in radial and ulnar fractures in children.

PURPOSE: Radial and ulnar fractures are one of the most common fractures in children. When closed reduction of fractures fails, elastic stable intramedullary nail (ESIN) fixation can mostly be used under the guidance of fluoroscopy. In this study, we evaluated the effect of ultrasound (US) as assistance for radial and ulnar fracture reduction and the insertion of ESINs. METHODS: There were 56 patients with midshaft radial and ulnar fractures included in our hospital from March 2019 to August 2021. After applying the inclusion and exclusion criteria and according to the treatment method, they
