Imaging of Congenital Anomalies and Developmental Abnormalities Involving the Arteries of the Head and Neck.

The embryonic development of the cerebrovascular system is a highly intricate process that gives rise to the arterial network supplying the brain, one of the most metabolically demanding organs in the human body. A hallmark of this system is the presence of extensive collateral circulation, exemplified by the circle of Willis, which can preserve cerebral perfusion in the event of arterial occlusion, severe stenosis, or dissection. Disruptions in the embryologic development of this system can lead to a spectrum of congenital and developmental arterial anomalies. While many of these variants are

[Anatomy of the Cavernous Sinus].

The cavernous sinus is the crossroad of veins from various embryological origins, including the brain, eye, pituitary gland, dura, and cranium. Embryologically, the cavernous sinus is mainly formed from the pro-otic sinus; secondary anastomosis between the cavernous sinus and primitive tentorial sinus results in various anatomical variations in the drainage patterns of the superficial middle cerebral vein. Moreover, connections between the cavernous sinus and basal vein via the uncal vein, bridging vein, and petrosal vein from the superior petrosal sinus may exist. Retrograde drainage from the

Anorectal Malformation: State of the Art in Translating Experimental Research to the Bedside.

The embryology of anorectal malformation (ARM) is a controversial issue. The study in humans is difficult due to the scarcity of fetuses with this anomaly. Therefore, ARM animal models, naturally obtained or induced by drugs, have been employed to understand physiopathology and possible treatments. Pigs, rabbits, rats, and mice have been employed as animal models. Additionally, many drugs have been used with this purpose: Etretinate, Ethylenethiourea, and Adriamycin. The animal more frequently used is the rat because of good reproducibility, low cost, and easy handling. Pig is a good model, bu…

A Sudden Infant Death Due to Congenital Diaphragmatic Hernia.

Congenital diaphragmatic hernia (CDH) is an uncommon condition in which the embryological elements of the diaphragm fail to fuse completely, leaving a defect in the barrier separating the thorax from the abdomen. Although most cases are symptomatic at birth and lead to prompt treatment, asymptomatic cases may go undetected, presenting later on as a result of sudden or exacerbated herniation of abdominal contents into the thoracic cavity. Presented here is the sudden death of a 6-week-old girl. At autopsy, the abdominal organs were found to be filling the left chest cavity, having herniated thr

Surgical strategy for primary hyperparathyreoidism with thyroid hemiagenesis.

BACKGROUND: Thyroid hemiagenesis is a rare congenital anomaly, and still more rarely associated with primary hyperparathyroidism (pHPT). Due to the embryologic pathways of the thyroid and parathyroid glands, it remains unclear whether or not thyroid hemiagenesis may be linked to ipsilateral parathyroid agenesis, and consequently, surgical strategy for thyroid hemiagenesis associated pHPT (THAP) does not only depend on preoperative localization but also on the thyroid anomaly. METHODS: Including the present case report, a total of nine cases with THAP retrieved from the literature were reviewe

Lateral congenital dermal sinuses extending into the spinal canal: case report and review of the literature.

Lateral congenital dermal sinus (CDS) typically blind-ends in the bone or soft tissue; meanwhile, medial CDS extends into the spinal canal through the spina bifida embryologically. We report the first case of lateral CDS extending into the spinal canal laterally without passing through the spina bifida. A 9-month-old infant presented with infected CDS refractory to partial sinus removal with a combination of surgical drainage and intravenous antibiotics. Eventually, a complete residual sinus removal was performed. Intraoperative findings revealed that the sinus extended into the spinal canal t

An Unusual Relation in an Infant With Left Hemitruncus and Tetralogy of Fallot Along With Pulmonary Valve Syndrome: A Case Report.

The abnormal origin of the left pulmonary artery (LPA) from the ascending aorta is a rare cardiac condition that is often associated with several other congenital defects. In this paper, we report the case of an infant who presented with recurrent infections and was prenatally diagnosed with tetralogy of Fallot (TOF). During echocardiography, various other cardiac defects such as ventricular septal defects (VSD), pulmonary stenosis (PS), and dilated right heart chambers were identified. Furthermore, cardiac catheterization revealed an anomalous origin of the LPA arising from the aorta associat

A Comprehensive Review of Congenital Lumbar Synostosis and Associated Findings.

Congenital vertebral synostosis (CVS) is a rare developmental condition due to failure of vertebral segmentation. Vertebrae and their intervertebral discs differentiate and resegment at the time of organogenesis during fetal life. Failure of this embryological process can result in the limitation of mobility of the involved segment. This inappropriate segmentation thus results in vertebral fusion or a block vertebra with subsequent vertebral synostosis. Long-term, such segmental fusion can increase osteoarthritis at levels below and above the fused segment due to excessive wear on these joints

The paediatric thymus: recognising normal and ectopic thymic tissue.

The appearance of the paediatric thymus changes as the normal process of thymic involution occurs. Thymic tissue may be orthotopic within the anterior mediastinum or ectopically located along the course of its embryological development. The variable appearance of orthotopic and ectopic thymic tissue in children on imaging studies may lead to misinterpretation of the normal thymus as pathology. Recognition of normal thymic tissue can mitigate unnecessary further diagnostic testing and patient anxiety. In this review, we discuss the embryological development and anatomical variants of normal thy

Bilateral persistent stapedial arteries associated with unilateral moyamoya disease: a particular anatomic variant.

PURPOSE: The persistence of the stapedial artery is a rare vascular variant that could explain the origin of the middle meningeal artery from the petrous segment of the internal carotid artery. The anatomic variations are illustrated and a summary of the possible hypothesis of the origin of the middle meningeal artery from the internal carotid artery is discussed, analysing the embryological works of the Carnegie Institute and the vascular development of the middle meningeal. METHODS AND RESULTS: The authors present a young patient showing a particular vascular variant on diagnostic imaging.

Branchial cleft cysts: a pictorial review.

Cystic masses of the neck may represent a wide variety of pathological conditions. Among these, branchial anomalies are the commonest paediatric congenital lesions of the head and neck. This article intends to review this condition under the same headings, as well as facilitate their diagnosis with the help of appropriate radiological findings of patients diagnosed with these anomalies. In order to make an accurate diagnosis, it is imperative to have an idea about the possible differentials, and the normal embryological development of the branchial arches, as well as their anomalies, in the fo

Intraspinal dermoid and epidermoid tumors: report of 18 cases and reappraisal of the literature.

Intraspinal dermoid and epidermoid tumors are two histopathological subtypes of cutaneous inclusion tumors of the spine. This classification is based on obsolete embryological knowledge. In fact, according to current embryology, both tumor types consist of ectodermal derivatives. Therefore, we hypothesized that dermoid and epidermoid tumors do not differ in clinical practice. To explore this hypothesis, we studied the clinical, radiological and intraoperative findings of 18 patients, and related these findings to the histopathological characteristics of the tumor. No differences were found bet…

Are kinking and coiling of carotid artery congenital or acquired?

Dolichoarteriopathies consist of tortuosity, kinking, or coiling of the extracranial carotid arteries. Some authors consider these alterations a consequence of atherosclerotic vessel remodeling, while others ascribe them to anatomical variations of embryological origin. The objective was to establish whether carotid dolichoarteriopathies belonged to a congenital origin or to acquired conditions. Color Doppler ultrasonography of neck vessels was performed in 885 participants, whose age ranged from 1-day-old infants to 90-year-old adults. Prevalence of kinking and coiling was evaluated, and it w
