Epilepsy and epileptiform activity in late-onset Alzheimer disease: clinical and pathophysiological advances, gaps and conundrums.

A growing body of evidence has demonstrated a link between Alzheimer disease (AD) and epilepsy. Late-onset epilepsy and epileptiform activity can precede cognitive deterioration in AD by years, and its presence has been shown to predict a faster disease course. In animal models of AD, amyloid and tau pathology are linked to cortical network hyperexcitability that precedes the first signs of memory decline. Thus, detection of epileptiform activity in AD has substantial clinical importance as a potential novel modifiable risk factor for dementia. In this Review, we summarize the epidemiological

Perinatal Hypoxia and Immune System Activation in Schizophrenia Pathogenesis: Critical Considerations During COVID-19 Pandemic.

Schizophrenia, a severe psychiatric, neurodevelopmental disorder affecting about 0.29-1 % of the global population, is characterized by hallucinations, delusions, cognitive impairments, disorganized thoughts and speech, leading to significant social withdrawal and emotional blunting. During the 1980s, considerations about diseases that result from complex interactions of genetic background and environmental factors started to appear. One of the critical times of vulnerability is the perinatal period. Concerning schizophrenia, obstetric complications that are associated with hypoxia of the fetu

Climate Changes Exacerbate the Spread of Ixodes ricinus and the Occurrence of Lyme Borreliosis and Tick-Borne Encephalitis in Europe-How Climate Models Are Used as a Risk Assessment Approach for Tick-Borne Diseases.

Climate change has influenced the transmission of a wide range of vector-borne diseases in Europe, which is a pressing public health challenge for the coming decades. Numerous theories have been developed in order to explain how tick-borne diseases are associated with climate change. These theories include higher proliferation rates, extended transmission season, changes in ecological balances, and climate-related migration of vectors, reservoir hosts, or human populations. Changes of the epidemiological pattern have potentially catastrophic consequences, resulting in increasing prevalence of

Body mass index and Hodgkin's lymphoma: UK population-based cohort study of 5.8 million individuals.

Previous epidemiological studies describe a positive association between body mass index (BMI) and Hodgkin's lymphoma, mainly in obese vs. normal weight individuals. We examined the shape of this relationship in individuals aged 16 years or older, using primary care data from the United Kingdom's Clinical Practice Research Datalink. Cox models were fitted with linear, non-linear (spline) and categorical BMI. Models were adjusted for potential confounders and effect modification was investigated. Five point eight two million patients were included, 927 of whom developed Hodgkin's lymphoma durin

Impact of traumatic brain injury on amyotrophic lateral sclerosis: from bedside to bench.

Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease characterized by the loss of upper and lower motor neurons, which manifests clinically as progressive weakness. Although several epidemiological studies have found an association between traumatic brain injury (TBI) and ALS, there is not a consensus on whether TBI is an ALS risk factor. It may be that it can cause ALS in a subset of susceptible patients, based on a history of repetitive mild TBI and genetic predisposition. This cannot be determined based on clinical observational studies alone. Better preclinical models are nec

Cystatin F is a biomarker of prion pathogenesis in mice.

Misfolding of the cellular prion protein (PrPC) into the scrapie prion protein (PrPSc) results in progressive, fatal, transmissible neurodegenerative conditions termed prion diseases. Experimental and epidemiological evidence point toward a protracted, clinically silent phase in prion diseases, yet there is no diagnostic test capable of identifying asymptomatic individuals incubating prions. In an effort to identify early biomarkers of prion diseases, we have compared global transcriptional profiles in brains from pre-symptomatic prion-infected mice and controls. We identified Cst7, which enco

High turnover drives prolonged persistence of influenza in managed pig herds.

Pigs have long been hypothesized to play a central role in the emergence of novel human influenza A virus (IAV) strains, by serving as mixing vessels for mammalian and avian variants. However, the key issue of viral persistence in swine populations at different scales is ill understood. We address this gap using epidemiological models calibrated against seroprevalence data from Dutch finishing pigs to estimate the 'critical herd size' (CHS) for IAV persistence. We then examine the viral phylogenetic evidence for persistence by comparing human and swine IAV. Models suggest a CHS of approximatel…

Diabetes in pregnancy and lung health in offspring: developmental origins of respiratory disease.

Diabetes is an increasingly common complication of pregnancy. In parallel with this trend, a rise in chronic lung disease in children has been observed in recent decades. While several adverse health outcomes associated with exposure to diabetes in utero have been documented in epidemiological and experimental studies, few have examined the impact of diabetes in pregnancy on offspring lung health and respiratory disease. We provide a comprehensive overview of current literature on this topic, finding suggestive evidence that exposure to diabetes in utero may have adverse effects on lung develo

Validity and reliability of the parental sun protection scales.

Skin cancer is the most common cancer diagnosed in the US and its incidence continues to rise. Epidemiological studies have shown that excessive sun exposure received during childhood may increase the risk of developing skin cancer later in life. Yet, there are few published reports on the development of reliable and valid theory-based scales that assess the factors associated with parental sun-protection practices to reduce sun exposure in preschool children. To fill this gap, the Parental Sun Protection Scales were developed and validated. Two series of confirmatory factor analytic models we

Using models to shape measles control and elimination strategies in low- and middle-income countries: A review of recent applications.

After many decades of vaccination, measles epidemiology varies greatly between and within countries. National immunization programs are therefore encouraged to conduct regular situation analyses and to leverage models to adapt interventions to local needs. Here, we review applications of models to develop locally tailored interventions to support control and elimination efforts. In general, statistical and semi-mechanistic transmission models can be used to synthesize information from vaccination coverage, measles incidence, demographic, and/or serological data, offering a means to estimate th

The Importance of Being FAIR and FAST - The Clinical Epidemiology and Study Platform of the German Network University Medicine (NUKLEUS).

The COVID-19 pandemic has urged the need to set up, conduct and analyze high-quality epidemiological studies within a very short time-scale to provide timely evidence on influential factors on the pandemic, e.g. COVID-19 severity and disease course. The comprehensive research infrastructure developed to run the German National Pandemic Cohort Network within the Network University Medicine is now maintained within a generic clinical epidemiology and study platform NUKLEUS. It is operated and subsequently extended to allow efficient joint planning, execution and evaluation of clinical and clinic

From Cardiovascular Risk Prediction to Precision Prevention: Methodological, Contextual, and Implementation Challenges.

Despite advances in risk prediction, cardiovascular prevention remains suboptimal, reflecting limitations in model architecture, incomplete integration of contextual determinants, and inadequate translation of risk estimates into clinical practice. This review synthesizes epidemiological, methodological, and health systems evidence published between 2000 and 2025 regarding the performance, evolution, and implementation of cardiovascular risk prediction models. Contemporary risk assessment frameworks remain largely driven by traditional risk factors, and the addition of novel biomarkers or vari

Prospective study using artificial neural networks for identification of high-risk COVID-19 patients.

The COVID-19 pandemic caused a major public health crisis, with severe impacts on global health and the economy. Machine learning (ML) has been crucial in developing new technologies to address challenges posed by the pandemic, particularly in identifying high-risk COVID-19 patients. This identification is vital for efficiently allocating hospital resources and controlling the virus's spread. Comprehensive validation of these intelligent approaches is necessary to confirm their clinical usefulness and help create future strategies for managing viral outbreaks. Here we present a prospective stu

Prediction models based on machine learning algorithms for COVID-19 severity risk.

BACKGROUND: The World Health Organization has highlighted the risk of Disease X, urging pandemic preparedness. Coronavirus disease 2019 (COVID-19) could be the first Disease X; therefore, understanding the epidemiological experiences of COVID-19 is crucial while preparing for future similar diseases. METHODS: Prediction models for COVID-19 severity risk in hospitalized patients were constructed based on four machine learning algorithms, namely, logistic regression, Cox regression, support vector machine (SVM), and random forest. These models were evaluated for prediction accuracy, area under

A district-level ensemble model to enhance dengue prediction and control for the Mekong Delta Region of Vietnam.

The Mekong Delta Region (MDR) of Vietnam faces increasing vulnerability to severe dengue outbreaks due to urbanization, globalization, and climate change, necessitating effective early warning systems for outbreak mitigation. This study developed a probabilistic forecasting model to predict dengue incidence and outbreaks with 1-3-month lead times, incorporating meteorological, sociodemographic, preventive, and epidemiological data. A total of 72 models were evaluated, with top performers from spatiotemporal models, supervised PCA, and semi-mechanistic hhh4 frameworks combined into an ensemble.

Neurological manifestations of encephalitic alphaviruses, traumatic brain injuries, and organophosphorus nerve agent exposure.

Encephalitic alphaviruses (EEVs), Traumatic Brain Injuries (TBI), and organophosphorus nerve agents (NAs) are three diverse biological, physical, and chemical injuries that can lead to long-term neurological deficits in humans. EEVs include Venezuelan, eastern, and western equine encephalitis viruses. This review describes the current understanding of neurological pathology during these three conditions, provides a comparative review of case studies vs. animal models, and summarizes current therapeutics. While epidemiological data on clinical and pathological manifestations of these conditions

Respiratory Health Impacts of Outdoor Air Pollution and the Efficacy of Local Risk Communication in Quito, Ecuador.

Relatively few studies on the adverse health impacts of outdoor air pollution have been conducted in Latin American cities, whose pollutant mixtures and baseline health risks are distinct from North America, Europe, and Asia. This study evaluates respiratory morbidity risk associated with ambient air pollution in Quito, Ecuador, and specifically evaluates if the local air quality index accurately reflects population-level health risks. Poisson generalized linear models using air pollution, meteorological, and hospital admission data from 2014 to 2015 were run to quantify the associations of ai

All Patient Refined-Diagnosis Related Groups' (APR-DRGs) Severity of Illness and Risk of Mortality as predictors of in-hospital mortality.

The aims of this study were to assess All-Patient Refined Diagnosis-Related Groups' (APR-DRG) Severity of Illness (SOI) and Risk of Mortality (ROM) as predictors of in-hospital mortality, comparing with Charlson Comorbidity Index (CCI) and Elixhauser Comorbidity Index (ECI) scores. We performed a retrospective observational study using mainland Portuguese public hospitalizations of adult patients from 2011 to 2016. Model discrimination (C-statistic/ area under the curve) and goodness-of-fit (R-squared) were calculated. Our results comprised 4,176,142 hospitalizations with 5.9% in-hospital deat

Computerized analysis of mammographic parenchymal patterns for breast cancer risk assessment: feature selection.

Our purpose in this study was to identify computer-extracted, mammographic parenchymal patterns that are associated with breast cancer risk. We extracted 14 features from the central breast region on digitized mammograms to characterize the mammographic parenchymal patterns of women at different risk levels. Two different approaches were employed to relate these mammographic features to breast cancer risk. In one approach, the features were used to distinguish mammographic patterns seen in low-risk women from those who inherited a mutated form of the BRCA1/BRCA2 gene, which confers a very high

A reappraisal of vector control strategies for babesiosis.

This review suggests that we are on the threshold of a new epoch in the control of ticks and the diseases transmitted by them. Ecological st … s of field research, as well as models to analyse these data, should accelerate the latter process. Epidemiological models should be included in the analysis of babesiosis in areas that run the risk of converting from stable zones to unstable zones due to strategic dipping. These observations and projections underscore the value of integration of traditional and modern …
