Risk factors for increased length of hospital stay among young pediatric patients who suffer facial fractures.

BACKGROUND/AIM: It is not known whether the mechanism of facial trauma influences the associated length of hospitalization. The aim of this study was to determine whether the mechanism of injury influenced the length of hospital stay (LOS) among pediatric patients who sustained facial fractures. MATERIALS AND METHODS: This retrospective cohort study was completed using the Kids' Inpatient Database (KID). The primary predictor variable was the mechanism of injury. The primary outcome variable was LOS. Linear regression was used to determine independent predictors of increased/decreased length

DHI Increases the Proliferation and Migration of Schwann Cells Through the PI3K/AKT Pathway and the Expression of CXCL12 and GDNF to Promote Facial Nerve Function Repair.

The facial nerve is one of the vulnerable nerves in otolaryngology. Repair and recovery of facial nerve injury have a high priority in clinical practice. The proliferation and migration of Schwann cells are considered of great significance in the process of nerve injury repair. Danhong injection (DHI), as a common drug for cardiovascular and cerebrovascular diseases, has been fully certified in neuroprotection research, but its role in facial nerve injury is still not clear. Our study found that DHI can promote the proliferation and migration of RSC96 cells, a Schwann cell line, and this effec

Review of Treatment Options for Facial Synkinesis: Working Toward a Standardized Algorithm.

BACKGROUND: Facial synkinesis describes a sequela of transient facial nerve injury that can occur following viral reactivation syndromes such as Bell's palsy or following traumatic facial nerve injury. Facial synkinesis is frequently characterized by muscle hypertonicity, involuntary co-contraction of facial muscles, and spasm and can have severe functional and esthetic consequences. Although this condition is not curable, several treatment options exist; however, there is currently a lack of consensus regarding the appropriate timeline and sequence of synkinesis treatment strategies. Recovery of motor neuron excitability after facial nerve impairment in rats.

Multiple studies have demonstrated alterations in excitability in the central nervous system after peripheral nerve injury. However, there are few reports on changes in the central nervous system after peripheral facial nerve injury. Our objective was to determine the excitability changes that occur in the facial nucleus after facial nerve impairment. The excitability changes in the facial nucleus were investigated by assessing two types of compound muscle action potentials (M and F waves) in the orbicularis oculi muscles, evoked by electrical stimulation of the zygomatic branch of the facial

The upregulated expression of sonic hedgehog in motor neurons after rat facial nerve axotomy.

Nerve injury leads to the induction of a large number of genes to repair the damage and to restore synaptic transmission. We have attempted to identify molecules whose mRNA expression is altered in response to facial nerve axotomy. Here we report that facial nerve axotomy upregulates Sonic hedgehog (Shh) and its receptor Smoothened (Smo) in facial motor neurons of adult rats, whereas facial nerve axotomy does not upregulate mRNA of Shh or Smo in neonatal rats. We tested whether overexpression of Shh in facial motor neurons of axotomized neonatal rats may promote neuronal survival. Adenovirus-m

Extratemporal facial nerve branching patterns: systematic review of 1497 cases.

OBJECTIVE: The aim of this systematic review was to analyse the complex anatomy of the extratemporal portion of the facial nerve with an accurate description of the branching patterns based on the Davis classification. METHOD: Medline, ScienceDirect and the Cochrane Library databases as well as other sources were searched by two independent reviewers. RESULTS: Analysis of 21 studies with a total of 1497 cases showed that type III is the most common branching pattern accounting for 26.8 per cent of cases. The type I pattern, previously considered as the normal anatomy in most textbooks, was t

[Modification of neurophysiological study in peripheral facial nerve palsy located in the orbicularis oris muscle after a dental procedure].

Peripheral facial nerve palsy (PFNP) has a substantial physical, psychological and social impact on patients. Neurophysiological study quantifies the degree of nerve injury and assesses prognosis. We present the case of a woman with a 3-month history of left PFNP after a dental implant, with facial functionality of 85.5% and with a normal neurophysiological study performed according to the standard protocol. By modifying the technique centred on the orbicularis oris in its upper portion, the procedure showed an asymmetry of amplitude and signs of denervation. This allowed us to detect a defici

Periocular Reconstruction in Patients with Facial Paralysis.

Facial paralysis can result in serious ocular consequences. All patients with orbicularis oculi weakness in the setting of facial nerve injury should undergo a thorough ophthalmologic evaluation. The main goal of management in these patients is to protect the ocular surface and preserve visual function. Patients with expected recovery of facial nerve function may only require temporary and conservative measures to protect the ocular surface. Patients with prolonged or unlikely recovery of facial nerve function benefit from surgical rehabilitation of the periorbital complex. Current reconstruct

Facial nerve paralysis secondary to mandibular fracture.

When facial nerve paralysis complicates a mandibular fracture, it may be difficult to locate the point of injury, since the nerve may be injured in the fallopian canal by a secondary temporal bone fracture or in soft tissue by mandibular fragments. Following a review of eight previously reported cases, this paper presents two additional cases, one with bilateral, complete paralyses and the other with a unilateral paresis. In the first, the condyles were driven posteriorly, resulting in bilateral temporal bone fractures, a unilateral external canal stenosis, and a unilateral sensorineural heari

Maxillofacial trauma secondary to airbag deployment: A scoping review.

OBJECTIVES: Motor vehicle accidents remain a leading cause of craniofacial trauma, with injury severity evolving alongside automotive safety advancements. While airbags and seatbelts have revolutionized trauma prevention, reducing worldwide mortality by over 70,000 lives in five years, their mechanics can paradoxically modify or exacerbate facial injuries due to occupant positioning, chemical factors, and collision dynamics. This study examines injury patterns, mechanisms, and trauma prevention strategies related to airbag-related maxillofacial trauma. METHODS: A scoping review was conducted …

Mechanisms of deaths in captive juvenile New Zealand fur seals (Arctocephalus forsteri).

Juvenile seals are sometimes encountered in waters around South Australia with injuries and/or diseases that require veterinary treatment. Two cases are reported where apparently stable animals died soon after being rescued due to quite disparate conditions. In Case 1 a juvenile male New Zealand fur seal (Arctocephalus forsteri) was found unexpectedly dead in its enclosure. A necropsy examination revealed an emaciated juvenile male with no injuries. The intestine was filled throughout its length with melena stool that was due to heavy infestation of the stomach with roundworms with adjacent ga

Pre-Operative Antibiotic Agents for Facial Fractures: Is More than One Day Necessary?

Background: Despite a paucity of evidence, patients with facial fractures often receive long courses of pre-operative antibiotic agents. This study compared the effect of a short versus long pre-operative antibiotic course on the development of post-operative head/neck infections in this population. Patients and Methods: All adult patients admitted between January 2010 and May 2015 to a level 1 trauma center with isolated head/neck injuries who underwent surgery for facial fracture(s) were included. Patients with infections prior to surgery were excluded. Our primary analysis compared head/nec

Fore! A 10-year Analysis of Golf-related Facial Fractures.

BACKGROUND: Over 30 million people in the United States play golf, which, while considered a low-impact sport, involves balls and clubs moving >100 miles/h (>160.93 km/h), creating potential for a significant facial trauma. The objective of this study was to characterize the epidemiology of golf-related facial fractures in the United States. METHODS: The National Electronic Injury Surveillance System, which records injuries from approximately 100 US emergency departments, was queried for golf-related facial fractures from 2009 to 2018. Entries were tabulated for mechanism of injury and fractu

Static treatment modalities in facial paralysis: a review.

INTRODUCTION: Facial nerve dysfunction can be attributed to several different causes. Several techniques have been developed to help treat the appearance and functional limitation of patients with sequelae of facial nerve dysfunction. There are options regarding static techniques of facial nerve injury treatment that range from facial musculature plication or shortening, fascial sling suspension via allograft or autograft, injectables and implants (ENDURAGen, AlloDerm, LifeCell, Bridgewater, New Jersey, USA) to techniques such as brow lift, open and endoscopic facelifts, and various eyelid sur

Management of temporal nerve injuries.

Transection of the temporal branch of the facial nerve results in paralysis of the frontalis muscle with subsequent inability to wrinkle the forehead. In addition there is ptosis of the eyebrow with upper visual field defects. The intent of this article is to outline an orderly, rational approach to the management of such injuries based on our experience with seven patients whose nerve was damaged during Mohs surgery for cancer ablation. Repair options include various combinations of direct brow-plasty, indirect temporal or coronal browlifts, and adjunctive upper lid blepharoplasty.
