Necrotizing Fasciitis of the Perineum and the Abdominal Wall-Surgical Approach.

Necrotizing fasciitis is a synergistic aerobical-anaerobical infection that rapidly spreads from its origin leading to foudroyant increasing soft tissue gangrene and systemic sepsis. In the perineal, perianal and genital regions it is referred to as Fournier's gangrene constituting a clinical entity with the gangrene of the abdominal wall. After a short presentation of the own experience with 17 patients the surgical approach to the life-threatening disease is presented reviewing the former and the recent literature. There is a worldwide consensus that immediate radical excision of the gangren…

Laboratory risk indicator for necrotizing fasciitis score and the outcomes.

BACKGROUND: Laboratory risk indicator for necrotizing fasciitis (LRINEC score) is a simple laboratory tool used to distinguish between necrotizing soft-tissue infections (NSTI) and other soft-tissue infections. A LRINEC score of > or =6 is considered as denoting a high risk of necrotizing fasciitis. A certain LRINEC score might also be associated with mortality and other outcomes of patients with NSTI. METHODS: A review of the medical charts of patients was carried out. The study sites were one tertiary academic centre and one community, university-affiliated hospital. All adult patients with…

Pulmonary arteriovenous malformation associated with necrotizing fasciitis of the thigh.

Pulmonary arteriovenous malformation (PAVM) is a rare condition often associated with hereditary hemorrhagic telangiectasia. The association between PAVMs and brain abscesses has been previously reported, but abscesses at other locations are extremely rare. We present a 51-year-old woman with PAVM (without signs of hereditary hemorrhagic telangiectasia) and necrotizing fasciitis of her thigh and discuss the possible relationship between the 2 disorders.

Staging of necrotizing fasciitis based on the evolving cutaneous features.

BACKGROUND: Necrotizing fasciitis is a severe soft-tissue infection characterized by a fulminant course and high mortality. Early recognition is difficult as the disease is often clinically indistinguishable from cellulitis and other soft-tissue infections early in its evolution. Our aim was to study the manifestations of the cutaneous signs of necrotizing fasciitis as the disease evolves. METHODS: This was a retrospective study on patients with necrotizing fasciitis at a single institution. Their charts were reviewed to document the daily cutaneous changes from the time of presentation (day …

Necrotizing fasciitis in a patient with type 2 diabetes mellitus.

Necrotizing fasciitis is a soft-tissue infection characterized by extensive necrosis of subcutaneous fat, neurovascular structures, and fascia. In general, fascial necrosis precedes muscle and skin involvement, hence its namesake. Initially, this uncommon and rapidly progressive disease process can present as a form of cellulitis or superficial abscess. However, the high morbidity and mortality rates associated with necrotizing fasciitis suggest a more serious, ominous condition. A delay in diagnosis can result in progressive advancement highlighted by widespread infection, multiple-organ invo…

Necrotizing fasciitis caused by Aeromonas hydrophilia in an immunocompetent child.

Necrotizing fasciitis is a rapidly progressive and potentially fatal infection. It is characterized by extensive subcutaneous and muscle necrosis. Aeromonus hydrophilia is a gram-negative faculatively anaerobic bacillus that is part of the normal flora of nonfecal sewage and is found in most water systems, soil, and fresh brackish water. Necrotizing fasciitis caused by A. hydrophilia have occurred in children who had underlying systemic diseases or immune dysfunction. We report a 14-year-old boy without underlying systemic diseases or immunodeficiency who developed necrotizing fasciitis second…

Multiple dimensions of caring for a patient with acute necrotizing fasciitis.

Critical care nurses within acute care settings are responsible for providing healthcare to a wide variety of patients and, consequently, knowledge regarding how to care for a patient with acute necrotizing fasciitis is imperative. A case study is presented to evidence the need for a multidisciplinary approach. Necrotizing fasciitis is defined and treatment options are presented. Caring for this patient is very challenging and demands a multidisciplinary team to coordinate all aspects of care to promote better patient outcomes.

Surgical debridement of craniocervical necrotizing fasciitis: the window of opportunity.

Craniocervical necrotizing fasciitis (CCNF) has a potentially high morbidity and mortality. Late presentation, shock and disseminated intravascular coagulation are associated with a particularly poor prognosis. Early recognition and aggressive treatment is advised. A case report is presented of a late presentation of necrotizing fasciitis of the face, neck and anterior thoracic wall. Despite adverse prognostic indicators, surgical debridement was performed. Intensive medical treatment included continuous renal replacement therapy and intravenous immunoglobulin. Survival in this case illustrate…

Multi-limb necrotizing fasciitis in a patient with rectal cancer.

Necrotizing fasciitis is a devastating soft tissue infection affecting fascias and subcutaneous soft tissues. Literature reviews have identified several related risk factors, including malignancy, alcoholism, malnutrition, diabetes, male gender and old age. There are only scanty case reports in the literature describing its rare association with colorectal malignancy. All published cases are attributed to bowel perforation resulting in necrotizing fasciitis over the perineal region. Isolated upper or lower limb diseases are rarely identified. Simultaneous upper and lower limb infection in colo…

Necrotizing fasciitis caused by Shewanella putrefaciens in a uremic patient.

Shewanella putrefaciens rarely causes infections in humans. This report describes a case of necrotizing fasciitis caused by S. putrefaciens in a uremic patient who recovered in spite of inadequate antibiotic treatment. S. putrefaciens is a possible causative organism of necrotizing fasciitis, and the absence of any sign of systemic infection cannot rule out the possibility of invasive infection in uremic patients. Surgical intervention is important in such cases.

The efficacy of a pneumatic compression device in the treatment of plantar fasciitis.

Plantar fasciitis is a common foot disorder that affects more than two million Americans each year. Conservative management of plantar fasciitis is the first line of treatment by the medical specialist who frequently encounters this common foot complaint. In this study, 50 patients with plantar fasciitis of 4 weeks duration but less than 12 weeks were randomized into two groups. One group of 25 patients used the AirHeel (Aircast, Inc.) and the second group of 25 used the 1st Step prefabricated foot insert (Wrymark, Inc). Standardized weight-bearing radiographs were obtained in order to categor…

Correlation of histopathologic findings with clinical outcome in necrotizing fasciitis.

BACKGROUND: Necrotizing fasciitis is an uncommon disease with high morbidity and mortality rates. Little is known about the role of histopathologic examination in disease prognosis. METHODS: A retrospective study was conducted to determine what correlations, if any, exist between the histopathologic features of resected tissue in patients with necrotizing fasciitis and clinical outcome. RESULTS: Eighty-two cases of necrotizing fasciitis that occurred between January 1990 and December 2002 were identified. Histopathologic findings were available for review in 63 cases. A novel histopathologic…

Necrotizing fasciitis: a rare complication of hypospadias surgery in a child.

Necrotizing fasciitis is a rapidly progressive soft tissue infection involving the skin, subcutaneous tissue, and superficial fascia. It is a rare entity in the pediatric population. The specific etiologic factors that contribute to the establishment of necrotizing fasciitis in children are not evident. It may occur after minor trauma or operative procedures. The authors report a case of necrotizing fasciitis after hypospadias repair. This complication is extremely unusual and to the authors' knowledge has not been previously described.

Postpartum retroperitoneal fasciitis: a case report and review of literature.

We present a case of a postpartum female with iliopsoas fasciitis in the puerperium. Two days after a spontaneous vaginal delivery at 38 weeks without any complications, the patient complained of pain in the left thigh and hip, associated with a temperature of 38 degrees C. Consequently, she could not walk 4 days after delivery and her body temperature had increased to 39 degrees C. Extreme left flank pain and tenderness in the left pelvic wall were prominent, whereas the tenderness in the pelvis was moderate. Magnetic resonance imaging led us to diagnose iliopsoas fasciitis. This was complica…

Necrotizing fasciitis of the leg presenting with chest wall emphysema.

We present the case of a 70-year-old woman with necrotizing fasciitis of the right leg, sepsis and bacteraemia with Escherichia coli. Chest wall emphysema, detected on standard radiograph and the presence of air in the soft-tissue of the foot was the reason for prompt surgical drainage in addition to standard fluid resuscitation and antibiotic therapy. There was no evidence of underlying diabetes mellitus, but unknown chronic renal failure and corticosteroid therapy for rheumatoid arthritis were considered predisposing factors. We present a short overview of this rare life-threatening conditio…

[Necrotizing fasciitis in an immunocompetent patient caused by Apophysomyces elegans].

A case study is presented of a 7-year-old boy, seriously injured in a car accident, who developed a fatal infection due to Aphophysomyces elegans--a mold of the Mucoracea family. Fungal invasion was initially manifested by a spotted wound in the left lumbar region which developed into a necrotizing fasciitis. Later this progressed to the right lumbar area, including the gluteus and the corresponding flank. Antimycotic treatment proved ineffective, and the child died 8 weeks after the accident. Other cases due to this fungus are reviewed.
