Anorectal and pelvic floor function. Relevance of continence, incontinence, and constipation.

Anorectal tests need to be tailored to the presentation of the individual patient. Clearly the tests are most useful when they identify anatomic or physiologic abnormalities for which there are successful treatments. For the incontinent patient, anal manometry is the most useful test. Sphincter injuries should be repaired, whereas neurogenic incontinence is best treated initially with biofeedback. Three tests are more useful for the constipated patient: colonic transit time, degree of pelvic floor descent on straining, and balloon expulsion. Colonic inertia responds to total colectomy and pelv

Bowel continence for the child with a neurogenic bowel.

For more than 10 years, the Alfred I. duPont Institute's Spinal Dysfunction Clinic in Wilmington, DE, has helped families and their children with myelomeningocele and neurogenic bowels to establish workable, acceptable, physiologically sound bowel programs. The basic bowel management program at the Alfred I. duPont Institute is described here, as is a program by which a child with a neurogenic bowel can obtain continence. The program is based on the child's developmental level, physiological abilities, and any necessary pharmacologic intervention. The program has enabled approximately 75% of i

Can the external anal sphincter be preserved in the treatment of trans-sphincteric fistula-in-ano?

A method of preserving the external anal sphincter in the treatment of complex trans-sphincteric fistula-in-ano is described. Cure without division of the external sphincter was possible in 44% of cases. Disturbances of continence, common after conventional fistula surgery, appeared to be reduced in those patients whose external sphincter remains intact.

Anal manometry, its applications and indications.

A manometric system is described to record the anal high-pressure zone. Anal manometry was performed in 77 individuals. It appeared that amplitude at rest is the most suitable variable to assess sphincter strength. Anal manometry is indicated in patients with chronic constipation or faecal incontinence. It may also be applied to postoperative assessment of operative procedures involving sphincter control.

Total anorectal reconstruction following congenital anorectal anomaly: report of a case.

Following several unsuccessful postnatal attempts at anoplasty to correct anal atresia with a rectovaginal fistula, the patient, 20 years later, underwent an anorectal sphincteric construction using a gracilis-muscle transplant. The five-stage procedure, which extended over a period of two years, resulted in highly satisfactory continence and full social rehabilitation of the patient. The continence achieved in this case seems to be in contradiction to some of the accepted concepts of the mechanisms of continence.

Anoplasty with skin tag flap for the treatment of chronic anal fissure.

OBJECTIVE: to evaluate the long-term results of the correction of chronic anal fissures (CAF) in patients without anal hypertonia using the technique of anoplasty with skin tag flap. METHODS: we conducted a prospective study of CAF patients refractory to conservative treatment without anal hypertonia, subjected to anoplasty with the use of anal plicoma to cover the bloody area. We confirmed the absence of anal hypertonia through rectal exam and electromanometry. We applied a visual pain questionnaire and the Cleveland Clinic fecal incontinence score before and after surgery. RESULTS: we foll…

Percutaneous endoscopic sigmoid colostomy for irrigation in the management of bowel dysfunction of adults with central neurologic disease.

BACKGROUND: Bowel dysfunction results in a major lifestyle disruption for many patients with severe central neurologic disease. Percutaneous endoscopic sigmoid colostomy for irrigation (PESCI) allows antegrade irrigation of the distal large bowel for the management of both incontinence and constipation. This study prospectively assessed the safety and efficacy of PESCI. METHODS: A PESCI tube was placed endoscopically in the sigmoid colon of 25 patients to allow antegrade irrigation. RESULTS: Control of constipation and fecal incontinence was improved for 21 (84%) of the 25 patients. These pa…

Long-term effects of epidural analgesia in labor: a randomized controlled trial comparing high dose with two mobile techniques.

Significantly less fecal incontinence (OR: 0.51, 95% CI: 0.30-0.87) and stress incontinence (OR: 0.65, 95% CI: 0.42-1.00) occurred with low-dose infusion. CONCLUSION: Trial evidence showed no long-term disadvantages and possible benefits of low-dose mobile relative to high-dose epidura …

Primary outcome measures encompassed fissure healing rates. Anorectal dysfunction after radical cystectomy for bladder cancer.

Objective: To prospectively assess anorectal dysfunction using patient-reported outcomes using validated questionnaires, manovolumetry and endoanal ultrasound before and 12 months after RC.Patients and methods: From 2014 to 2019, we prospectively included 44 patients scheduled for RC. Preoperatively and 12 months after surgery, 41 patients filled in a low anterior resection syndrome score (LARS-score) to assess fecal incontinence, increased frequency, urgency and emptying difficulties and a St Mark's score to assess fecal incontinence in conjunction with manovolumetry and endoanal ultrasound e…

An active duty woman with anal incontinence after vaginal delivery: an elusive diagnosis.

Anal sphincter injury as a result of birth trauma is the leading cause of fecal incontinence in women presenting to surgical clinics. A 36-year-old active duty woman was referred to the Urogynecology Clinic at Madigan Army Medical Center for evaluation of a deficient perineal body. She reluctantly reported a 12-year history of anal incontinence since the birth of her first child, predating her entry into the naval service. Examination revealed defects of the perineum and anal sphincter resembling a cloacal deformity. Uncomplicated overlapping anal sphincteroplasty and perineorrhaphy were perfo…

Repair of obstetric perineal lacerations.

Family physicians who deliver babies must frequently repair perineal lacerations after episiotomy or spontaneous obstetric tears. Effective repair requires a knowledge of perineal anatomy and surgical technique. Perineal lacerations are classified according to their depth. Sequelae of obstetric lacerations include chronic perineal pain, dyspareunia, urinary incontinence, and fecal incontinence. With lacerations involving the anal sphincter complex, particular attention must be given to anatomy and surgical technique because of the high incidence of poor functional outcomes after repair. An ove…

Fecal continence revisited: the anal external sphincter continence reflex.

BACKGROUND: None of the current theories on fecal incontinence can explain fecal continence adequately. OBJECTIVE: This study aims to evaluate the mechanism controlling fecal continence. DESIGN: Anal electrosensitivity, anorectal pressures, and rectal pressure volumetry tests were performed in 17 controls before and after superficial local anal anesthesia and in 6 controls before and after spinal anesthesia. The same tests were performed in 1 patient before and after injected local anal anesthesia and in 3 patients with spinal cord lesions at levels Th3 to L3. RESULTS: After superficial loc…
