Fetal growth standards: the NICHD fetal growth study approach in context with INTERGROWTH-21st and the World Health Organization Multicentre Growth Reference Study.

Three recently completed longitudinal cohort studies have developed intrauterine fetal growth charts, one in the United States and two international. This expert review compares and contrasts the Eunice Kennedy Shriver National Institute of Child Health and Human Development Fetal Growth Studies, INTERGROWTH-21st and World Health Organization Multicentre Growth Reference Study conclusions in light of differences in aims, sampling frames, and analytical approaches. An area of controversy is whether a single growth reference is representative of growth, regardless of ethnic or country origin. Th

In Utero Fetal Weight in Pigs Is Regulated by microRNAs and Their Target Genes.

Impaired skeletal muscle growth in utero can result in reduced birth weight and poor carcass quality in pigs. Recently, we showed the role of microRNAs (miRNAs) and their target genes in prenatal skeletal muscle development and pathogenesis of intrauterine growth restriction (IUGR). In this study, we performed an integrative miRNA-mRNA transcriptomic analysis in longissimus dorsi muscle (LDM) of pig fetuses at 63 days post conception (dpc) to identify miRNAs and genes correlated to fetal weight. We found 13 miRNAs in LDM significantly correlated to fetal weight, including miR-140, miR-186, miR

Ultrasonographic fetal growth charts: an informatic approach by quantitative analysis of the impact of ethnicity on diagnoses based on a preliminary report on Salentinian population.

Clear guidance on fetal growth assessment is important because of the strong links between growth restriction or macrosomia and adverse perinatal outcome in order to reduce associated morbidity and mortality. Fetal growth curves are extensively adopted to track fetal sizes from the early phases of pregnancy up to delivery. In the literature, a large variety of reference charts are reported but they are mostly up to five decades old. Furthermore, they do not address several variables and factors (e.g., ethnicity, foods, lifestyle, smoke, and physiological and pathological variables), which are

The impact of prenatal environment on postnatal life and performance: Future perspectives for prevention and treatment.

The present review aims to offer a non-comprehensive outline of the current state-of-the-art and future perspectives on management and therapeutic tools for intrauterine growth restriction (IUGR) and associated prenatal programming in both human and animal species. Animals are used as models for the study of phenomena related to IUGR, but also for research on prenatal therapies with the main objective of designing and developing preventive and therapeutic strategies. The research is currently paying attention on maternal-focused pharmacological treatments and nutritional strategies but also on

Growth charts and prediction of abnormal growth - what is known, what is not known and what is misunderstood.

OBJECTIVES: Assessment of fetal growth has an important effect on perinatal morbidity and mortality. To understand what tool to choose best for a given population a basic knowledge of how growth charts are developed and used has to be acquired. For this reason, this literature review was performed. MATERIAL AND METHODS: An extensive literature review aimed at identifying articles related to the development of growth assessment in both spectrums of abnormal fetal growth - large and small. The analyzed articles were chosen and presented to show both the historical aspects of growth assessment,

Teratogenic effect of hyperthermia during early organogenesis period in mice.

Pregnant Swiss albino mice were subjected to 41, 42, or 43 degrees C temperature for 10 minutes on day 6.5 of gestation. Another group of animals treated at 37 degrees C was used as control. All animals were killed on the 18th day of gestation and fetuses were examined for prenatal mortality, growth retardation, and microphthalmia incidence. Results indicated a dose dependent increase in the mortality rates with a 42% death in the 43 degrees C group. Treatment with the higher temperatures (42 and 43 degrees C) resulted in a significant increase in the number of growth retarded fetuses and in t

Effect of heat stress in the first 1000 days of life on fetal and infant growth: a secondary analysis of the ENID randomised controlled trial.

BACKGROUND: The intersecting crises of climate change, food insecurity, and undernutrition disproportionately affect children. Understanding the effect of heat on growth from conception to 2 years of age is important because of mortality and morbidity implications in the near term and over the life course. METHODS: In this secondary analysis, we used longitudinal pregnancy cohort data from the Early Nutrition and Immunity Development (ENID) randomised controlled trial in West Kiang, The Gambia, which occurred between Jan 20, 2010, and Feb 10, 2015. The ENID trial assessed micronutrient supple

A practical approach to the examination of the malformed fetal brain: impact on genetic counselling.

Birth defects of the brain result from malformation and disruptions. They remain an important cause of childhood morbidity and mortality. Effective treatments are scarce and prevention strategies limited. As aetiological screening is costly and uncertain, genetic counselling remains empirical in most cases. A pathological study of the malformed brain is the best approach to establish the diagnosis of a brain malformation. It relies on a thorough description of the brain, including its size, external pattern and/or internal configuration. When evaluating a malformed brain two major factors shou

Initiating a Fetal Cardiac Program from Scratch in Low- and Middle-Income Countries: Structure, Challenges, and Hopes for Solutions.

Although fetal cardiac programs are well established in developed countries, establishing an efficient program in low- and middle-income countries (LMICs) is still considered a significant challenge. Substantial obstacles usually face the initiation of fetal cardiac service from scratch in LMICs. The primary structural frame of a successful fetal cardiac program is described in detail, emphasizing the required team members. The potential challenges for starting fetal cardiac services in LMICs include financial, awareness-related, prenatal obstetric screening, sociocultural, psychosocial, and s

The maternal lifestyle in pregnancy: Implications for foetal skeletal muscle development.

The world is facing a global nutrition crisis, as evidenced by the rising incidence of metabolic disorders such as obesity, insulin resistance and chronic inflammation. Skeletal muscle is the largest tissue in humans and plays an important role in movement and host metabolism. Muscle fibre formation occurs mainly during the embryonic stage. Therefore, maternal lifestyle, especially nutrition and exercise during pregnancy, has a critical influence on foetal skeletal muscle development and the subsequent metabolic health of the offspring. In this review, the influence of maternal obesity, malnut

Critical Growth Processes for the Midfacial Morphogenesis in the Early Prenatal Period.

BACKGROUND: Congenital midfacial hypoplasia often requires intensive treatments and is a typical condition for the Binder phenotype and syndromic craniosynostosis. The growth trait of the midfacial skeleton during the early fetal period has been assumed to be critical for such an anomaly. However, previous embryological studies using 2-dimensional analyses and specimens during the late fetal period have not been sufficient to reveal it. OBJECTIVE: To understand the morphogenesis of the midfacial skeleton in the early fetal period via 3-dimensional quantification of the growth trait and invest

Do placental weights have clinical significance?

This study attempted to determine if placental size has implications for fetal/neonatal health and for subsequent childhood growth and development. 38,351 placentas were trimmed and weighed in a standardized way. The following factors were found to be associated with low placental weight: low maternal pregravid body weight, low pregnancy weight gain, high maternal hemoglobin levels during pregnancy, gestational hypertension, paid employment outside the home during pregnancy, and low parity. Taking these factors into consideration, placentas that were underweight for birth weight were associate

Fetal Growth Restriction and Its Metabolism-Related Long-Term Outcomes-Underlying Mechanisms and Clinical Implications.

The developmental origins of adult disease theory support the concept that undernourished fetuses are at risk of developing metabolic syndrome due to the energy-saving 'Thrifty Phenotype'. This metabolic plasticity represents an evolutionary adaptation that allows individuals to resist the intense pressure caused by cyclically recurring periods of nutritional deprivation. A comprehensive review was conducted following an extensive literature search in the PubMed/Medline and EMBASE databases concerning reports on fetal/intrauterine growth restriction and its metabolic-related long-term outcomes

The effect of maternal fatigue on the risk of fetal growth retardation.

OBJECTIVES: The purpose of this study is to investigate the influence of fatigue trajectories on fetal growth during pregnancy. METHODS: Pregnant women and their fetuses were recruited at 11-13 weeks of gestation. Fatigue was assessed using the Chalder Fatigue Scale. Fetal growth was monitored through three ultrasound examinations. Growth Mixture Models characterized fatigue trajectories during pregnancy, while Generalized Estimating Equations analyzed the relationship between fatigue and fetal growth. Latent Growth Curve Models further explored how changes in fatigue and fetal growth affecte

Measures of Fetal Growth and Cardiac Structure in Stillbirths With Placental Maternal Vascular Malperfusion: Evidence for Heart Weight Sparing and Structural Cardiac Alterations in Humans.

BACKGROUND: Placental maternal vascular malperfusion (MVM) is associated with fetal growth restriction (FGR). While FGR increases the risk of cardiovascular disease, the impact of MVM on fetal cardiac structure is understudied. METHODS: We utilized a cohort of autopsied stillbirths; 29 with MVM as the cause of death and 21 with a cause of death unrelated to MVM. Fetal and organ weights and heart measurements were standardized by gestational age and compared between MVM and non-MVM stillbirths. Differences in standardized fetal organ and cardiac measures as compared to standardized fetal body

Human placental development and function.

The placenta is a transient fetal organ that plays a critical role in the health and wellbeing of both the fetus and its mother. Functionally, the placenta sustains the growth of the fetus as it facilitates delivery of oxygen and nutrients and removal of waste products. Not surprisingly, defective early placental development is the primary cause of common disorders of pregnancy, including recurrent miscarriage, fetal growth restriction, pre-eclampsia and stillbirth. Adverse pregnancy conditions will also affect the life-long health of the fetus via developmental programming[1]. Despite its cri

[Fetal uropathies and pulmonary hypoplasia].

The author discusses the role of fetal uropathies in the development of pulmonary hypoplasia. The predisposing abnormalities may occur at various locations, including the kidneys and the efferent urinary ducts (ureters, urocyst, urethra). Following a brief general overview of the normal development of the urinary system, the developmental abnormalities are discussed. These anomalies interfere with the secretion of urine or, alternatively, may hinder or obstruct its elimination through the urinary ducts, thus leading to oligohydramnios. Inadequate production of amniotic fluid hinders the respir
