Point-of-Care Ultrasonography for Ankle Injuries in Children.

OBJECTIVES: The aim of the study was to determine whether point-of-care ultrasound (US) can decrease x-rays in children with ankle injuries. Secondary objectives were to determine the test performance characteristics for ankle US, analyze diagnostic errors, and compare US with the Ottawa Ankle Rules (OAR). METHODS: This was a prospective study of children younger than 21 years presenting to an emergency department with an ankle injury requiring x-rays. Pediatric emergency medicine physicians received a 1-hour training session, performed ankle US with a standardized scanning protocol of the di

The sonographic Ottawa Foot and Ankle Rules study (the SOFAR study).

INTRODUCTION: Foot and ankle injuries are common in the Emergency Department (ED). Of those requiring radiographs in accordance with the Ottawa Foot and Ankle Rules, approximately 22% have a fracture. Ultrasound is developing as a tool for emergency musculoskeletal assessment--it is inexpensive and rapid, and visualises soft tissue and bony structures. METHODS: This diagnostic cohort study examined if ultrasound could detect acute bony foot and ankle injuries. Ottawa Rules-positive patients over 16 years were eligible. An ultrasound scan (USS) was performed blind to radiograph findings by an

Ultrasound diagnosis of either an occult or missed fracture of an extremity in pediatric-aged children.

OBJECTIVE: To report and assess the usefulness of ultrasound (US) findings for occult fractures of growing bones. MATERIALS AND METHODS: For six years, US scans were performed in children younger than 15 years who were referred with trauma-related local pain and swelling of the extremities. As a routine US examination, the soft tissue, bones, and adjacent joints were examined in the area of discomfort, in addition to the asymptomatic contralateral extremity for comparison. Twenty-five occult fractures in 25 children (age range, five months-15 years; average age, 7.7 years) were confirmed by i

Roentgen stereophotogrammetric analysis of growth pattern after supination--eversion ankle injuries in children.

In a prospective study of ankle fractures in children, the growth rate was registered with a roentgen stereophotogrammetric method. The injuries were classified traumatologically according to Gerner-Smidt and anatomically according to Salter and Harris. Twenty-six ankle fractures were classified as supination--eversion injuries. Of these, eight had intraarticular injuries, 16 had extraarticular injuries of the distal tibia, and two had a zigzag fracture through the growth plate of the distal fibula. Information about growth was obtained within 3 months after fracture, and the growth pattern wa

Performance of two clinical decision rules for knee radiography.

We designed a prospective observational study to attempt to validate two recently described clinical decision rules for knee radiography. Consecutive patients aged > or = 15 yr with acute knee injuries occurring less than 1 wk prior to presentation were included for study. Patients with distracting conditions, open knee injuries, or previous surgery were excluded. Each patient was assessed for 7 historical and 15 physical examination criteria that were recorded on a standardized data collection instrument. Radiographs were ordered at the discretion of the attending physician and were read by t

Roentgen stereophotogrammetric analysis of growth pattern after supination-adduction ankle injuries in children.

In a prospective study of supination-adduction (SA) ankle fractures in children, the post-traumatic growth pattern was registered with a roentgen stereophotogrammetric technique allowing exact determination of the growth rate within 3 months after fracture. It was possible to predict future growth pattern within 6 months. The SA injuries (10 cases) were divided into two stages according to Gerner-Smidt. One Stage I injury (physeal separation through the distal fibula) showed a growth stimulation. Nine Stage II injuries (including a fracture through the medial malleolus) showed four types of gr

Assessing the Need for Additional Syndesmotic Stabilization in Open Reduction of the Posterior Malleolus: A Biomechanical Study.

BACKGROUND: The treatment of ankle fractures involving the posterior malleolus (PM) has changed in favor of open reduction and internal fixation (ORIF), and the need for additional syndesmotic stabilization has decreased; however, there are still doubts regarding the diagnosis and treatment of residual syndesmotic instability. The aim of the present study was to evaluate the effect of fixation of the PM and to assess the need for additional stabilization methods. We hypothesized that ORIF of the PM would not sufficiently stabilize the syndesmosis and that additional syndesmotic reconstruction

Simultaneous Correction of a Tibia Deformity and Non-union in Achondroplasia: A Case Report.

Tibial non-union with deformity in abnormal bone is rarely reported in literature. We report a case of a 65 years old male with a history of achondroplasia. The patient presented after a mechanical fall with an undisplaced right midshaft tibia fracture associated with pre-existing varus and procurvatum tibial deformities, which was initially managed non-operatively. However, after nine months he developed a painful non-union. Because of the symptomatic non-union as well as the pre-existing deformities, osteotomy of the tibia and fibula was performed with the application of a Truelok-Hexapod (T

Functional treatment and early weightbearing after an ankle fracture: a prospective study.

OBJECTIVE: Postoperative care for ankle fractures is generally 1 of 2 regimens: 1) functional treatment combined with early weightbearing (EWB), or 2) immobilization in a cast/orthosis for 6 weeks without weightbearing (6WC). The objective of this study was 2-fold: 1) to follow a prospective group treated with EWB as to long-term subjective and objective outcomes, and 2) to compare a subset of this group with a matched group of historic controls treated with 6WC. DESIGN: Prospective, clinical, cohort observation, and retrospective matched pair analysis. SETTING: University hospital, level 1

Management of Mandibular Ameloblastoma: A Case Report From a Bahrain Government Hospital.

Ameloblastoma is a locally aggressive yet benign odontogenic neoplasm. Its three main clinical variants - intraosseous, unicystic, and extraosseous - differ vastly in their biological behavior and recurrence rate. Here we present a case of intraosseous ameloblastoma in a 32-year-old male managed by wide en bloc excision. The mandible was reinforced with reconstruction plate to prevent pathological fracture, as the residual bone height was less than 1 cm. The patient has been on regular follow-up for one year with no clinical or radiological evidence of disease. This case demonstrates that marg

A unique case of segmental avulsion of mandible in pediatric patient: why is plastic surgery referral mandatory at initial visit?

The mandible is one of the most frequently fractured facial bone, most commonly by automobile accidents followed by fall from heights. However, segmental avulsion of mandible, that too in pediatric population is rarely reported. We report a case of 10-year-old male child sustaining facial injury following fall from a tree, resulting in complete segmental avulsion of the mandible highlighting the importance of early plastic surgery referral to optimize treatment outcomes. After clinical and radiological evaluation, the patient underwent free fibula osseocutaneous flap reconstruction. Given the

Vascularised bone grafts for the management of non-union.

Non-union of the long bones may have severe consequences, particularly when combined with other post-traumatic sequelae, such as tendon adhesions, reflex sympathetic dystrophy and infection, among others. In these cases, it is important to treat the delayed union or non-union first or at the same time with the other problems in order to achieve adequate function. Once the normal bony healing process has been slowed or stopped, it is necessary to provide both stability to the fracture site, as well as a biological stimulus for the fibrocartilagenous callus to finish the healing process. The additional value of weight-bearing radiographs in assessing stability of isolated type B fibular fractures.

INTRODUCTION: In isolated type B fibular fractures, the decision whether to operate or treat conservatively is principally based on congruency and stability of the ankle joint. The purpose of the current study is to examine the additional diagnostic value of the weight-bearing radiograph (WBR) in assessing stability of potentially unstable type B fibular fractures. MATERIALS AND METHODS: In this retrospective cohort study, patients were selected based on who presented an isolated type B fracture of the fibula. Unstable fractures were directly planned for surgery. Other patients underwent addi

Does Proximal Placement of the Syndesmotic Reduction Clamp Affect the Optimal Position for the Medial Tine? A Cadaveric Pilot Study.

Sagittal plane syndesmotic malreduction is associated with off-axis, eccentric reduction clamping and preferential placement of the medial tine anteriorly has been proposed to minimize the malreduction risk. Although clamp placement 1cm proximal to the plafond is recommend, no previous study has assessed whether differences in the anatomic position of the fibula within the incisura (eccentric 1cm superior and concentric 2 cm superior to the tibial plafond) affect the optimal position for the clamps medial tine during reduction of the syndesmosis. The purpose of the present cadaveric pilot stud

Maisonneuve Equivalent Injury With Proximal Tibiofibular Joint Dislocation: Case Report and Literature Review.

We report the case of a 31-year-old male who presented with traumatic dislocation of both proximal and distal tibiofibular joints without fibular fracture. The patient underwent closed reduction of the proximal tibiofibular joint and surgical stabilization of the distal tibiofibular joint. An 18-month postoperative evaluation confirmed a very good clinical outcome after anatomic reduction. This type of trauma is very rare and, to our knowledge, has only been described 3 times previously. Furthermore, our surgical management was different from that used for the previous cases and proved to be s

Paediatric open tibiofibular fractures following a donkey bite. A report of two cases.

Open tibiofibular fractures resulting from domestic animal bites are exceptional. We report two such cases, in patients of 11 and 13 years of age, and discuss the etiological, pathogenic, anatomic, and clinical aspects of these injuries. These were type III fractures according to the Cauchoix and Duparc classification. The fractures were treated with emergency wound care, wound dressing, and conservative orthopaedic treatment using a long leg cast for immobilization. Preventive parenteral antibiotics were also given. Evolution was favorable in both cases and the patients returned to their norm

[Advances of screw internal fixation for the treatment of distal tibiofibular syndesmosis injuries].

The distal tibiofibular syndesmosis is important for ankle mortise stability, weight transmission and walking. Incorrect treatment is correlate with chronicd ankle instability, long-term pain and the development of osteoarthritis. Some controversy still exists regarding the evaluation and treatment of syndesmotic injuries with screws. The goal of this review was to collect evidence on the technical aspects for the treatment of distal tibiofibular syndesmosis injuries with screw and to formulate some recommendations for clinical practices.

Reverse-flow vascularized fibular graft: a new method.

The reverse-flow island flap is a relatively recent concept. We have applied this concept to the vascularized bone graft. We report a new method of the reverse-flow vascularized fibular graft for two patients with a pseudarthrosis and massive bone defect of the tibia. The peroneal artery and venae comitantes were severed proximally and elevated with the fibula while maintaining distal vascular continuity. The reverse-flow vascularized fibula was grafted to the posterior aspect of the bone defect of the tibia. Bony union was obtained in both cases. The reverse-flow vascularized fibular graft ha
