Treatment principles for proximal and middle phalangeal fractures.

After a proximal phalangeal fracture, optimal results are obtained by methods that permit active interphalangeal joint motion and tendon gliding during fracture healing. Typical apex palmar angulation of proximal phalangeal fractures demonstrates dorsal skeletal shortening and secondary incompetence of the extensor mechanism with PIP joint extensor lag. Apex palmar deformities of the middle phalangeal fractures demonstrate similar problems with skeletal shortening resulting in loss of distal joint extension. Proximal and middle phalangeal shaft fracture deformities rotate about their flexor te

Comparison of hand structure and function in early and late term dental students.

BACKGROUND: The ability of dentists to perform complex surgical procedures and minimize tissue damage during treatment requires a certain level of dexterity, hand grip strength, and upper extremity motor coordination. AIM: The aim of this study was to investigate the effect of the vocational training process on the structural and functional status of the hand in dentistry students. METHODS: The study was carried out on Term 1 (n = 36) and Term 5 (n = 34) dentistry students. The anthropometric measurements of the hands and wrists of the participants were measured by a Martin-type anthropomete

Teeth syndrome: diagnosis, complications and management.

Teeth syndrome or fight bite is a specific entity in hand surgery that is little known. It includes infectious complications of the hand following a fist against the teeth. Neglected or misdiagnosed this injury frequently leads to serious complications that could compromise the function of the hand. A retrospective study was performed on 20 patients treated for teeth syndrome at our department, during a period of 12 years (January 2003 to April 2015). All young adults with a mean age of 28 years and a significant male predominance. The dominant side was involved in 15 patients. Lesions were di

Fracture dislocation of carpometacarpal joints: a missed injury.

Fracture dislocation of the carpometacarpal joints on the ulnar side of the hand is an uncommon injury. These are high-energy injuries seen in motorcyclists and boxers. The mechanism of injury involves violent, forceful dorsiflexion of the wrist combined with longitudinal impact on the closed hand. This article reports a case of fracture of the base of the middle finger with dislocation of the ring and little finger carpometacarpal joints. On first examination, a diagnosis of isolated, minimally-displaced, middle-metacarpal base fracture was made and deemed suitable for nonoperative management

The Influence of Hand Dominance on the Degree of Deformities in Patients With Systemic Lupus Erythematosus and Jaccoud Arthropathy.

BACKGROUND/OBJECTIVE: Joint deformities in Jaccoud arthropathy (JA), associated with systemic lupus erythematosus (SLE), can lead to a reduction in articular function and an impaired quality of life. There is controversy in the literature as to whether hand dominance contributes to deformities observed in rheumatoid arthritis and other forms of rheumatism. The purpose of this study was to assess whether hand dominance has any influence on the degree of deformity or joint range of motion in patients with JA associated with SLE. METHODS: This was a pilot cross-sectional study of 22 female patie

A post hoc analysis of Dupuytren contracture treated with collagenase Clostridium histolyticum across disease stages.

This post hoc analysis from a multicenter study (NCT01674634) was designed to evaluate the efficacy of collagenase Clostridium histolyticum (CCH) treatment in patients with different stages of Dupuytren contracture. Previously untreated patients who received two concurrent injections of CCH in two affected joints in the same finger were assessed by disease severity (Tubiana stage). The mean (SD) improvement in total fixed flexion contraction (FFC) 31 days post-CCH treatment in 181 patients was: 71.1 (36.5)% for Tubiana I, 77.0 (21.0)% for Tubiana II, 72.0 (20.4)% for Tubiana III and 66.4 (22.2

The i-LIMB hand and the DMC plus hand compared: a case report.

The i-LIMB hand is a novel upper limb myoelectric prosthetic hand with several joints in the fingers and thumb. This study aimed to determine whether this new device had more functionality than a more conventional myoelectric prosthetic hand with only a single joint between the thumb and two fingers. Therefore, a 45-year-old man with a wrist disarticulation used the i-LIMB hand and the widely used Dynamic Mode Control hand (DMC plus hand) in a test procedure that covered all functional levels of the International Classification of Function (ICF). Functional outcomes of the i-LIMB seemed to be

Open versus percutaneous release of the A1-pulley for stenosing tendovaginitis: a prospective randomized trial.

A prospective randomized trial for release of the first annular pulley (A-1 pulley) in trigger fingers with a percutaneous technique versus the open surgical technique is presented. Thirty-six patients were randomized to either open (n = 16) or percutaneous (#15 blade; n = 20) release of the A-1 pulley. All patients were evaluated for grip strength, active range of motion of the proximal interphalangeal joint, and residual pain at 1 and 12 weeks after release. Furthermore, the operation time was assessed, and the costs were calculated. Overall, 100% success in terms of grip strength, active ra

Reduction techniques in distal radius fractures.

External skeletal fixation is an important minimal invasive procedure in the management of fractures at the distal end of the radius. Attention to detail is important not only in the recognition of indications and function of the external fixator but also in its specific application. To improve anatomical restoration (e.g. palmar tilt) multi-planar ligamentotaxis is recommended. The demonstrated dynamic fixator is easy to apply and the double ball joint facilitates reduction after mounting by multi-planar ligamentotaxis. This transarticular unilateral external fixation system permits restorati

Management of simple camptodactyly.

From a review of 57 patients with flexion deformity of the fingers (camptodactyly), 21 patients with 38 digits treated operatively had 18% good or excellent results, whereas 14 patients (41 digits) treated conservatively had 66% good or excellent results. Mild deformities responded well to splints and stretching. Moderate deformities treated operatively gained extension but at the loss of finger flexion. Severe deformities had improvement in extension (averaging 19 degrees in operative cases and 27 degrees in conservative), but there was significant loss of flexion in the operative group. Over

Histopathology of Gottron's papules--utility in diagnosing dermatomyositis.

Dermatomyositis (DM) is an uncommon connective tissue disease that presents with a characteristic violaceous skin eruption as well as proximal muscle weakness, primarily of the upper extremities. Cutaneous stigmata of DM include Gottron's papules, similarly colored papules and plaques overlying the extensor surfaces of finger joints. While biopsy of the typical poikilodermatous skin eruption found in patients with suspected DM is a standard algorithmic component in the workup and diagnosis of the disease, Gottron's papules are rarely sampled for histopathologic assessment. The precise reason f…

Differences in the coordinative structure of object manipulation between 3- and 6-year-old children: kinematic analysis with markerless pose estimation.

Object manipulation is fundamental to daily activities and undergoes marked developmental changes during early childhood. This study examined differences in the coordinative structure of object manipulation during a bead-threading task in typically developing children aged 3 and 6 years. Participants were enrolled in a longitudinal follow-up study and consisted of 25 three-year-olds and 22 six-year-olds. Kinematic data were obtained from overhead video recordings and analyzed using markerless pose estimation. Joint angle time series of the upper limb and digits were extracted, and manipulation

Bone grafting combined with a spiral flap technique for the reconstruction of fingertip amputations.

BACKGROUND: Various reconstructive options exist for distal finger and pulp defects, including grafting and local or distant flaps. In addition to reconstructing the normal anatomical structure, preserving the sensory function of the finger is crucial. This study presents the results of using bone grafting combined with a spiral flap (BGcSF) technique for reconstructing pulp defects accompanied by bone loss. METHODS: Twenty-three patients with fingertip defects were treated using the BGcSF technique. Flap sensitivity was assessed us-ing the Semmes-Weinstein monofilament (SWM) and static two-p

A human, allogeneic cortical bone screw for distal interphalangeal joint (DIP) arthrodesis: a retrospective cohort study with at least 10 months follow-up.

INTRODUCTION: The prime requisites of a good digital arthrodesis are a painless and stable union in a proper position. Arthrodesis of the distal interphalangeal joint of the fingers is not without potential complications including nonunion, malunion, and deep tissue infections. The Shark Screw® is a human, cortical bone allograft for osteosynthesis and an alternative to metal or bioabsorbable devices in orthopedics and trauma surgery. The primary hypothesis is that the fusion and complication rate, using the Shark Screw®, is at least similar to those reported in the literature, using metal or

Flexion contractures of fingers: contracture elimination with trapeze-flap plasty.

Scar flexion contracture of fingers is one of the most serious consequences of hand burns and patient disability after burn. Many kinds of reconstructive techniques are currently used and new procedures are being investigated. The author presents a new method of finger contracture reconstruction developed in the process of burn reconstructive operations on hands of over a thousand patients. Finger flexion contractures are caused by a semilunar fold, both sheets of which are scars. The sheets have a surface deficiency in length, which causes a contracture, and excess of skin in width, which all

Relative contributions of neural mechanisms versus muscle mechanics in promoting finger extension deficits following stroke.

The origins of impaired finger and hand function were examined in 10 stroke survivors with chronic spastic hemiparesis, with the intent of assessing whether mechanical restraint or altered neurophysiological control mechanisms are responsible for the well-known impairment of finger extension. Simultaneous extension of all four metacarpophalangeal (MCP) joints of the impaired hand was either externally imposed using a rotary actuator or attempted voluntarily by the subject. Trials were conducted both before and after administration of a local anesthetic, blocking the median and ulnar nerves at

Closed reduction of multiple MCP joint dislocations.

We report a young man in his 20s who fell from the roof of his house and sustained injury to his left hand. Physical examination and radiographic evaluation revealed closed dislocations of the second, third, and fourth metacarpophalangeal (MCP) joint with fractures of the bases of the third and fourth metacarpals and the base of the proximal phalanx of the middle finger. The injury was managed with closed reduction and splintage with an uneventful outcome. Multiple MCP joint dislocations are rare, and there are only a few reports of closed reduction with a good outcome.

Bone Scan With SPECT/CT Demonstrated C1 to C2 Involvement in Rheumatic Arthritis.

An 80-year-old man was treated with rituximab for active rheumatoid arthritis until 2019, now controlled with Salazopyrin, prednisolone, methotrexate, and folic acid. However, laboratory data showed elevated C-reactive protein and erythrocyte sedimentation rate. Whole-body bone scan showed bony and joint destruction to the upper cervical vertebra (C spine), bilateral shoulders, wrists, finger joints, ankles, and left knee. SPECT/CT localized the upper C spine uptake to the C1/C2 joint and adjacent C1 and C2 with C1/C2 subluxation. C spine CT showed vertical atlantoaxial subluxation and bony er…
