[Macro- and microreplantations of the lower leg and foot].

Amputations suitable for replantation in the distal lower leg and foot are seldom. There is no indication if shortening will exceed 5 cm or loss of sensitivity is to be expected. Forefood and toe replantations are to be seen predominantly under aesthetic points of view. Satisfactory results in 10 own cases could be achieved if indications for lower leg replantations were limited to guillotine-type amputations, children and some bilateral lesions. Polytrauma as a rule is not a good indication.

[Morphology and diagnosis of bone tumors of the foot].

We report on 554 bone tumors and tumor-like lesions of the foot, which have been diagnosed within the period from 1945-1985 in the Department of Diagnostic Radiology, in the Surgical and Orthopedic Departments and in the Division of Pathology of Bone Tumors of the University of Freiburg. Schajowicz' WHO classification criteria for bone tumors were used. Morphological aspects, clinical symptoms and pattern of distribution differ widely from lesions of other skeletal regions. Bone tumors of the foot are much more frequently associated with pain caused by the special anatomy of the foot. Malignan…

[Static disorders or venous pathology of the foot?].

UNLABELLED: Three types of disorder observed in the foot may have either a mechanical or a vascular aetiology. They are: A) Painful or tired legs, caused by: eversion, inversion or arch collapse of one or both feet. TREATMENT: corrective or compensatory soles depending on the joint mobility and the integrity of the skin. B) Metatarsalgia due to static disorders, incorrect shoes or vascular disorders. TREATMENT: orthopaedic soles, appropriate shoes. C) Burning pain in the toes exacerbated by heat, relieved by cold, due to claw or lateral deformity of the toes or compression of the forefoot an…

Rehabilitation of the person with arthritis of the ankle and foot.

When combined with skillful pharmacologic management, applying the full stock of biomechanical principles available to the rheumatoid foot and ankle is very effective in avoiding the need for surgery and slowing or preventing deformity. The general principles of arthritis rehabilitation also help restore function. When good medical management, joint protection, exercise, and energy conservation techniques are combined with psychosocial vocational therapy as needed, the crippling loss of function so often seen in the past can be largely avoided.

The STA operation for the pronated foot in childhood.

The STA operation is a modified subtalar arthroereisis that uses an endoprosthesis manufactured from ultrahigh-molecular-weight polyethylene. It is fashioned into the shape of a small peg and implanted into the dorsal surface of the calcaneus. Its purpose is to eliminate abnormal pronation, correct heel valgus, and produce an increase of the medial longitudinal arch.

A histochemical study of muscle in club foot.

A histochemical analysis was made of 103 muscle biopsies taken from 62 patients with idiopathic club feet. Any reduction in the diameter of … mber of fibres may be caused by a defective neural influence on the development of the limb in club foot.

Non-infective disease of the diabetic foot.

Six different conditions of non-infective bone and joint pathology have been seen amongst 67 patients with diabetic neuropathy. The characteristics of each are described. Not all the conditions require treatment but they should be differentiated from osteomyelitis. Charcot osteoarthropathy is the most common condition seen but spontaneous fractures and dislocations generally present greater therapeutic problems. The aim of treatment should be to obtain a stable foot in which there is no undue pressure on the skin from a bony prominence.

Surgery of the rheumatoid foot: preferable procedures.

Management of patients who have rheumatoid arthritis is best accomplished by a team approach utilizing the help of the family physician, the rheumatologist, the physical therapist and the orthopaedist. Such an approach insures careful screening and proper selection of those who can be helped by surgical procedures. Those who have painful feet welcome surgical intervention if nonoperative methods do not furnish relief. The forefoot is often involved and severe deformities of the metatarsophalangeal joints with painful plantar calluses are frequent. The deformities can be corrected and the callu…

The foot and ankle in multiple hereditary exostoses.

Twenty-two cases of multiple hereditary exostoses revealed pathological changes in two areas: the first group caused by epiphyseal disturbances, and the second group due to mechanical problems created by the exostoses. The epiphyseal disturbances resulted in ankle valgus, shortened metatarsals, and angular deformities of the necks of the metatarsals. The exostoses produced local tenderness, synostosis of the medical subtalar facet resulting is loss of subtalar motion, and asymptomatic synostosis of the lower tibial fibular syndesmosis.

Amputations of the foot and ankle. Current status.

Doppler assessment of ankle and foot systolic pressure is a practical aid in the evaluation of neurocirculatory problems. If the ratio of ankle pressure to arm pressure is over 0.45, more than 90 per cent of operative procedures will heal. Intraoperative testing of blood supply aids in determining level of amputation. Healing results in 80 per cent of cases if bleeding occurs in the skin within three minutes after release of tourniquet. With vigorous debridement of infected soft tissue and bone, the remaining tissue will often heal in local foot operations. A high percentage of patients with n…
