Simultaneous bilateral anterior fracture dislocation of the shoulder with neurovascular injury: report of a case.

This paper reports an unusual case of orthopedic trauma in a 29-year-old man who experienced concomitant bilateral anterior shoulder fracture dislocation with associated brachial plexus and axillary artery injury. We also highlight the mechanism underlying this rare workplace injury related to use of a forklift.

Anterior traumatic shoulder dislocation associated with displaced greater tuberosity fracture: the necessity of operative treatment.

OBJECTIVE: To evaluate the radiographic and clinical outcome, including the incidence of recurrence, in patients with displaced greater tuberosity (GT) fractures associated with a traumatic anterior shoulder dislocation. DESIGN: Retrospective study. SETTING: University Hospital (Level 1 trauma center). PATIENTS: There were 34 completely evaluated patients (19 male, 15 female) seen between 1993 and 2002 with a displaced GT fracture associated with a traumatic anterior shoulder dislocation. Average age was 52.8 years and the mean follow-up period was 4.8 years (range, 2.0 to 10 years). INTER

Unreduced chronic dislocation of the humeral head with ipsilateral humeral shaft fracture: a case report.

The purpose of reporting this case is to illustrate a treatment plan for a chronically anteriorly dislocated shoulder associated with an ipsilateral humerus fracture, a condition heretofore not addressed in the literature to our knowledge. An 18-year-old female, left hand dominant, injured her left upper extremity and liver in a motor vehicle accident. X-rays at time of injury revealed a diaphyseal facture of her left humerus. No x-rays of the shoulder were taken at time of injury. Treatment consisted of a plaster cast application and discharge at 1 week. The patient was seen again 4 weeks pos

[Ipsilateral femoral fracture and anterior hip dislocation in a 15-year-old girl: case report].

Operative management of concomitant ipsilateral femur fracture and anterior hip dislocation has not been previously described in the literature. We report the case of a 15-years-old girl who was injured in a motorcycle accident and presented with a femoral shaft fracture and a concomitant ipsilateral anterior hip dislocation. Operative management consisted of an attempted closed reduction of the femoral fracture, which was unsuccessful; thus, an external fixator was temporarily applied. Subsequently, the hip dislocation was treated by open reduction through an anterolateral approach. Finally,

Irreducible dorsal radiocarpal fracture dislocation with dissociation of the distal radioulnar joint: a case report.

We report a case of dorsal radiocarpal fracture dislocation with dissociation of the distal radioulnar joint. Closed reduction was unsuccessful due to interposition of the osteochondral fragments and open reduction and fixation was carried out with a satisfactory end result. The advantages of volar approach and use of external fixator in the management of this injury are discussed.

Acute disruption of the posterior tibial tendon associated with open-fracture dislocation of the ankle.

A 27-year-old man with a high-energy, open-fracture dislocation of the ankle underwent debridement and open reduction of his injuries. During surgery a disruption-laceration of the posterior tibial tendon at the level of the fracture of the medial malleolus was found. Repair required location of the proximal muscle and tendon and controlled traction of the musculotendinous unit from its retracted position. Common aspects in the nine previously reported instances of this lesion include relatively high energy of the injury and the fracture type, a transverse fracture in the medial malleolus. Uni…

Fracture of the entire posterior process of talus associated with subtalar dislocation: a case report.

This is a case report of a 52-year-old woman who sustained a medial subtalar dislocation with fracture of the posterior process of the talus in a traffic accident. After closed reduction of the subtalar dislocation, tomography demonstrated that the talus fracture involved the entire posterior process and the posterior portion of the talar body. The fracture of the talus was treated with an open reduction and miniscrew fixation. At follow-up examination 32 months later, the functional and radiographic results were graded as good. The proposed mechanism of this case was a forced plantarflexion a

Ejection of the proximal scaphoid in a trans-scaphoid perilunate fracture dislocation. A case report.

Trans-scaphoid perilunate fracture dislocations are difficult injuries to treat. Described herein is the unusual case of a trans-scaphoid perilunate fracture dislocation in which the proximal scaphoid fragment was ejected into the volar aspect of the distal forearm. The significant displacement of the proximal fragment represented complete devascularization of this piece. This unique fracture raised difficult management issues in the treatment of this injury. A primary proximal row carpectomy was performed, with a successful clinical result.

[Proximal fibulo-tibial dislocation and ischemia associated with a leg fracture. Apropos of 2 cases].

Proximal fibulotibial dislocation may remain escape detection in leg injuries. Posterior dislocations are unusual and are associated with shortening of the limb due to the tibial fracture. Two cases of such dislocation are reported, associated with lower limb ischemia due to an injury of the arterial popliteal trifurcation. An anatomical model indicates the mechanism of this lesion. Such an unusual association has not be found elsewhere in literature.

"Ratchet-type" marginal fracture of the radial head with locked anterior dislocation. A case report.

Locked anterior dislocation of the superior radioulnar joint occurred in a 26-year-old man. The configuration of the associated radial head fracture maintained the locked position and predisposed to recurrence after open reduction. Radial head excision was necessary to resolve the problem. Both medial ligament repair and a radial head prosthesis were necessary to stabilize the elbow.

Posterior spinal osteosynthesis for cervical fracture/dislocation using a flexible multistrand cable system: technical note.

Cervical instability secondary to fracture/dislocation or traumatic subluxation involving the posterior elements may be treated by a variety of fusion techniques. The rigidity of the stainless steel wires used in posterior cervical fusions often leads to difficulty with insertion, adequate tension, and conformation of the graft construct. This report describes a technique of posterior cervical fusion employing a wire system using flexible stainless steel cables. The wire consists of a flexible, 49-strand, stainless steel cable connected on one end to a short, malleable, blunt leader with the o

Fatigue fracture of the inferior pubic ramus following total hip replacement for congenital hip dislocation.

Fatigue fracture of the inferior pubic ramus occurred following total hip arthroplasty (THA) for congenital dislocation of the hip 9 months postoperative in a 71-year-old woman. When anatomic restoration is attempted by total hip arthroplasty for congenital dislocation of the hip in an aged individual, new stress patterns are created about the joint.

Prospective evaluation of a pragmatic treatment rationale: open reduction and internal fixation of displaced and dislocated condyle and condylar head fractures and closed reduction of non-displaced, non-dislocated fractures. Part I: condyle and subcondylar fractures.

This prospective study evaluated outcomes of closed reduction (CR) in non-displaced, non-dislocated condyle and subcondylar fractures (Class I) and open reduction and internal fixation (ORIF) of displaced (Class II) and dislocated (Class IV) fractures. Forty-five patients with 51 fractures (six (13%) with bilateral fractures), 11 (25%) CR and 34 (75%) ORIF, were enrolled in a 1-year follow up that 20 patients with 25 fractures completed. Condylar translation in Class I fractures recovered to 12mm for vertical opening, 9mm for protrusion, 8mm for mediotrusion; in Class II, respectively, 10, 7 a

[Importance of initial fracture crack width in minimally dislocated fractures of humeral lateral condyle in children for evaluation of fracture stability and treatment choice].

INTRODUCTION: The fracture of the external condyle is the most common fracture of the distal end of the humerus. This is an intraarticular fracture, which, if not properly treated, can cause serious complications, difficult to treat. OBJECTIVE: To define the importance of the initial width of the fracture crack for the evaluation of stability of the minimally dislocated fractures of the humeral lateral condyle and for the selection of the method of treatment. METHODS: The target group included the children with minimally dislocated fractures or fractures of uncertain stability. The number of

Comminuted Radial Head Fracture in All-arthroscopic Repair of Elbow Fracture-dislocation: Is Partial Excision of the Radial Head an Acceptable Treatment Option?

BACKGROUND: In elbow fracture-dislocation, partial excision of the comminuted radial head fracture that is not amenable to fixation remains controversial considering the accompanying symptoms. This study was undertaken to evaluate the results of radial head partial excision when the comminuted radial head fracture involved <50% of the articular surface in all-arthroscopic repair of elbow fracture-dislocation. METHODS: Patients were divided into two groups based on the condition of the radial head fracture. In Group A, the patients had a radial head comminuted fracture involving <50% of the ar
