Fine needle aspiration cytology in diagnosing tubercular mastitis: A rare entity with differential diagnoses and learning points.

INTRODUCTION: Extra-pulmonary tuberculosis occurring in the breast is a rarity. The clinical presentation is multifaceted, often mimicking malignancy and pyogenic breast abscess. Fine needle aspiration cytology (FNAC) is an essential component of the triple test in evaluating breast lumps. METHODS: The present study is a retrospective analysis of 18 cases of tubercular mastitis (TM) over two years who underwent FNAC and were found to respond to anti-tubercular therapy on follow-up. Air-dried and alcohol-fixed smears were made and stained with May Grünwald Giemsa and hematoxylin and eosin stai

Vascular cell tumors of the hand in children.

The diagnosis of vascular cell tumors of the upper limb in children can be extremely difficult. There is considerable variation in their presentation and natural history. In most instances, the correct diagnosis can be achieved after a careful history and examination. Almost invariably, they are benign lesions and, of these, hemangiomas and vascular malformations account for more than 90%. Worrying tumors of the intermediate and malignant grade are rare. Any atypical, rapidly growing tumor with superficial ulceration and bony destruction on radiology should be regarded as malignant. Histologic

Glomus tumor of the right little finger: a case report and review of diagnostic challenges.

INTRODUCTION AND IMPORTANCE: Glomus tumors are rare, benign growths originating from the glomus body, typically presenting with severe pain, pinpoint tenderness, and sensitivity to cold. Their small size and rarity often lead to misdiagnosis, with an average delay of 7 years. CASE PRESENTATION: This report details a 37-year-old man experiencing burning pain in his right little finger, aggravated by cold, for 6 months. Magnetic resonance imaging (MRI) showed a lesion indicative of a glomus tumor, confirmed by histopathology after surgical excision. CLINICAL DISCUSSION: Differential diagnoses

Acral melanoma foot lesions. Part 2: clinical presentation, diagnosis, and management.

Acral melanoma (AM) is a rare subtype of cutaneous malignant melanoma found on acral skin, primarily on the soles of the feet. Although rare, it is the most common subtype of melanoma found in patients of African or Asian ethnicity and has a poor prognosis, often because of the more advanced stage of presentation at diagnosis. In the second of this two-part series, we review the clinical presentation, histopathology, diagnosis and management of AM. Clinically, AM presents as a variegated lesion with blue-black pigment and irregular borders on acral skin. A parallel-ridge pattern is a very spec

Subungual Exostosis in a Young Soccer Player.

BACKGROUND: Subungual exostosis is a relatively uncommon, benign osteocartilaginous tumor of the distal phalanx of the toes or fingers in young adults, considered as a rare variant of osteochondroma. Differential diagnoses include subungual verruca (viral wart), pyogenic granuloma, osteochondroma, amelanotic subungual melanoma and glomus tumour. Misdiagnosis and total onychodystrophy frequently occur as a result of late treatment or inadequate treatment strategy. Dermoscopy could be a useful technique, involved in the diagnostic process, although X-ray examination and histopathology are mandat

Oral and vulvovaginal changes in pregnancy.

Physiologic alterations of the oral and vulvovaginal mucosal surfaces result from the profound hormonal and immunologic changes of gestation. High estrogen levels are responsible for the vascular changes noted on mucosal surfaces. Gingival hyperemia and edema, gingivitis and pyogenic granuloma are the most common alterations of the oral mucosa during gestation. Physiologic changes of the vulvovaginal area are mainly of vascular nature, and include among others varicose veins. The oral and vulvovaginal mucosal surfaces can be affected by diseases that can worsen or develop in pregnancy. Oral le

[Malignant melanoma in children and adolescents].

According to data from the literature and to new data from the German central registry for malignant melanomas from the German Society of Dermatology, a summary is given about the occurrence of malignant melanoma in children and adolescents up to 18 years of age. During the first 10 years of life the occurrence of a melanoma is extremely rare, but slowly increases after the first decade. Several studies have shown that young people often have amelanotic tumors resembling pyogenic granuloma. This correlates with a histopathological high number of nodular melanomas in this age group. A reason mi

Transcaruncular medial canthal ligament plication for repair of lower eyelid malposition.

OBJECTIVE: To evaluate the long-term efficacy of transcaruncular medial canthal ligament (MCL) plication in the treatment of eyelid malposition. METHODS: Transcaruncular MCL plication was performed on 176 eyelids of 125 patients with symptomatic ocular exposure due to lower eyelid malposition in which MCL laxity was an important component. Preoperative and postoperative ocular exposure symptoms, lower eyelid position, lagophthalmos, and keratopathy were compared. RESULTS: At an average +/- SD follow-up time of 25 +/- 27 months (range, 1-103 months), 88% of preoperative symptoms resolved or i

Isolated caruncular approach for orbital decompression.

PURPOSE: To evaluate the effectiveness of the isolated caruncular approach to orbital decompression of thyroid ophthalmopathy. METHODS: In a retrospective, noncomparative, interventional case series, we reviewed the records of 29 patients (48 orbits) who had thyroid ophthalmopathy and had undergone orbital decompression using the caruncular approach. The medial wall was decompressed in two patients (three orbits), and the medial and inferior walls were decompressed in 27 patients (45 orbits). RESULTS: The mean retrodisplacement achieved was 2.7 mm of decompression of the medial wall, and 4.2

Intravascular fasciitis of the forearm vein: a case report with immunohistochemical characterization.

Intravascular fasciitis is a very unusual variant of nodular fasciitis. A unique case of this lesion occurring in the proximal portion of the superficial vein of the forearm in an otherwise healthy 26-year-old man is reported. The intravascular polypoid lesion grew longitudinally along the vascular lumen, was loosely attached to the intimal layer, and was partly anchored beyond the internal elastic lamina into the medial smooth muscle layer. However, extravascular involvement was absent. The histological features were identical to those observed in ordinary cellular nodular fasciitis. Because

Intraoral lobular capillary hemangioma in the esthetic zone.

Lobular capillary hemangioma (LCH), previously known as pyogenic granuloma, is a benign vascular lesion commonly affecting the oral cavity. Despite its misleading name, LCH represents a reactive hyperplastic proliferation of capillaries, often triggered by local irritation or hormonal changes. A 30-year-old male presented with a painless, fibrous, pedunculated swelling between the maxillary central incisors, causing pathological tooth migration and difficulty in mastication. Clinical and radiographic examination revealed no bony involvement, and a provisional diagnosis of LCH was made. The les

Lobular capillary hemangioma with an intraosseous feeder vessel through an accessory buccal foramen in the mandible: A unique case report.

The aim of this case report is to present a unique and rare finding in a 29-year-old male with the chief complaint of swelling in the gums related to the back teeth of the lower left jaw, since 20 days. In the absence of any alarming findings on the intraoral periapical radiograph and blood profile, an excisional biopsy of the provisionally diagnosed pyogenic granuloma was planned with a high-power diode LASER (Light Amplification by Stimulated Emission of Radiation). Following the excision, an unusual amount of bleeding was encountered from a single point on the buccal cortical plate between
