Stroke volume variation during acute normovolemic hemodilution.

BACKGROUND: The intravascular volume of surgical patients should be optimized to avoid complications associated with both overhydration and underresuscitation. In patients undergoing intraoperative acute normovolemic hemodilution, we investigated whether stroke volume variation (SVV) derived from an arterial pressure-based cardiac output (CO) monitor system (FloTrac/Vigileo, Edwards Lifesciences, Irvine, CA) tracked the changes associated with blood removal and replacement. We further evaluated the correlations between SVV and 3-dimensional (3D) transesophageal echocardiographic (TEE) left ven

Graft vascular disease in the great vessels and vasa vasorum.

To determine whether the great vessels were subject to transplantation-associated arteriosclerosis (graft vascular disease) we studied sections of aorta and pulmonary arteries from 19 cardiac explant cases. Sections of the same vessels from five autopsied hearts from patients who had received liver and/or kidney transplants with immunotherapy as well as from eight recipient hearts served as controls. In addition, eight donor hearts (three aortae and eight pulmonary arteries) that were not used for heart transplantation were examined. Intimal proliferation was noted in the aorta but affected th

Intrauterine supraventricular tachycardia.

Six examples of intrauterine supraventricular tachycardia together with 31 previously reported cases are described and analyzed. Among the 37 infants, structural heart disease was present in only four (11%), three of whom died. Males comprised 68% of the group without identifiable heart disease or pre-excitation. Congestive heart failure was evident in 62% of the infants at birth or shortly thereafter; ascites was the predominant finding in three (8%). Neither the duration of SVT nor heart rate was predictive of the clinical status at birth. Infants without underlying heart disease or conducti

A review of congenital heart block.

Congenital heart block is a rare disorder. It has an incidence of about 1 in 22,000 live births. It may be associated with high mortality and morbidity. This should generate a high index of suspicion for early diagnosis and aggressive therapy when appropriate. The congenital heart block associated with neonatal lupus is considered a form of passively acquired autoimmune disease in which maternal autoantibodies to the intracellular ribonucleoproteins Ro (SS-A) and La (SS-B), cross the placenta and injure the previously normal fetal heart. Women with serum titers of anti-Ro antibody carry a 3% r

Rating of perceived exertion as a tool for managing exercise intensity during pregnancy: a scoping review.

OBJECTIVE: During pregnancy, international guidelines recommend ≥150 min of moderate-intensity aerobic physical activity per week, with an intensity perceived as fairly light to somewhat hard on the Borg Rating of Perceived Exertion (RPE) scale (ranging from 6 'no exertion' to 20 'maximal exertion', corresponding to 60% to 80% of maximum heart rate). However, the determination and monitoring of exercise intensity seem to be a particular source of confusion, and the most effective method to monitor exercise intensity remains uncertain. This study aimed to examine existing research on the correl

Normal fetal motility: an overview.

After 35 years of real-time two-dimensional sonography, and now that 4D sonography is within our grasp, this article presents an overview of present-day knowledge of normal fetal motility. A literature search was carried out on articles from 1970, using the keywords: 'fetal', 'movements', 'motility', 'movement patterns', 'ultrasound' and 'sonography'. Inclusion criteria were human studies and use of real-time sonography. Articles were screened for type of motor assessment procedure, in terms of whether they: specified movements for participating body parts (specific movement pattern, SMP), wer

Changes in Fitness Parameters in Ridden Trained Showjumping Horses After Healing of Gastric Ulcers: Preliminary Results.

Equine Gastric Ulcer Syndrome (EGUS) is a worldwide disease described in different categories of animals. A causal relationship between exercise and EGUS has been suggested, attributed to stress as well as physiological changes in the position of abdominal organs, especially during trot and gallop. EGUS can influence athletic fitness, as has been described in various papers on racehorses. The aim of this work was to determine if gastric ulcer healing is associated with changes in selected fitness parameters (i.e., speed, heart rate during exercise and recovery, stride length and frequency) in

Effect of forest walking on autonomic nervous system activity in middle-aged hypertensive individuals: a pilot study.

There has been increasing attention on the therapeutic effects of the forest environment. However, evidence-based research that clarifies the physiological effects of the forest environment on hypertensive individuals is lacking. This study provides scientific evidence suggesting that a brief forest walk affects autonomic nervous system activity in middle-aged hypertensive individuals. Twenty participants (58.0±10.6 years) were instructed to walk predetermined courses in forest and urban environments (as control). Course length (17-min walk), walking speed, and energy expenditure were equal be

[BNP in HIV-infected patients].

BACKGROUND AND PURPOSE: HIV-infected patients exhibit an increased rate of cardiac diseases, due to an elevated rate of cardiac risk factors and side effects of antiretroviral therapy. The aim of the present study was to analyze the impact of B-type natriuretic peptide (BNP) in this patient population. PATIENTS AND METHODS: The HIV-HEART (HIV infection and HEART disease) study is a prospective and multicenter cohort study. Outpatients with a known HIV infection were included over a 20-month recruiting period. BNP values were determined by a standardized laboratory test at study inclusion. RE

Extracorporeal circulation for great vessels surgery: a review of 131 cases.

A retrospective analysis of 131 cases of major vascular surgery, mainly aneurysms of the ascending and descending aorta, was undertaken to determine whether use of different bypass methods chosen according to location of the individual lesions resulted in improved results. For the 93 cases of ascending aortic aneurysm, the method for cardiac protection was improved by the use of continuous retrograde coronary sinus perfusion with cardioplegic blood. In sixteen cases with dissection involving the aortic arch, deep hypothermic circulatory arrest and continuous retrograde cerebral perfusion throu

Fetal heart rate and transcutaneous monitoring during experimentally induced hypoxia in the fetal dog.

A model of impaired oxygen delivery, using an acute surgical preparation in the fetal dog, is described. Fetal heart rate, transcutaneous pO2 and pCO2, and tissue pH were simultaneously recorded during hypoxic episodes produced by a series of successive occlusions of the maternal abdominal aorta. Corresponding values were also determined in the arterial blood of the fetus. The following pathophysiologic sequence of events was observed: 1) a latency period with no changes; 2) a drop in pO2 which stabilized later at a lower pressure; 3) late fetal heart rate deceleration, the pattern of which wa

Longitudinal assessment of bone mineral density changes after heart transplant: Implications for DXA screening frequency.

We evaluated the rate of bone mineral density (BMD) loss after heart transplant using dual-energy X-ray absorptiometry (DXA) to determine the optimal DXA surveillance frequency. This retrospective study involved 153 heart transplant recipients aged 50 years or greater, followed for up to 5,614 (median 1,111) days after transplant. Mixed-effects models with random intercepts were used to evaluate BMD changes over time. We found that males had higher BMD at each site before transplant (p < 0.05 for all). Patients with normal BMD pretransplant had a greater rate of BMD loss compared to patients w

Preconditioning with hyperbaric oxygen and calcium and potassium channel modulators in the rat heart.

The aim of this study was to establish the effect of combined therapy with hyperbaric oxygen (HBO2) therapy and verapamil, amlodipine or nicorandil on functional recovery and oxidative stress markers after ischemia in the isolated rat heart. The study included 48 rats (Wistar albino, male gender, eight weeks old, body weight 200±50g). All animals were exposed to HBO2 treatment over 14 days. Isolated heart rats were perfused by the Langendorff retrograde method at a constant coronary pressure of 70 cm H2O. After stabilization period the hearts were divided into the following groups: HBO2 group

The paradox of survival results after heart transplantation for cardiomyopathy caused by Trypanosoma cruzi. First Guidelines Group for Heart Transplantation of the Brazilian Society of Cardiology.

BACKGROUND: Donor supply limits heart transplantation (HT) and relative priority should be given to cases with greater chances of success. The objectives of this multicenter study were (1) to determine the survival rate after heart transplantation for patients with Chagas' heart disease (ChHD) in comparison with other causes; and (2) to identify the causes of death specifically due to reactivation of the Trypanosoma cruzi infection. METHODS: We studied 720 patients who had undergone orthotopic heart transplantation and were followed in 16 heart transplantation centers. The etiology was idiopa

Transcranial Direct Current Stimulation Decreases the Decline of Speed during Repeated Sprinting in Basketball Athletes.

The purpose of this study was to determine whether transcranial direct current stimulation (tDCS) can improve countermovement jump performance, fatigue index and alleviate the speed decline during repeated shuttle sprints in trained basketball players. Thirteen trained basketball players were divided into the tDCS trial and sham trial by the random crossover design. The tDCS trial was stimulated with 2-mA current in the M1 area in the middle of the top of the head for 20 min. For the sham trial, the current was turned off after 5 s, stopping the electrical stimulation. After warming up, the pl

Heart rate score, a measure related to chronotropic incompetence in pacemaker patients.

BACKGROUND: Heart rate score (HrSc) ≥70% in cardiac resynchronization therapy defibrillator and implantable cardioverter-defibrillator subjects predicts 5-year mortality risk. A high HrSc suggests few sensed cardiac cycles above the programmed lower rate. OBJECTIVE: To determine if HrSc is related to chronotropic incompetence (CI) in pacemaker (PM) subjects. METHODS: HrSc is the percentage of all atrial-paced and sensed events in the single tallest 10 beats/min histogram bin programmed to DDD 60/min. The prospective LIFE study of PM subjects examined multiple treadmill-based measures of CI.

Device closure of secundum atrial septal defects in children <15 kg: complication rates and indications for referral.

OBJECTIVES: This study sought to determine institutional complication rates in a previously underreported patient population and discuss referral indications. BACKGROUND: There has been a trend over the years for referral of younger and smaller patients for "elective" closure of atrial septal defects (ASD). In general, the risks associated with ASD device closure are believed and reported to be relatively low. Complication rates in this group of smaller patients are not well described in the literature for either percutaneous or surgical approaches. METHODS: Retrospective review of all patie
