The authors blindly reviewed the charts of 20 patients with normal-pressure hydrocephalus (a disease of unknown cause characterized radiologically as chronic communicating hydrocephalus and clinically by gait apraxia, dementia, and incontinence) who had undergone creation of a ventriculoperitoneal shunt. The initial clinical response to surgery was graded excellent, good, fair, or poor; 5-year follow-up was available in 55% of cases. The magnetic resonance (MR) images obtained in these patients were also blindly reviewed for the magnitude of cerebrospinal fluid (CSF) flow void (graded on the b

Regional cerebral blood flow and regional cerebral circulation time were measured in 13 demented patients with chronic hydrocephalus, mostly normal pressure hydrocephalus. The average hemispheric, frontal, and temporal cerebral blood flows were significantly reduced. The average regional cerebral circulation time values were significantly prolonged in the frontal, temporal, and thalamic regions, most markedly in the frontal white matter, where periventricular lucency was observed on computed tomography. Clinical improvement was obtained in all patients after operation. While postoperative regi

The synaptic membrane glycoprotein D2 was measured in cerebrospinal fluid (CSF) and plasma in 13 patients with normal pressure hydrocephalus (NPH), in 14 patients with primary degenerative dementia of Alzheimer type (PDD), and in 24 patients without evidence of organic nervous disease (controls). Mean CSF D2 concentration was significantly lower in NPH patients: 299 +/- 48 ng/ml (SEM) (P less than 0.001) than in PDD patients: 658 +/- 50 ng/ml (SEM) and in controls 641 +/- 45 ng/ml (SEM). Plasma D2 concentrations were higher in PDD patients compared with those found in controls. Determination o

This retrospective investigation of 55 patients, who had been treated with shunts, on account of normal pressure hydrocephalus (NPH) is an evaluation of the results in relation to the type of shunt and the complications developing from it. Sixty per cent of these patients showed an objective improvement after diversion of the CSF, 44 per cent had complications, which were fatal in 9 per cent. The patients who had a primary theco-perintoneal shunt showed a greater amount of improvement and fewer severe complications after the shunt. The successfully treated complications were not incompatible w

BACKGROUND: The usefulness of CERAD Neuropsychological Battery for describing the cognitive impairment in idiopathic normal pressure hydrocephalus (iNPH) is unknown.
OBJECTIVE: To compare the cognitive profile of patients with iNPH to patients with mild Alzheimer's disease (AD) and age-matched cognitively healthy individuals by using the CERAD-NB.
METHODS: We studied CERAD-NB subtest results, including the Mini-Mental State Examination (MMSE), between 199 patients with probable iNPH, 236 patients with mild AD, and 309 people with normal cognition, using age, education, and gender adjusted mu

Evidence suggests that normal pressure hydrocephalus (NPH) is underdiagnosed in day to day radiologic practice, and differentiating NPH from cerebral atrophy due to other neurodegenerative diseases and normal aging remains a challenge. To better characterize NPH, we test the hypothesis that a prediction model based on automated MRI brain tissue segmentation can help differentiate shunt-responsive NPH patients from cerebral atrophy due to Alzheimer disease (AD) and normal aging. Brain segmentation into gray and white matter (GM, WM), and intracranial cerebrospinal fluid was derived from pre-shu

BACKGROUND/OBJECTIVES: Ventriculoperitoneal shunting is a validated procedure for the treatment of idiopathic normal-pressure hydrocephalus. To select shunt-responsive patients, infusion and tap tests can be used. Only gait is evaluated after the procedure to establish a potential improvement. In this study, we present our Hydro-Real-Time Neuropsychological Testing protocol to assess the feasibility of performing an ultra-fast assessment of patients during the infusion and tap test.
METHODS: We tested 57 patients during the infusion and tap test to obtain real-time feedback on their cognitive

OBJECTIVE The presence of disproportionately enlarged subarachnoid space hydrocephalus (DESH) on brain imaging is a recognized finding of idiopathic normal pressure hydrocephalus (iNPH), but the features of DESH can vary across patients. The aim of this study was to evaluate the utility of MRI-based DESH scoring for predicting prognosis after surgery. METHODS In this single-center, retrospective cohort study, the DESH score was determined by consensus between a group of neurosurgeons, neurologists, and a neuroradiologist based on the preoperative MRI findings of the patients with suspected iNP

Lumber subarachnoid pressure was monitored via intrathecally introduced catheter in 9 patients, who showed ventricular dilatation on brain CT and were suspected of normal pressure hydrocephalus. During the pressure measurement, 500 mg of acetazolamide was rapidly injected intravenously and changes of CSF pressure were recorded. The results of the study were compared to clinical signs, brain CT findings and results of continuous pressure monitoring. The types of response of CSF pressure induced by acetazolamide administration were classified into 2 groups. In 5 cases (Group A), more than 50 per

BACKGROUND: Tap test improves symptoms of idiopathic normal pressure hydrocephalus (iNPH); hence, it is widely used as a diagnostic procedure. However, it has a low sensitivity and there is no consensus on the parameters that should be used nor the volume to be extracted. We propose draining cerebrospinal fluid (CSF) during tap test until a closing pressure of 0 cm H2O is reached as a standard practice. We use this method with all our patients at our clinic.
METHODS: This is a descriptive cross-sectional study where all patients with presumptive diagnosis of iNPH from January 2014 to December

BACKGROUND: Maintenance of pressure gradient within the intracranial chamber, shunt hardware, and the abdominal cavity plays a significant role in the optimal functioning of the ventriculo peritoneal shunt. We report a rare and a complex scenario in a patient with normal pressure hydrocephalus (NPH) who had recurrent and refractory ventricular peritoneal shunt dysfunction. Following a meticulous analysis, this was attributed to a very rare, and, first to be documented in the literature, a combination of an evolved very low pressure hydrocephalus (VLPH) system and asymptomatic raised intra-abdo

Does the opening pressure of hydrostatic shunts influence the clinical outcome for patients suffering from idiopathic normal-pressure hydrocephalus (NPH)? Between September 1997 and January 2003, 123 patients with idiopathic NPH were surgically treated by implanting a hydrostatic shunt at the Departments of Neurosurgery of the Unfallkrankenhaus Berlin and the University Homburg/Saar. As part of a prospective randomized study, all patients were examined preoperatively, postoperatively, and 1 year after the intervention. Forty-three percent of the patients showed a very good outcome, 25% good ou

This review aims to summarize the frequency, pathophysiological features, and therapeutic effectiveness of shunt surgery in cases of coexisting progressive supranuclear palsy (PSP) and idiopathic normal pressure hydrocephalus (iNPH). In this review, we showed that the coexistence of these two disorders is not rare, that they may be related through the glymphatic system, and that in cases clinically suspected of having both conditions, improvement after shunt surgery may persist for one year or more, whereas in six pathologically confirmed cases, the duration of improvement was one year or less

INTRODUCTION: Endoscopic third ventriculostomy (ETV) has been proposed for selected patients with idiopathic normal pressure hydrocephalus (iNPH), but predictors of sustained benefit are uncertain. Phase-contrast MRI (PC-MRI) may quantify cerebrospinal fluid (CSF) pulsatility and support patient selection.
RESEARCH QUESTION: Is sustained clinical improvement after primary ETV associated with preoperative aqueductal CSF flow parameters measured by PC-MRI?
MATERIAL AND METHODS: In this prospective single-centre observational study, patients with iNPH underwent primary ETV after standardized di

BACKGROUND AND OBJECTIVES: Idiopathic normal pressure hydrocephalus is a condition with gait impairment, incontinence, and cognitive decline, treated with shunt surgery. The Gothenburg scale (GBS) provides comprehensive assessment of symptoms and outcomes. The cerebrospinal fluid tap test (CSF-TT) is used as a prognostic tool, but its predictive value remains inconsistent. This study aimed to evaluate the clinical utility of CSF-TT and its association with postoperative outcomes measured using GBS, a rarely applied multidomain outcome measure in CSF-TT evaluation.
METHODS: A prospective, popu

OBJECTIVE: The objective of this study was to determine whether cerebral atrophy is associated with ventriculoperitoneal (VP) shunt overdrainage in patients with normal pressure hydrocephalus (NPH).
METHODS: A 3-year retrospective chart review included 100 patients diagnosed with NPH who underwent VP shunt implantation for treatment. Radiologist CT or MRI reports were analyzed for a description of brain atrophy before shunting and overdrainage complications after shunting. Patients were categorized into four groups based on the presence or absence of cerebral atrophy and overdrainage complica

BACKGROUND AND PURPOSE: Idiopathic normal pressure hydrocephalus (iNPH) symptoms can be alleviated through shunt placement. However, the identification of suitable patients is often challenging despite several existing biomarkers. DTI is commonly used to assess white matter integrity, with DTI analysis along the perivascular space being a recently described though less extensively evaluated metric. It has been proposed to monitor glial-lymphatic (glymphatic) activity in the brain, the impairment of which may play a role in several neurologic diseases. The goal of this study is to evaluate the

BACKGROUND: Idiopathic Normal Pressure Hydrocephalus (iNPH) is a leading cause of reversible gait disturbance in older adults, yet diagnosis and treatment selection remain limited by low-sensitivity clinical assessments. Conventional tests such as the 10-metre walk and Timed Up-and-Go capture overall performance but overlook the multidimensional, joint-level alterations that characterise iNPH gait. This study aimed to deliver the first comprehensive, three-dimensional (3D) biomechanical characterisation of iNPH gait to inform more objective diagnostic frameworks.
METHODS: Twenty-three partici
