Epiploic Appendagitis in an Incarcerated Inguinal Hernia: A Case Report and Literature Review.

Epiploic appendagitis (EA) is a rare and frequently overlooked cause of abdominal pain. EA within the sigmoid colon in a right-sided inguinal hernia is a rare entity with only a few cases reported. In this article, we present a case of an 80-year-old male with a palpable mass within an incarcerated right inguinal hernia. The patient underwent urgent surgical intervention due to incarceration and the operative findings were of a large indirect inguinoscrotal hernia containing sigmoid colon, with an inflamed epiploic appendage. The colon was reduced into the abdominal cavity and standard tension…

A case report of incarcerated inguinal hernia: Amyand's hernia with adenocarcinoma tumor.

INTRODUCTION: Inguinal hernia is one of the most common surgical problems, often posing technical challenges even to expert surgeons. Amyand's hernia (AH) is an inguinal hernia whose sac contains the vermiform appendix. Primary appendiceal neoplasms are rare. We report the case of an appendiceal adenocarcinoma within an Amyand's hernia, presenting as an incarcerated right inguinal hernia. PRESENTATION OF CASE: A 87-year-old male presented in the emergency department due to a persistent right inguinal pain. Clinical examination revealed a tender right groin mass. Under the diagnosis of an righ

Participation in a Yoga Study Decreases Stress and Depression Scores for Incarcerated Women.

Incarcerated individuals exhibit a high incidence of stress-related disorders, including addiction and posttraumatic stress disorder (PTSD), as well as the added stress of captivity. Access to stress-reduction tools is limited for these individuals. One possible approach may be regular structured yoga classes. Using two approaches, we tested the effectiveness of a brief, intensive yoga intervention in a population of incarcerated women in a county jail. The first approach was an examination of archival data collected as part of a program analysis. Individuals showed considerable reduction in s

A Legal Right to Clozapine Therapy for Incarcerated Individuals With Treatment-Resistant Schizophrenia.

People with serious mental illnesses increasingly are being treated in jails and prisons, and during incarceration are afforded a constitutional right to medical care. This right pertains to both general medical and mental illnesses and both acute and chronic conditions. However, incarcerated patients with treatment-resistant schizophrenia (TRS) often are not offered clozapine, the only medication for this debilitating illness approved by the U.S. Food and Drug Administration. In this column, the authors argue that incarcerated individuals with TRS have a statutory and constitutional right to …

Breastfeeding in Incarcerated Settings in the United States: A National Survey of Frequency and Policies.

Objectives: To assess the existence of prison and jail policies and practices that allow incarcerated women to breastfeed while in custody, and prevalence of women in custody who pumped human milk for their infants. Methods: We surveyed 22 state prison systems and 6 county jails from 2016 to 2017 about policies related to breastfeeding and other programs for pregnant and parenting women in custody. In addition, 11 prisons and 5 jails reported 6 months of monthly, prospective data on the number of women pumping human milk, as well as information on placement of infants born to women in custody.

Treatment preference for opioid use disorder among people who are incarcerated.

INTRODUCTION: The devastating overdose crisis remains a leading cause of death in the United States, especially among individuals involved in the criminal legal system. Currently, three classes (opioid agonist, partial agonist-antagonist, and antagonist) of FDA-approved medications for opioid use disorder (MOUD) exist, yet few correctional settings offer any medication treatment for people who are incarcerated. Facilities that do often provide only one medication. METHODS: We conducted 40 semi-structured qualitative interviews with individuals receiving MOUD incarcerated at the Rhode Island D

Addressing Hepatitis C in the American Incarcerated Population: Strategies for Nationwide Elimination.

PURPOSE OF REVIEW: The prevalence of Hepatitis C virus (HCV) in the US incarcerated population is disproportionately high, and when inmates with infection are released back into the general population, they play a substantial role in the spread of disease. This review provides support for targeting the jail/prison population to eliminate HCV in the general population. It will also summarize various screening/treatment models to curtail the burden of disease behind and beyond bars. RECENT FINDINGS: Transitioning from risk-based testing to opt-out testing in prisons/jails would be cost-effectiv

Effects of Astrobiology Lectures on Knowledge and Attitudes about Science in Incarcerated Populations.

The incarcerated population has little or no access to science education programs, STEM resources, or scientists. We explored the effects of a low-cost, potentially high-impact informal science education program that enabled NASA scientists to provide astrobiology lectures to adults inside 16 correctional institutions in three states. Post- versus pre-lecture surveys suggest that presentations significantly increased science content knowledge, positively shifted attitudes about science and scientists, increased a sense of science self-identity, and enhanced behavioral intentions about communic

Correlates of Posttraumatic Stress Symptoms among Formerly Incarcerated, Homeless Women.

Posttraumatic stress symptoms are a pressing issue among women experiencing incarceration and homelessness. Baseline data were collected among formerly incarcerated homeless women (N = 130) who were on average 38.9 (SD = 11.36, range 19-64) years of age and recruited into a pilot randomized control trial (RCT) intervention program. A logistic regression was used to assess correlates of PTSD symptoms. The majority of the sample self-reported witnessing violence (85%) and had moderate PTSD symptoms (M = 1.61, SD = 1.62, range: 0-4). No past month drug use (p = 0.006), higher anger scores (p = 0.…

Mechanical dilation to remove incarcerated laminaria during a second trimester abortion.

Cervical preparation with laminaria reduces complications with 2nd trimester dilation and evacuation. During a surgical abortion at 22 weeks, we could not remove laminaria manually or with ring forceps due to laminaria "dumbbelling" [1]. Without pushing laminaria into the uterus, we mechanically dilated the cervix and removed the incarcerated laminaria.

Incarcerating Pregnant and Parenting Women, the New Witch Hunt: A Policy Analysis.

The mass incarceration of pregnant and parenting women is a serious problem in U.S. A policy focus is needed that takes a public health approach. This paper outlines the problems with health care delivery in correctional facilities. It is argued that correctional facilities are not the place for pregnant and parenting women. Alternative policies and programs are recommended that align with a public health approach and directly targets the pathways for imprisonment.

Incarcerated Spigelian hernia: A rare cause of abdominal wall tender mass.

In this study, we aimed to present that incarcerated Spigelian hernia is an important cause of acute abdomen although it is rare and accounts for 1-2% of all abdominal wall hernias. Spigelian hernia arises from a defect in the aponeurosis of the transversus abdominis muscle, also known as the Spiegel fascia. This case analysis aims to present a Spigelian hernia case in which the sigmoid colon is incarcerated. The patient was referred to our emergency surgery clinic complaining of severe abdominal pain and a palpable mass in the left quadrant of the abdomen, presenting tenderness on the front a

Chronic nodules of sigmoid perforation caused by incarcerated intrauterine contraception device.

RATIONALE: It is extremely rare for an intrauterine contraception device (IUD) to cause uterine perforation and Sigmoid perforation for a long time without being detected. PATIENT CONCERNS: We present a case of a patient who has suffered from abdominal pain after 4 years of placement of an IUD, and found that the IUD was incarcerated by ultrasound. DIAGNOSES: Laparoscopic and hysteroscopic examination revealed that the incarcerated IUD caused uterine perforation and sigmoid perforation for a long time. One end of the intrauterine device completely penetrated the anterior wall muscle layer of

Treatment of an incarcerated inguinal hernia associated with abdominal tuberculosis in an adult patient.

The incidence of tuberculosis is increasing worldwide, especially in developing countries. The prevalence of abdominal tuberculosis has been found to be as high as 12% in people with extrapulmonary tuberculosis. Peritoneal thickening and intestinal adhesions can occur in patients with abdominal tuberculosis. Inguinal hernias are extremely rare in people with abdominal tuberculosis; only 11 cases have been reported in the English-language literature, half of which involved pediatric patients. No definitive guideline on the management of such cases is available. In this report, we describe the u
