Successfully managed aluminum phosphide poisoning: A case report.

INTRODUCTION: and Importance: Aluminum phosphide (ALP) is a commonly available pesticide in agricultural countries like Nepal. Upon ingestion, this releases highly toxic phosphine gas in the gastrointestinal tract when it comes in contact with humidity. This leads to refractory shock, metabolic acidosis, cardiac arrhythmia, renal failure, and hepato-biliary impairment. CASE PRESENTATION: We present a successfully managed case of a 17-year-old girl who ingested 6 g (2 tablets) of ALP tablets with suicidal intent. Although the mortality has been reported as 70-100% with mere ingestion of 150-50

Emergent catheter ablation for atrial fibrillation in a patient with acute decompensated heart failure on a mechanical haemodynamic support: a case report.

BACKGROUND: Atrial fibrillation (AF) is associated with the exacerbation of heart failure (HF). Although AF ablation has become an established treatment for patients with HF, it is usually an elective procedure. Here, we present a case of acute decompensated heart failure (ADHF) exacerbated by refractory AF, which was successfully treated with emergent AF ablation. CASE SUMMARY: A 53-year-old, obese man with a history of myocardial infarction presented to our hospital. Heart function deteriorated with an ejection fraction of 9.8%, and he was repeatedly hospitalized due to worsening HF. This t

Mechanical Circulatory Support as a Bridge to Definitive Treatment in Post-Infarction Ventricular Septal Rupture.

Ventricular septal rupture (VSR) represents a rare complication of acute myocardial infarction, often presenting with cardiogenic shock and associated with high in-hospital mortality despite prompt intervention. Although immediate surgery is recommended for patients who cannot be effectively stabilized, the ideal timing of intervention remains controversial. Mechanical circulatory support (MCS) may allow hemodynamic stabilization and delay definitive treatment even in critical patients. However, the interactions between MCS and VSR pathophysiology as well as potentially related adverse effects

Hemodynamic adaptation of heart failure to percutaneous venoarterial extracorporeal circulatory supports.

Extracorporeal life support (ECLS) is a treatment modality that provides prolonged blood circulation, gas exchange and can partially support or fully substitute functions of heart and lungs in patients with severe but potentially reversible cardiopulmonary failure refractory to conventional therapy. Due to high-volume bypass, the extracorporeal flow is interacting with native cardiac output. The pathophysiology of circulation and ECLS support reveals significant effects on arterial pressure waveforms, cardiac hemodynamics, and myocardial perfusion. Moreover, it is still subject of research, wh

Novel technique of performing multivessel PCI through an Impella sheath.

A 69-year-old woman with diabetes was found to have multi-vessel coronary artery disease and underwent 5-vessel coronary artery bypass grafting. Patient had persistent cardiogenic shock postoperatively despite intra-aortic balloon pump and escalating pressor requirements. Electrocardiogram showed new ischemic changes and the patient was urgently taken to the catheterization lab for coronary angiography and placement of an Impella CP for higher degree of hemodynamic support via the left femoral artery. Due to limitations in vascular access the Impella CP sheath was utilized for vascular access

Coronary artery vasospasm and cardiogenic shock as the initial presentation for eosinophilic granulomatosis with polyangiitis.

A 68-year-old woman presented to our hospital with unstable angina and was admitted for further evaluation. While hospitalized, she developed persistent angina with hypotension along with ST-segment elevation in leads V1-V2 along with lead aVR elevation on 12-lead electrocardiogram. Coronary angiography revealed diffuse multi-vessel coronary vasospasm most notably in the left anterior descending artery (LAD). Due to incomplete resolution of vasospasm with intracoronary verapamil and nitroglycerin, along with hemodynamic compromise requiring an intra-aortic balloon pump, percutaneous coronary i

Bileaflet versus posterior-leaflet-only preservation in mitral valve replacement.

In the present study of mitral valve replacement, we investigated whether complete preservation of both leaflets (that is, the subvalvular apparatus) is superior to preservation of the posterior leaflet alone. Seventy patients who underwent mitral valve replacement in our clinic were divided into 2 groups: MVR-B (n=16), in whom both leaflets were preserved, and MVR-P (n=54), in whom only the posterior leaflet was preserved. The preoperative and postoperative clinical and echocardiographic findings were evaluated retrospectively. No signs of left ventricular outflow tract obstruction were obser

Hemi-fontan or bidirectional cavopulmonary shunt for right ventricular failure after mitral valve replacement and acute ascending aortic dissection: report of two cases.

Right ventricular failure due to right coronary artery disease, right ventricular hypertrophy, stunning, abnormal septal motion, myocardial infarction, or non-homogeneous distribution of cardioplegia is an uncommon but serious complication of open heart surgery. We report a patient with severe right ventricular hypertrophy secondary to severe mitral valve stenosis and another patient with detachment of the right coronary artery due to the dissection of the ascending aorta. The patients developed right ventricular failure, which persisted after surgery and rendered weaning from cardiopulmonary

Outcome of patients after coronary artery bypass grafting in cardiogenic shock.

PURPOSE: Coronary artery bypass graft (CABG) surgery in patients with cardiogenic shock (CS) is a rare and very high-risk procedure carrying high mortality. In this study we reviewed hospital outcomes and 1-year survivals in these high-risk patients. MATERIALS AND METHODS: During a 4-year period (May 2001 to April 2005), 412 patients were operated on for CABG by a single surgeon, and 13 (3.1%) of them were in CS at the time of procedure. RESULTS: The 30-day mortality of patients who underwent CABG during CS was 16%, and mean age was 57+/-10 years. A total of 77% were male, 77% were hypertens

Myocardial stunning secondary to aneurysmal subarachnoid hemorrhage.

BACKGROUND: Although subarachnoid hemorrhage (SAH) is often associated with electrocardiographic abnormalities, profound effects on cardiac performance are rare. CASE DESCRIPTION: A 57-year-old woman who developed loss of consciousness, respiratory distress, severe hypotension, and left ventricular hypokinesis with minimal coronary artery disease is described. Despite normal appearance of the coronary arteries on angiography, left ventricle function was so severely depressed that she required intra-aortic balloon pump support for 24 hours. Mental status changes prompted a head computed tomogr

True emergency coronary artery bypass surgery.

Previous reports of emergency coronary artery bypass grafting often included cases that were not true surgical emergencies, thereby creating inappropriately favorable results. To accurately investigate this important subgroup of patients, we analyzed our recent experience with truly emergent coronary artery bypass grafting. From January 1984 to January 1989, 117 patients underwent true emergency bypass grafting for acute refractory coronary artery ischemia. Clinical deterioration was associated with failure of percutaneous angioplasty in 37 patients and instability during diagnostic catheteriz

Case Report: De Winter electrocardiographic pattern associated with acute total left main coronary artery occlusion.

BACKGROUND: The de Winter electrocardiograph (ECG) pattern is a well-established ST-elevation myocardial infarction (STEMI) equivalent, most commonly associated with acute occlusion of the proximal left anterior descending (LAD) artery. CASE PRESENTATION: A 54-year-old man presented to the emergency department with two hours of chest pain, sweating, and nausea. The 12-lead ECG showed upsloping J-point depression in leads V2-V6 and lead I, with tall, symmetrical T waves in leads V2-V5, ST-segment elevation in lead aVR, and ST-segment depression in the inferior leads, consistent with the de Win

Novel Use of Helium Insufflation to Facilitate Epicardial Access in Ventricular Tachycardia Ablation.

INTRODUCTION: Epicardial access for ventricular tachycardia (VT) ablation is challenging in patients with prior cardiac surgery or dense pericardial adhesions. Although carbon dioxide (CO₂) insufflation has been used to improve visualization, concerns remain regarding its hemodynamic and electrophysiological effects. METHODS AND RESULTS: We report the first clinical use of helium to facilitate epicardial access in three patients undergoing VT ablation. All patients had ischemic cardiomyopathy, prior coronary artery bypass grafting, and failed or incomplete endocardial ablation. Helium was del

Thrombus-In-Transit Across Patent Foramen Ovale Causing Dynamic Embolism to the Left Main Coronary and Pulmonary Arteries.

BACKGROUND: Paradoxical embolism via a patent foramen ovale (PFO) is a rare cause of acute myocardial infarction; its simultaneous or sequential occurrence with pulmonary embolism is even rarer. CASE SUMMARY: A 60-year-old woman with no prior cardiac history presented with anterior ST-segment elevation myocardial infarction and cardiogenic shock. Preprocedural transthoracic echocardiography showed a large mobile thrombus straddling the interatrial septum through a PFO. Coronary angiography revealed complete left main coronary artery occlusion; thrombus aspiration restored TIMI flow grade 3. M

Contrast-Induced Type II Kounis Syndrome Complicated by a Cardiac Arrest Successfully Managed With Extracorporeal Membrane Oxygenation (ECMO): A Case Report.

Kounis syndrome (KS) is a rare allergic-mediated acute coronary syndrome (ACS) characterized by coronary vasospasm, plaque rupture, or thrombosis. It is typically triggered by allergens such as medications, contrast agents, or environmental factors. We report the case of a 58-year-old man with hypertension who developed Type II KS after iodinated contrast administration for coronary evaluation. Shortly after contrast exposure, he experienced sudden hypotension, chest pain, and loss of consciousness, followed by cardiac arrest despite prompt resuscitation. Veno-arterial extracorporeal membrane

Venoarterial extracorporeal membrane oxygenation with and without left ventricular unloading in patients with cardiac arrest: A nationwide analysis from the United States.

BACKGROUND: Extracorporeal cardiopulmonary resuscitation (ECPR) using veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is a promising strategy for refractory cardiac arrest. However, the role of left ventricular (LV) unloading during ECPR remains uncertain. METHODS: We queried the United States National Inpatient Sample (2016-2021) to identify adult patients with out-of-hospital (OHCA) and in-hospital cardiac arrest (IHCA) who received ECPR. Patients were stratified based on the use of either VA-ECMO only or VA-ECMO with intra-aortic balloon pump (IABP) or VA-ECMO with Impella (ECP

Recommendations for the management of ST-elevation myocardial infarction after reperfusion.

The management of ST-segment elevation myocardial infarction (STEMI) after reperfusion requires critical decision-making in patients with multivessel disease (MVD), acute heart failure, and left ventricular (LV) thrombus. Complete revascularisation of non-culprit lesions ≥70% is recommended either during the index procedure or within the first 19 days, particularly in haemodynamically stable patients. Coronary artery bypass grafting is indicated for high-risk anatomy or complex lesions following successful percutaneous coronary intervention, where a hybrid approach may combine percutaneous int
