Studies on subtype "d" biliary atresia.

Nine cases of specific type (subtype d) biliary atresia were experienced at the Tohoku University Hospital and were classified into three groups on the basis of their clinical and pathological findings. Group 1 cases were defined as cases showing obstruction on the side of the hepatic duct and cystic dilatation, communicating with the gallbladder, in the common bile duct. Group 2 cases were those showing dilatation similar to Group 1, but measuring less than 20 mm, the dilated part being covered with thick connective tissue. Group 3 cases were those showing isolated dilatation in the common he

Afferent Loop Syndrome Due to Volvulus of the Afferent Limb.

A 78-year-old woman with a history of stage IIB gastric adenocarcinoma with previous Billroth II subtotal gastrectomy was admitted with pancreatitis, with subsequent development of fevers and acute jaundice. Transabdominal ultrasound demonstrated bile duct obstruction. An endoscopic retrograde cholangiopancreatography was attempted, but the lumen of the afferent limb appeared distorted without an obstructing lesion. A computed tomography scan demonstrated volvulus of the afferent limb near the gastrojejunal anastomosis, with afferent limb dilation and significant biliary dilation.

Portal cavernoma cholangiopathy - history, definition and nomenclature.

Biliary changes secondary to portal hypertension, especially in portal cavernoma secondary to extrahepatic portal vein obstruction have long been described in literature under different names by various authors. Most of the times these changes are asymptomatic and discovered on imaging, but can occasionally cause obstructive jaundice. There is no consensus on the appropriate nomenclature and definition of this entity. This article reviews the history of portal hypertensive biliopathy and the Indian Association for the Study of Liver Working Party consensus definition and nomenclature for it.

Embryonal rhabdomyosarcoma of the ampulla of Vater with long-term survival following pancreaticoduodenectomy.

Rhabdomyosarcoma of the biliary tree is a rare cause of biliary tract obstruction in childhood. A 3-year-old child is reported here after presenting with obstructive jaundice secondary to an embryonal rhabdomyosarcoma of the ampulla of Vater. He underwent pancreaticoduodenectomy followed by adjuvant chemotherapy and irradiation. He is now well and free of disease 5 years following treatment. This child appears to be the first long-term survivor who has required pancreaticoduodenal resection for this lesion.

Haemophilus influenza sepsis and shock secondary to biliary infection in an adult.

A 46-year-old woman was seen with biliary obstruction secondary to a pancreatic tumor. After undergoing a percutaneous liver biopsy, she became septic and went into shock. Haemophilus influenzae type be bacteremia and biliary infection were verified. With treatment, the patient recovered from the infection. Biliary infection by H influenzae is rare, and fulminant infections in adults are even more uncommon. Hazards of invasive diagnostic procedures in patients with obstructive jaundice are discussed.

Percutaneous transhepatic cholangiography with a thin needle. Experience with 25 cases.

Percutaneous transhepatic cholangiography with a thin needle was successfully employed in 23 of 25 patients with jaundice. A 100% success rate was seen with dilated bile ducts. Not a single complication was observed in our series. With our limited series, it can be assumed that proximal obstruction is more likely to mean neoplasm of either the gallbladder or bile duct, whereas distal obstruction with visualization of dilated cystic duct means either neoplasm of the head of pancreas or metastatic carcinoma.

Aggressive behavior of anaplastic undifferentiated carcinoma arising from the hilar bile duct.

BACKGROUND: Undifferentiated carcinoma of the biliary tree is extremely rare, and biliary undifferentiated carcinoma mostly originates from the gallbladder. We herein present a case of anaplastic undifferentiated carcinoma of the hilar bile duct and reviewed the literature. CASE PRESENTATION: The patient was an 81-year-old male with obstructive jaundice. Contrast-enhanced computed tomography (CT) showed a protruded tumor located at the hepatic hilum. Obstructive jaundice was relieved by endoscopic drainage. Endoscopic biopsy revealed carcinoma without glandular differentiation, and the patien

Risk factors for conversion during laparoscopic cholecystectomy - experiences from a general teaching hospital.

BACKGROUND AND AIMS: Laparoscopic cholecystectomy (LC) is the gold standard for treating symptomatic cholelithiasis. Conversion, however, is sometimes necessary. The aim of this study was to determine predictive factors of conversion in patients undergoing LC for various indications in elective and acute settings in a general teaching hospital. MATERIAL AND METHODS: A retrospective analysis was performed on 972 consecutive patients who underwent a laparoscopic cholecystectomy in Máxima Medical Centre in Veldhoven, the Netherlands, from January 2000 till January 2006. Recorded data were sex, a

Endoscopic ultrasound-guided choledochoduodenostomy in patients with failed endoscopic retrograde cholangiopancreatography.

Endoscopic ultrasonography (EUS)-guided biliary drainage was performed for treatment of patients who have obstructive jaundice in cases of failed endoscopic retrograde cholangiopancreatography (ERCP). In the present study, we introduced the feasibility and outcome of EUS-guided choledochoduodenostomy in four patients who failed in ERCP. We performed the procedure in 2 papilla of Vater, including one resectable case, and 2 cases of cancer of the head of pancreas. Using a curved linear array echoendoscope, a 19 G needle or a needle knife was punctured transduodenally into the bile duct under EUS

Mucin-producing adenoma associated with pancreas divisum and hepatic hilar carcinoma: an autopsy case.

We present the autopsy case of an 82-year-old Japanese woman with a mucin-producing adenoma accompanied by pancreas divisum and a hepatic hilar carcinoma. She had suffered from a cholangiocellular carcinoma at the hepatic hilus for 2 months, which was treated with radiation and chemotherapy. She did not complain of any abdominal pain. Obstructive jaundice deteriorated despite percutaneous transhepatic bile duct drainage, and she died of hepatic insufficiency. At autopsy, a hepatic tumor was confirmed to have caused severe obstructive jaundice. Histological examinations showed moderately to poo

Endoscopic pancreatic stenting in pancreatic cancer.

Most pancreatic carcinomas are unresectable at the time of diagnosis; therefore, palliative treatment is very often the main concern of clinicians in this setting. The main symptoms resulting in the need for palliation in pancreatic cancer are obstructive jaundice, duodenal obstruction and pain. Therapeutic endoscopy plays a major role in the palliation of obstructive jaundice by stent placement into the biliary ducts. Initial experience has also been gained recently with endoscopic placement of expandable metallic stents to treat gastric outlet obstruction. Much less is known about the possib

Gastric outlet obstruction after palliative surgery for cancer of head of pancreas.

Gastroenterostomy as part of the palliation of unresectable cancer of the head of the pancreas, in the absence of gastric outlet obstruction at the time of surgery, has been both rejected, and, more recently, advocated for all such patients. A study of 105 cases yielded four factors that correlated with the subsequent occurrence of gastric outlet obstruction: age, 60 years or younger; hemoglobin level, 11.5 g/dL or less; absence of liver metastases; and survival, three months or longer. A fifth factor, absence of clinical jaundice, may be an indicator of a poor prognosis with a small risk of o

Mirizzi syndrome secondary to an adenoma of the cystic duct.

Bile duct adenomas are uncommon lesions that can cause obstructive jaundice. We report the unusual case of a 54-year-old man who developed Mirizzi syndrome secondary to a bile duct papillary adenoma located in the cystic duct remnant. A case report is presented, together with a review of extrahepatic bile duct adenomas published in the English-language literature, with special attention directed toward the clinical manifestations, locations, and prognosis of these tumors. Bile duct adenomas are very rare tumors. Although cholangiography can detect many of these lesions, few cases were correctl

A Complex Diagnosis of Ampullary Adenocarcinoma Presenting As Decompensated Cirrhosis.

Neoplasms arising from the ampulla of Vater are exceedingly rare, and there is a paucity of literature regarding their diagnosis and management. Ampullary cancer typically presents with jaundice and signs of biliary obstruction. We present a case of ampullary adenocarcinoma with concomitant choledocholithiasis that proved complex and diagnostically challenging.

Biliary phytobezoar: a medical curiosity.

This report describes a case of cholestatic jaundice caused by concretions of vegetable material obstructing the extrahepatic bile ducts. We have named this a biliary phytobezoar, by analogy with its gastric counterpart. The anomaly underlying this curiosity was a surgical cholecystogastrostomy performed 15 yr earlier.

Ampullary Adenoma and Hepatic Inflammatory Pseudotumor Mimicking Malignancies on 18 F-FDG PET/CT.

We report a rare case of ampullary tubulovillous adenoma with secondary jaundice and hepatic inflammatory pseudotumor mimicking malignancies on 18 F-FDG PET/CT. A 68-year-old woman presented with fever, jaundice and abdominal pain. Serum tumor markers (CEA, AFP, CA19-9, CA125, and CA242) were normal, except for a mild elevation CA72-4. 18 F-FDG PET/CT revealed biliary obstruction, hypermetabolic foci in the descending duodenum, and a hepatic mass, initially suggesting ampullary adenocarcinoma with hepatic metastasis. Post-surgery pathology confirmed ampullary tubulovillous adenoma and hepatic

Immunoglobulin G4-related cholangitis: a variant of IgG4-related systemic disease.

IgG4-related sclerosing cholangitis as part of IgG4 systemic-related diseases is commonly associated with autoimmune pancreatitis. Major clinical manifestations of IgG4-related sclerosing diseases are apparent in the organs in which tissue fibrosis with obstructive phlebitis is pathologically induced. IgG4-related sclerosing cholangitis is included within the heterogeneous group of 'sclerosing cholangitis'. Sclerosing cholangitis may be associated with choledocholithiasis, infection or biliary malignancies. Sclerosing cholangitis of unknown etiology is called primary sclerosing cholangitis (PS
