Prediction Model for Failed Vacuum Assisted Delivery: A Retrospective Cohort Study.

Background/Objectives: We aimed to determine risk factors and to design a clinically based predictive model for a failed vacuum assisted delivery (VAD). Methods: We conducted a retrospective cohort study in a single tertiary university-affiliated medical center between 2011 and 2023. The study population consisted of singleton pregnancies with a VAD trial. The study group comprised cases of a failed VAD, defined as the occurrence of any of the following: (1) more than two vacuum cup detachments; (2) extraction duration exceeding 20 min; or (3) abandonment of the vacuum attempt by the operating

Vaginal breech delivery in all-fours position-Hands off instead of intervention: A prospective observational study.

INTRODUCTION: Vaginal delivery in cases of breech presentation is considered potentially stressful for the newborn. The maternal upright position may represent a more physiological approach to facilitating birth. We compare the safety and efficacy of two maternal positions in vaginal breech delivery. MATERIAL AND METHODS: A prospective, single-center, observational cohort study from October 2006 to January 2021 in a high-level obstetric center, in Salzburg, Austria. Vaginal breech deliveries in maternal all-fours position (n = 140) were compared with those in assisted supine position (n = 92)

Protracted postpartum urinary retention-a long-term problem or a transient condition?

INTRODUCTION AND HYPOTHESIS: Protracted postpartum urinary retention (P-PUR) is a rare puerperal complication of overt urinary retention that proceeds beyond the 3rd postpartum day. Long-term consequences of P-PUR are poorly reported. The objective of the study was to compare the long-term outcome of patients with P-PUR with a matched control group, using a validated pelvic floor distress questionnaire. METHODS: All medical files of women diagnosed with P-PUR between 2005 and 2016 were reviewed. The control group was comprised of women who had a consecutive birth, matched in a 1:2 ratio, by m

Pregnancy Outcomes of Healthy Parturients Delivered With or Without Episiotomy.

BACKGROUND: The use of episiotomy, a previously popular obstetric procedure, has been declining for the last 25 years, falling from 65% of deliveries in 1979 to 18% in 2003. Some complications have been reported in terms of maternal side effects; however, many episiotomies are still performed in Rajavithi Hospital. OBJECTIVE: To compare maternal and neonatal outcomes and complications in the 48-hour period after vaginal delivery in healthy parturients with and without episiotomy. MATERIAL AND METHOD: A historical cohort study was conducted by reviewing the medical records of 920 healthy part

Risk factors for obstetric anal sphincter injury: a prospective study.

BACKGROUND: Anal incontinence is an embarrassing condition that is largely underreported. Obstetric anal sphincter injuries are the major etiological factor. Recognition of risk factors may minimize the development of sphincter injuries. The objective of this study was to identify risk factors for sphincter injuries and measure dimensions of mediolateral episiotomies. METHODS: Women expecting their first vaginal delivery were invited to participate, and an experienced research fellow performed a perineal and rectal examination and classified tears according to the new international classifica

[Epidemiology of anal lesions (fissure and thrombosed external hemorroid) during pregnancy and post-partum].

Thrombosed external hemorrhoids (TEH) and anal fissure (AF) are 2 frequent sources of anal pains during childbirth. We are going to define their incidences as available in publications and in our experience in Bichat hospital. Then we will define their risk factors. According to Martin's and Corby's studies, AF was observed in 10% of the delivered women. In Bichat hospital we performed a proctological assessment to 165 pregnant women during the last third of pregnancy and within the 2 months following delivery. We observed 2 AF (1,2%) during the first period and 25 (15,2%) during the second. R

Maternal and Fetal Outcomes of Pregnant Women with Hepatic Cirrhosis.

AIM: The reproductive hormone levels and systemic physiology of women with hepatic cirrhosis are altered. Existing data have indicated the adverse effects of cirrhosis on both the mother and the fetus. Pregnancy is successful in most of the patients with chronic liver disease. But maternal and fetal complication rates are still high for decompensated hepatic cirrhosis. In this study, we aimed to evaluate the clinical features, etiological factors, medications, morbidity, mortality, and obstetric outcomes of pregnant women with hepatic cirrhosis. METHODS: Pregnant women, who were diagnosed wit

Remifentanil target-controlled infusion during second stage labour in high-risk parturients: a case series.

BACKGROUND: Providing adequate analgesia and appropriate sedation to high-risk parturients during late second stage labour without compromising foetal safety remains a major challenge, especially in situations when neuraxial block is not applicable. Remifentanil emerged as an option for labour analgesia during the last decade but may be suitable for the facilitation of complicated vaginal deliveries as well. METHODS: A retrospective chart review of nine labouring women with significant medical and/or obstetrical risk factors was conducted. According to the assessment of an experienced obstetr

Head-to-perineum distance measured transperineally as a predictor of failed midcavity vacuum-assisted delivery.

BACKGROUND: During the second stage of labor, in case of a need for a fetal extraction at midcavity, the choice of attempting the procedure between operative vaginal delivery and cesarean delivery is difficult. Moreover, guidelines on this subject are not clear. OBJECTIVE: This study aimed to identify antenatal and intrapartum parameters associated with a failed midcavity vacuum-assisted delivery and its association with maternal and neonatal adverse outcomes. STUDY DESIGN: This was a single-center, retrospective, cohort study conducted at a tertiary maternity hospital in France from January

Transfer of Restrictive Episiotomy Practice with Low OASIS Rate from Tertiary to Primary Care Level-Lessons Learned and Future Challenges.

Introduction: For more than 40 years, obstetricians and midwives worldwide have continued to debate the significance of episiotomy and methods of reducing the rate of obstetric anal sphincter injuries (OASIS). Methods: A concise review of relevant recently published articles on obstetric perineal trauma was conducted, together with an analysis and comparison of the annual rates of episiotomy and OASIS (primary outcomes) at Clinical Hospital "Sveti Duh", Zagreb (tertiary care level), and Požega General County Hospital, Požega (primary care level), over a ten-year period (2015-2024). Indicators for mode of delivery in pregnant women with uteruses scarred by prior caesarean section: a retrospective study of 679 pregnant women.

BACKGROUND: The delivery mode for pregnant women with uteruses scarred by prior caesarean section (CS) is a controversial issue, even though the CS rate has risen in the past 20 years. We performed this retrospective study to identify the factors associated with preference for CS or vaginal birth after CS (VBAC). METHODS: Pregnant women (n = 679) with scarred uteruses from Moulay Ali Cherif Provincial Hospital, Rashidiya, Morocco, were enrolled. Gestational age, comorbidity, fetal position, gravidity and parity, abnormal amniotic fluid, macrosomia, placenta previa or abruptio, abnormal fetal
