[Glaucoma treatment by transscleral cyclophotocoagulation in micropulse technology in a low-income setting].

BACKGROUND: Glaucoma is the leading cause of irreversible blindness worldwide. The prevalence of glaucoma is particularly high in rural regions of Tanzania. Poverty and an inadequate ophthalmic infrastructure increase the difficulty of glaucoma treatment. OBJECTIVE: Due to the limited access to eye drops or surgical therapy, the effectiveness of transscleral cyclophotocoagulation in micropulse technology (CPC-M) in advanced glaucoma was investigated in the present study. MATERIALS AND METHODS: We included n = 50 eyes of 35 adult patients with advanced glaucoma and a glaucoma-typical papillar

A new approach for CW-TSCPC to improve its safety and efficacy in glaucoma treatment.

PURPOSE: The goal of our study was to compare the outcomes of transscleral diode cyclophotocoagulation using a new protocol with new settings. In fact, we targeted the ciliary body guided by transillumination, reduced the energy applied at each spot, and enlarged the treatment area posteriorly from the pars plicata to the pars plana. PATIENTS AND METHODS: Data were collected retrospectively from two groups of glaucoma patients. The first group of patients underwent transscleral diode laser cyclophotocoagulation as usual, with one-row applications of a maximum of 1200mW of energy and a duratio

Aggressive posterior retinopathy of prematurity: a review on current understanding.

A review of literature was performed, focused on the etiopathogenesis of aggressive posterior retinopathy of prematurity (APROP), the characteristic and atypical clinical features, management strategies, anatomical and visual outcomes. Characteristically APROP has zone I/posterior zone II involvement with prominent plus disease, featureless junction, large vascular loops, flat extra-retinal fibrovascular proliferation, and a rapidly progressive course. The risk factors for APROP are extreme prematurity (birth weight ≤1000 gram and/or gestational age ≤28 weeks), dysregulated oxygen supplementat

[Diabetic retinopathy].

Diabetic retinopathy (DR) is a vision-threatening microvascular complication of diabetes and the leading cause of blindness in working-age people. At the beginning of the metabolic disorder and in early stages of DR the patient's eyesight is often not affected. Depending on the duration of diabetes and in more advanced stages of DR the vision is compromised through the presence of diabetic macular edema (DME) and/or proliferative retinal complications. The management of DR comprises regular ophthalmic examinations according to clinical guidelines, the targeted application of multimodal imaging

Persistent Plus Disease Subsequent to Panretinal Photocoagulation in an Infant With Retinopathy of Prematurity.

Plus disease, or the presence of vascular dilation and tortuosity, is the most reliable predictor of the progression of retinopathy of prematurity (ROP), making resolution of plus disease one of the earliest signs of ROP regression. Patients with certain comorbid conditions such as anemia and cardiovascular disease may have persistent plus-like disease following successful resolution of ROP. The authors present a case of a 24-week premature infant who was treated with panretinal photocoagulation for stage 3, zone II. [Ophthalmic Surg Lasers Imaging Retina. 2019;50:520-521.].

A case report of May-Thurner syndrome induced by anterior lumbar disc herniation: Novel treatment with radiofrequency thermocoagulation.

RATIONALE: Lumbar degeneration-related May-Thurner syndrome (dMTS) is characterized by venous compression induced by degenerated lower lumbar structures. Treatment strategies for May-Thurner syndrome (MTS) include clearing the thrombus and correcting venous compression. Despite having different etiological factors from other MTS types, treatments for dMTS are similar, including endovascular angioplasty and continuous anticoagulation therapies. Thus, a particular treatment was designed herein to focus on compressive lumbar structures instead of intravenous management. PATIENT CONCERNS: A 59-ye

Refractive outcomes following the treatment of retinopathy of prematurity in the anti-VEGF era: a literature review.

A growing body of evidence indicates that antivascular endothelial growth factor (VEGF) therapy is effective in the treatment of retinopathy of prematurity (ROP). We conducted a comprehensive literature review on refractive outcomes of anti-VEGF treatments compared to laser treatment or a combination of laser therapy and anti-VEGF injections. Of the 9 studies analyzed, the final mean refractive error was myopic in 3 studies (37%) with IVB monotherapy, 7 studies (87.5%) with laser photocoagulation, and 1 study (50%) with combined therapy. In comparing IVB with laser monotherapy, 6 of 7 studies

Twin-to-Twin Transfusion Syndrome: From Observational Evidence to Randomized Controlled Trials.

Fetoscopic surgery is widely accepted as the preferred first-line treatment for twin-twin transfusion syndrome (TTTS). Nonetheless, the broad diffusion of this technique relies on a single multicentric-randomized trial. We hereby question this trial in a post-hoc Bayesian analysis, submitting its results to several scenarios comprising the alternative published non-randomized literature and pessimistic opinions regarding this surgery. Furthermore, we also discuss further refinements in indications, questioning potential alternatives in early stages of the disease.

Outcomes Disparities between Black and White Populations in the Surgical Management of Glaucoma.

African descent is a well-documented risk factor for glaucoma development, progression, and medical and surgical failure. We reviewed the literature for evidence of outcomes disparities between Black and White populations after trabeculectomy, Ex-PRESS shunt, viscocanalostomy, canaloplasy, tube shunt, laser trabeculoplasty, and cyclophotocoagulation. There are reports of decreased surgical success in Black patients after trabeculectomy, Ex-PRESS shunt, tube-shunt, and canaloplasty. At this time, there is no strong evidence that any procedure is more effective for intraocular pressure control t

Glaucoma after chemical burns and radiation.

Glaucoma after chemical burns represents a posttraumatic glaucoma, usually open-angle glaucoma. It is a frequent complication of chemical burns, especially with alkali and it can appear in the acute stage or as a late complication. Because of the complications and scars, the treatment is very difficult. Topical treatment is based on AC inhibitors, β-blockers, α2-agonists. Trabeculectomy, shunts, cyclophotocoagulation, and cryotherapy are the solutions in the late stages. Glaucoma after irradiation is a closing-angle secondary glaucoma. The risk factors such as the radiation dose and the volume

The impact of phacoemulsification cataract surgery on eyes previously treated by laser photocoagulation for diabetic retinopathy.

AIM OF STUDY: to evaluate the evolution of diabetic retinopathy (DR) after cataract surgery on eyes previously treated by photocoagulation for DR. The compensation of glycaemia (glycosylated haemoglobin, HbA1c) and prevalence of attendant diseases (hypertension, ischemic heart disease and alteration of lipid metabolisms) were evaluated. METHODS: Forty eyes (40 patients) were evaluated, of which ten (25 %) eyes with DR were treated without laser photocoagulation (LK), and 30 (75 %) eyes were previously treated by LK. Two criteria for evaluating the stabilisation were used, particularly the bes

Management of radiation proctitis.

Radiation damage to the rectum following radiotherapy for pelvic malignancies can range from acute dose-limiting side effects to major morbidity affecting health-related quality of life. No standard guidelines exist for diagnosis and management of radiation proctitis. This article reviews the definitions, staging, and clinical features of radiation proctitis, and summarizes the modalities available for the treatment of acute and chronic radiation proctitis. Because of the paucity of well-controlled, blinded, randomized studies, it is not possible to fully assess the comparative efficacy of the

Twin-twin transfusion syndrome.

Twin-twin transfusion syndrome (TTTS) is a serious condition that affects 10% to 15% of twin pregnancies with monochorionic diamniotic placentation. The pathophysiology of TTTS is not completely understood; however, the presence of unbalanced placental vascular communications within a shared circulation has been implicated in its development. The presentation of TTTS is highly variable, and it does not always progress in a predictable manner. Monochorionic twin gestations should, therefore, be monitored for signs of TTTS with serial sonograms starting in the second trimester. Early TTTS can be
