Cleft lip and cleft palate affect about one in every 700 children born in the United States. Parents of newborns with orofacial clefts, often unaware of the cleft until birth, need emotional support and professional guidance in caring for their baby. As the child grows, it is important to properly time surgical procedures, speech therapy, hearing evaluations, and dental interventions and to assess the child's psychological status. Multidisciplinary cleft palate teams provide a significant resource to families of children with facial clefts. This article describes the role of team members, prov…

Xeroderma pigmentosum (XP) is a rare, usually autosomal recessive disorder related to DNA repair defects. Atypical fibroxanthoma (AFX) is a pleomorphic tumour that occurs infrequently on the limbs and trunk in children. We report a child with XP who presented with AFX of the facial skin and the lower lip. The diagnosis of AFX was confirmed using histological and immunohistochemical techniques. We discuss the possibility that ultraviolet-induced damage might be implicated in the pathogenesis of AFX.

The bilateral cleft anomaly is difficult to correct and camouflage because of the double lack of many important landmarks and the shortness of skin in the midvertical plane. A possible solution in patients who have some columella or in those of races not needing a long columella is the strap flap advancement of the nostril sills and alar bases. In all other cases, the forked flap is the method of choice for adequate correction and camouflage of the bilateral cleft lip-nose deformity.

In the last 40 years, great progress has been made toward a better understanding of many aspects of the cleft lip and palate defect, but there is still a long way to go before there is agreement on the optimal treatment procedures. With regard to the primary operations, it can be stated, in a somewhat simplified form, that there are two main schools of thought in cleft treatment. One advocates early closure of the lip and palate, a procedure which imparts a high priority to early speech function. The other recommends delayed closure of the hard palate, thereby according a high priority to the …

This case describes a repair of a unilateral cleft lip complicated by a lip hemangioma. The relevant issues regarding timing of the repair include safety of the procedure by minimizing blood loss during excision of the hemangioma and minimizing anesthesia risk. In obtaining a lasting cosmetic result, excision of the hemangioma would need to be done when the potential for residual tumor growth would be least likely (during tumor involution). Finally, the difficult issue of psychosocial concerns would balance the need for traditional early cleft repair with a definitive repair prior to increased…

From December 1965 to December 1987 (22 years), 279 patients with histologically confirmed squamous cell carcinoma of the lip were treated in this Department with roentgenotherapy. All the patients that had completed the treatment were evaluated with a follow-up time of 5 years (n=255). Distribution by age, sex, occupation, residence, evolution time of the lesion, localization (upper lip, lower lip and commissure), tumoral size and histological grade were analysed. Concerning technical conditions and fractionation, we evaluated the type and which dose of radiation elicit acute reactions as wel…

Previous studies have found that children with cleft lip and palate (CLP) and isolated cleft palate (CP) have elevated risk for a variety of psychosocial problems, but the origins of such problems are unclear. We expected that early medical and other stressors during infancy--including feeding problems and facial disfigurement--would have adverse effects on the infant, his or her caregivers, and the family environment, leading to a higher than expected rate of insecure attachments among infants with clefts. Twelve-month attachment classifications of CLP, CP, and comparison group infants were e…

Clinical and epidemiologic studies of defined geographic populations can serve as a means of establishing data important for genetic counseling and as a first step in identifying strategies best suited for identification of causes. Under the sponsorship of Operation Smile International, clinical, genetic, and epidemiologic studies were carried out at six sites within the Philippines between 1989 and 1996. Patients who were being evaluated for surgical repair of craniofacial anomalies (primarily clefts of the lip and palate) were briefly examined for the presence of associated anomalies, and a …

The management of cleft lip and palate patients should achieve good function, particularly concerning nasal breathing. For this purpose, we must have a sound knowledge of the pathological anatomy, to correct the deformity of the cartilaginous structures, and to restore a good muscular balance. However, surgery is the main cause of facial growth disturbances. Procedures which produce areas of denuded bone are the worst and the VY Veau-Wardill closure of the palate was abandoned for this reason. Since 1981, the new protocol consists of simultaneous repair of the lip, nostril and soft palate at 6…

Squamous cell carcinomas of the lip can be treated by surgery or radiation therapy. Since local control rates exceed 90% with both treatment modalities, therapy is chosen according to functional and cosmetic results, to the patient's wishes and to the physician's medical policy. This study was aimed at retrospectively reviewing our local control results and patterns of failure relative to survival as well as evaluating the results according to technical surgical variables. From 1982 to 1991, sixty-nine patients with squamous cell carcinoma of the lip received a course of interstitial brachythe…

The aims of this study were to determine the frequency of hypodontia of permanent teeth in twins discordant or concordant for clefts in each … f hypodontia was 37% for monozygotic and 32.7% for dizygotic twins, 16.7% in the noncleft and cleft lip groups, 41.1% in the cleft palate group, and 64.3% in the cleft lip and palate group. All these were above the values observed earlier in the noncleft twins and in the Finnish normal and cleft group populations. Four of 13 pairs of monozygotic twins …

Median cleft lip is a midline vertical cleft through the upper lip in the absence of a prolabial remnant. This may occur as a sporadic event or be part of an inherited sequence of anomalies. A failure of formation or fusion of the medial nasal prominences derived from the frontonasal prominence is ultimately responsible for this aberration. Two categories of dysplasia are associated: (1) frontonasal deformity associated with hypotelorism and (2) median facial cleft syndrome associated with hypertelorism. A patient presents with median cleft lip, mild bifid nose, and hypertelorism. Following su…

Alongside orthodontic treatment, tooth shape modification may be indicated for teeth associated with clefts of the lip and alveolus, and in non-carious teeth, minimal tooth preparation techniques appear to be most appropriate. Two methods of tooth shape modification are described. In one, hybrid composite materials, bonded to tooth enamel using the acid-etch technique, may be considered to provide a permanent, low cost, aesthetically satisfactory result, often without tooth preparation and with minimal operating time. Alternatively, indirect veneers may be appropriate for treatment of more sev…

A classification designed to be used as a simple and reproducible method to evaluate the residual lip deformities following unilateral cleft lip repair is presented. It was developed by analysing the appearance of the upper lips of 81 patients referred to the author with repaired unilateral clefts. The assessment of each lip was divided into 3 separate components: the vermilion (V), the lip itself (L), and the scar (S), and the resulting classification was termed the V.L.S. classification.

A short review of the literature is presented outlining the role of alveolar bone grafting in cleft patients. An analysis of a series of 47 secondary alveolar bone grafts in 34 patients is presented. The age range was 7-24 years. At the time of evaluation, 30 canines were in final occlusion and good alveolar bone levels were present in 83% of patients. Of the 13 bilateral cases (26 clefts), 14 canines were in final occlusion and all cases had good alveolar bone levels. No major complications were encountered. The graft was completely resorbed in 3 cases (6.4%) only.

A cleft palate case is reported in which there is an unusual pattern of hypodontia involving aplasia of the primary second molars, but development of the corresponding second premolars. This paper discusses some of the implications of this situation and stresses the need for very thorough clinical and radiographic examination.
